    本研究の目的は、日本の超高齢社会において、高齢者を地域で支援するための看護力の強化に繋がり看護職者のメンタルヘルス対策となり得る「バーチャルコミュニティで展開する対人スキル小集団プログラム」の実用化に向けて、プログラム提供の持続可能なシステムを構築することである。特に高齢の潜在看護職者がアドバイザーとして活躍し続けるシステムを構築することを目指している。具体的には、対人コミュニケーションの訓練効果が実証されているSSTの基本訓練モデル(認知行動療法的援助法である小集団訓練)の枠組みを使用し、アドバイスの基本理論となるアサーション技法のDESC(Describe, Express, Specify, Consequences)法やSFA(Solution-Focused approach)を活用したアドバイザーのためのマニュアルを作成し、システムに対して利便性、経済性、必要性、重要性、効果、肯定的感情から評価し、実用化と持続可能なシステムを提案することにある。 今年度は、システムを試行して、本稼働へ向けての課題の明確化とホームページの修正を行うことを中心に取り組んだ。昨年度の「実用化に向けて、相談内容の多彩さに対応できるように分類内容を充実させる」については、分類の多さが逆にアドバイザー側の対応の煩雑さに繋がるというデメリットを生じさせることが明確になり、更なる検討が求められた。ホームページについては、システムの試行を繰り返すことで、文言など詳細に検討し、修正箇所を明確にした。

    本研究の目的は,患者の望む生き方を支えるための看護師の対話力の獲得過程および対話力の確定に影響する要因を明かにすることである.研究実施計画では,面接調査の実施後,対話力を測定できる尺度を開発する予定となっている. 今年度は,病棟で終末期がん患者の看護に携わる看護師に対して,終末期がん患者との対話の実態と対話力の獲得過程,およびその影響要因について面接調査を実施する予定であった.しかし,昨年度に引き続き,COVID-19の影響を受け,研究フィールドである病院に大規模な面接調査を依頼することはできなかった.そのため,リクルート方法を変更した.また,面接は感染の危険性を回避するために,対面面接からオンラインによる面接に方法を変更した.現在までに,3名の看護師と面接を行い,逐語録を作成してデータとし,「終末期がん患者との対話の内容」「対話力の獲得過程」「対話力の獲得に影響を与えたもの」について質的な分析を進めているところである.次年度は,引き続き病棟看護師の対話力に関する面接調査を継続するとともに,研究実施計画に沿って,訪問看護師を対象とした面接調査にも着手する予定であり,現在研究倫理審査の申請の準備を進めているところである. また,昨年度実施した訪問看護師が終末期がん患者および家族に行っているコミュニケーション支援に関する質問紙調査による研究は,分析を進めているところであり,次年度中に論文投稿する予定である.

    The aim of this study was to develop communication skills for everyday life activities which will provide positive stimulation for the elderly with dementia, by analyzing positive emotions and feelings from speech data. After questionnaire and interview surveys of staff in psychiatric hospitals, simulated scenes were set and speech data of healthy adults in these situations were collected and analyzed. From the results, it was considered that it is possible for care providers to conduct communication providing pleasant stimuli to elderly persons with dementia in care situations even for a short time of period. It was confirmed that positive feelings arose from situations of greetings and choosing, and also in discovery and achievement after trials and errors. The results also suggest that expressions and behaviors that cannot be captured by speech are related to feelings of pleasure.

    The purpose of this study was to evaluate nursing care for parents with schizophrenia through the use of outpatient service based on a community network. A nurse and a PSW working in a mental clinic employed the Positive Parenting Program (triple P) for parents using the mental clinic. Triple P was utilized individually and for group support of the parents and improved their rearing attitude and reduced stress. A ripple effect on interpersonal relationships and self-help activity was also found. For parents with schizophrenia, it was the foundation of support for their power of assertion and to build relationships with their supporters, including their families. Individual consultation and home visiting by psychiatric nurses based on an active cooperative network with various facilities was useful. Nursing care including a parenting program for parents with schizophrenia seemed to be effective based on the good relationship among the patients, their families, and the community network.

    Nurses are expected to have higher stress management abilities to tackle major social and medical changes in the future. It is reported, however, that nurses in our country are under high stress conditions, especially interpersonal stress. In this study, we developed an interpersonal skill small group training program as the support method to promote interpersonal stress management among nurses. The program is specialized for nurses and is executed via the Internet. We clarified the evaluation and future challenges of the program through 17 trial results. Being responsive to irregular working styles and taking advantage of the positive characteristics of nurses, the program is proven to be effective and to have high potential.

    This study was carried out for the ultimate purpose of developing a nursing care guideline for schizophrenic mothers during their pregnancy, delivery and parenting. It involved field work at pioneering facilities ; a questionnaire-based descriptive study of nurses from 128 health and medical institutions ; interviews with 24 nurses ; and interviews with two schizophrenic mothers. The study identified aggravated symptoms resulting from reduced medication during pregnancy ; fragile family background and poor interpersonal skills as specific needs of schizophrenic mothers, as well as the need to provide, on an ongoing basis, a multi-faceted and -tiered support to the mother and her family from early in pregnancy to ensure their smooth transition into the parenting life. There was also a lack of collaboration among nurses. A care guideline was developed on the basis of these findings to support schizophrenic mothers from pre-pregnancy through to parenting stages. The guideline, which has been highly evaluated in clinical practice, is expected to guide the nurses to forming a common ground of understanding in the care of schizophrenic mothers.

    This study attempts to clarify the difference in life strategies of those elderly women who contrive to maintain their own health care. Two types of life-styles of elderly women, (a) living at home by themselves and, (b) living with family although in the daytime she is left alone-were analyzed by two methods, i) direct interviews and, ii) observing participants inductively. These analysis show that elderly women living at home cared for by the visiting nurses tend to play the role of the main person on their own lives and also to maintain their selfidentity, which can not be observed among elderly women living in the old-age nursery institute. Women living at home with family showed as much intimate feeling toward visiting nurses as toward family members, mainly in an effort to smooth their health care, which is named "parafamiliarized action". And as to the relations between mother and child, women living with family showed the behaviors to reproduce "the mother and infant relationships".
