    Abstract Background Screening and intervention in pre-frailty can help prevent or delay frailty among older adults. Being overweight has shown associated with pre-frailty, and overweight is highly prevalent among community-dwelling older adults during COVID-19. However, the impact of visceral fat accumulation remains unclear. This study aimed to explore the association between visceral fat area and pre-frailty in community-dwelling older adults. Methods The participants of this study included community-dwelling older adults from three elderly welfare centers. The frailty phenotype was assessed using the frailty screening index. The body composition was measured using bioelectrical impedance analysis. Results A total of 214 community-dwelling older adults completed the questionnaire and measurements. After excluding 16 frail participants, 149 (75.3%) were pre-frailty. The mean age of participants was 75.4 ± 5.4 years, and 69.7% (138) of participants were women. There were 54 (27.3%) participants with high visceral fat area. The multivariable model showed that participants with high visceral fat area were at increased risk for pre-frailty (adjusted OR, 3.15; 95% CI, 1.26 − 7.87; P = 0.014), even after adjusted for age, sex, health status, and impact of COVID-19 pandemic. Conclusions This study suggests that the association between visceral fat accumulation and pre-frailty may help to identify a new target for prevention. Further longitudinal studies are needed to determine their mechanisms in older adults.

    AIM: To analyse the interactions of associated factors with post stroke fatigue (PSF) after discharge home and determine the predictors of PSF and their impact on stroke survivors. DESIGN: A prospective observational study. METHODS: A total of 94 patients with acute stroke were recruited between May 2019 -July 2020. The main outcomes were fatigue, depression, insomnia, sarcopenia, and health-related quality of life (HRQOL) and were assessed at admission and 1 month after discharge. Fatigue was measured using the Fatigue Assessment Scale. Depression and Insomnia were assessed using the Hospital Anxiety and Depression Scale-Depression and Insomnia Severity Index, respectively. Sarcopenia was measured using the SARC-F questionnaire, and HRQOL was assessed using the Short Form-8. RESULTS: Acute phase PSF was an independent predictor of PSF after discharge home. Moreover the path analysis revealed that this effect is mediated through both the direct effect of acute-phase PSF on PSF after discharge home and through the indirect effect of interaction with pre-stroke SARC-F, acute phase depression, and acute phase insomnia, which remains a separate predictor of acute-phase PSF. In total, 17% of the survivors had persistent PSF. Persistent PSF was significantly associated with depression, insomnia, sarcopenia, and a lower quality of life scores. CONCLUSIONS: Post-stroke fatigue may occur in the acute phase and persists after discharge, it will not only affect later depression, insomnia, and quality of life, but also sarcopenia. IMPACT: Acute phase PSF was found to be an independent predictor of PSF after discharge home. In addition, the interaction with pre-stroke SARC-F, acute phase depression and insomnia had an indirect connection with PSF after discharge home, which remains a separate predictor of acute-phase PSF. Thus, early assessment and management of mental status, sleep problems, and sarcopenia during hospitalization might be an important step in post-stroke rehabilitation and home transition.

    Most studies have associated denture wearing with a higher risk of malnutrition and lower nutritional intake in the elderly. As the effect of wearing dentures remains debatable, the aim of this study was to determine if older adults wearing dentures have a higher risk of malnutrition. We recruited 294 Japanese community-dwelling adults aged ≥65 years from two elderly welfare centers in Sapporo. The Mini Nutritional Assessment-Short Form (MNA-SF) was used to assess their nutritional status. Their demographic characteristics, eating habits, and health status were assessed using a questionnaire. Moreover, salt-impregnated taste paper and bioelectrical impedance analysis were used to obtain salt taste sensitivity and body composition (body mass index and protein mass). The mean age of older adults was 75.8 ± 5.8 years and 71.1% were women. We found that 74.1% of the older adults wore dentures. The proportion of the whole population with a risk of malnutrition was 23.5%. Multivariate analysis revealed that partial denture wearing could be protective against the risk of malnutrition. In this study, we found more partial denture wearers with normal nutritional conditions and cooking by themselves than non-denture-wearing older adults. Furthermore, complete denture wearers were eating significantly less meats or fish every day. This study suggests that using partial dentures to replace the lost teeth to meet the dietary needs of the elderly is critical to maintaining a healthy nutritional status. Moreover, it also indicates the importance of providing education to older adults who wear complete dentures.

    Background: Few studies have investigated the relationship between living arrangements and dietary intake among evacuees after disasters. Objectives: To examine the relationship between living arrangements and dietary intake using the data of a large-scale cohort survey of evacuees after the Great East Japan Earthquake in 2011. Methods: 73,433 residents in evacuation zones responded to the Fukushima Health Management Survey questionnaire. Subjects were excluded if they did not report their living conditions or were missing more than three pieces of information about dietary intake. The data of 52,314 subjects (23,149 men and 29,165 women >= 15 years old) were used for the analyses. Evacuees' living arrangements were characterized into three categories: evacuation shelters or temporary housing, rental houses or apartments, or a relative's home or their own home. Dietary intake was characterized in terms of grains, fruits and vegetables, meat, soybean products, dairy products, and fish. Daily consumption of the third quartile (Q3) or higher for each food group was defined as `high consumption'. Prevalence ratios (PRs) and 95% confidence intervals (CIs) were estimated using modified Poisson regression analyses. Results: Modified Poisson regression analyses showed that, compared with respondents living in a relative's home or their own home, the PRs and 95% CIs for the people living in rental apartments of high consumption of fruits and vegetables (non-juice), meat, soybean products, and dairy products were 0.69 (95% CI, 0.61e-0.77), 0.82 (95% CI, 0.73-0.91), 0.89 (95% CI, 0.83-0.94), and 0.83 (95% CI, 0.74-0.93) respectively. The corresponding PRs and 95% CIs for people living in evacuation shelters or temporary housing were 0.83 (95% CI, 0.78-0.88), 0.90 (95% CI, 0.86-0.95), 0.94 (95% CI, 0.91-0.97), and 0.91 (95% CI, 0.86-0.96) for high consumption of fruits and vegetables (non-juice), meat, soybean products, and dairy products, respectively. Conclusion: The present study suggests that, after the earthquake, living in non-home conditions was associated with poor dietary intake of fruits and vegetables (non-juice), meat, soybean products, and dairy products and suggesting the need for early improvements in the provision of balanced meals among evacuees living in non-home conditions (S) 2016 The Authors Publishing services by Elsevier B.V. on behalf of the japan Epidermiological Association

    Objectives: Prevalence of life-style disease has increased dramatically in evacuees due to the Great East Japan Earthquake. One reason may be that physical activity level decreased from life environment changes due to evacuation. However, associations between evacuation condition and habitual physical activity have not been studied. We examined this association in Fukushima residents who participated in the Fukushima Health Management Survey. Methods: In this study, 37,843 evacuees from 13 municipal evacuation zones from the nuclear-power accident caused by the Great East Japan Earthquake, born before April 1, 1995, were included in the analysis. Evacuation condition was defined by disaster living place (13 zones), evacuation place (inside or outside the prefecture), and current living status (evacuation shelter or temporary housing, rental housing/ apartment, and relative's home or own home). Habitual physical activity was defined from self-administered questionnaires as participants who responded "almost every day" and "2-4 times/week" of regular exercise. In the analysis, habitual physical activity prevalence was aggregated by gender and variables (living place in the disaster, evacuation place, and current living status). Prevalence was adjusted for age, disaster living place, evacuation place, and current living status by standard analysis of covariance methods. Results: Adjusted prevalences of habitual physical activity were: men, 27.9-46.5%; women, 27.0-43.7% in each disaster living place. The differences were 18.6% point in men and 16.7% point in women. For evacuation place, physical activity outside the prefecture for men (37.7%) and inside the prefecture for women (32.1%) were higher, but those differences were only 2.2% point and 1.8% point in men and women, respectively. For current living status, physical activity of those in rental housing/ apartment was the lowest; evacuation shelter or temporary housing was the highest in both genders (men: 38.9%, women: 36.7%). Compared with residents in evacuation shelter or temporary housing, those in rental housing/apartment were 5.4% point and 7.1% point lower and those in relative's home or own home were 2.0% point and 4.2% point lower in men and women. Conclusion: Habitual physical activity in residents who lived in 13 municipal evacuation zone differed by disaster living place and current living status, while it was similar regardless of placement in the prefecture. In particular, prevalence was the lowest in participants who lived in rental housing/ apartment. We need to plan and perform additional life-style disease prevention strategies for participants who become isolated.

    Purpose/Objectives: To measure the total amount of cyclophosphamide (CPA) excreted in the urine of patients with cancer and their cohabitating family members seven days after CPA administration. Design: Biological monitoring. Setting: Home setting with outpatients receiving chemotherapy. Sample: 8 patients administered CPA, 10 cohabitating family members, and 10 control participants. Methods: During the first seven days after CPA administration, urine samples were collected from the participants. The samples were analyzed for the unchanged form of CPA using gas chromatography in tandem with mass spectroscopy. Main Research Variables: CPA levels. Findings: CPA was detected in 112 of 276 patient urine samples. The last sample containing detectable CPA levels was collected after more than 48 hours in 63% of the patients, with a maximum length of five days post-treatment. In addition, 243 urine samples were collected from family members, and CPA was detected in the samples of five family members (17-252 ng per member). CPA was not detected in any control participants. Conclusions: These findings indicate that family members in close contact with patients receiving CPA are at high risk for drug exposure as many as seven days post-treatment. Implications for Nursing: Nurses should educate patients and their family members about preventing exposure to antineoplastic drugs in the home setting.

    【Purpose】 The aim of this study was to investigate the social activities and health-related quality of life (KDQOL-SFTM) of community-dwelling diabetic nephropathy patients undergoing dialysis.【Subjects and methods】The subjects comprised 75 diabetic nephropathy patients (aged≥40 years) who were undergoing dialysis at one of the three medical institutions in A prefecture. The data were obtained from their medical records and an interview survey using a questionnaire. Social activities were composed of three subscales ([personal activities],[interaction with close friends]and[participation in group activities]).The relationship between health-related quality of life for the implementation status and willingness of the subject to undertake social activities was examined.【Results】There were associations of quality of life with[personal activities],[interaction with close friends]and[participation in group activities]. Patients who engaged in social activities at least once per week on non-dialysis days had higher health-related QOL than those who did not.【Conclusions】Health-related QOL was higher in patients who engaged in social activities on non-dialysis days, indicating the importance of encouraging patients to being willing to participate in and continue with social activities even when their daily lifestyle is restricted by regular outpatient treatment.

    Aim: The purpose of this study was to investigate changes over one year in the prevalence of kyphosis, the ability to perform activities of daily living and self-efficacy among elderly residents of mountainous areas. Methods: The subjects included 155 elderly residents of mountainous areas (68 males, 87 females) aged 65 or older who participated in both our initial and follow-up investigations. The investigations were conducted via interviews using questionnaires and the index of kyphosis (I/K). The subjects were divided into three groups (non-kyphosis, kyphosis without changes, kyphosis with worsening) according to the change in I/K. Results: The average patient age was 74.9±6.2 years. The average I/K on the initial investigation was 10.0±3.7 (2.4-20.0). On the follow-up investigation, the average I/K increased slightly to 10.2±3.6 (3.0-22.9). A particularly large number of females 65 to 69 years of age exhibited an increase in the I/K. The percentage of subjects with an I/K of 13.0 or above was 20.6%. The male kyphosis sufferers demonstrated a significant decrease in the health-related QOL categories of "able to fulfill everyday life roles (physical)" and "able to fulfill everyday life roles (mental)." There were no significant differences between the females with and without kyphosis. One year later, there were significant differences between the three groups in terms of the health-related QOL categories of "vitality" and "ability to live in society." Conclusions: Female kyphosis sufferers are affected by psychological factors. Kyphosis should be detected early and carefully followed up in elderly patients in order to prevent the need for nursing care, and appropriately estimating the patient's physical condition is required to prevent a decline in the lower limb function.

    Purposes: (a) Tomeasure the urinary excretion of antineoplastic drugs of three patients during 48 h after the administrationof cyclophosphamide (two patients) and 5-fluorouracil (one patient). (b) To evaluate environmental contamination withantineoplastic drugs via excreta of patients in the home setting. (c) To evaluate exposure of family members to antineoplasticdrugs by measuring the drugs in their urine during the 48 h after completion of the chemotherapy by the patients.Methods: Two patients were administered cyclophosphamide by i.v. bolus injection. One patient was administered5- fluorouracil by i.v. bolus injection and thereafter immediately administered the same drug by continuous infusion for46 h. Urine samples from the patients administered cyclophosphamide and their family members, and wipe samples fromtheir home environment, were analysed for the unchanged form of cyclophosphamide. For 5-fluorouracil, the urine samplesfrom the patient and the family member were analysed for the 5-fluorouracil metabolite α-fluoro-β-alanine. Wipe sampleswere analysed for 5-fluorouracil. Drugs were detected and quantified with gas chromatography in tandem with massspectroscopy-mass spectroscopy or by high-performance liquid chromatography with ultraviolet-light detection. Results: A total of 35 and 16 urine samples were collected from the three patients and their family members, respectively.The drugs were detected in all samples.Cyclophosphamide was detected at levels of 0.037.34 ng/cm2 in 8 of the 12 wipe samples obtained from the homes ofthe patients administered cyclophosphamide. For the patient administered 5-fluorouracil, drug levels in his home environmentwere below the limit of detection.Conclusion: We demonstrated contamination of the home setting and exposure of family members to cyclophosphamidevia the excreta of outpatient receiving chemotherapy. Exposure of the family member of the patient administered5-fluorouracil was also demonstrated. These findings indicate the importance of strict precautions by the members oftreated cancer patients as well as healthcare workers, to reduce the risk of exposure to antineoplastic drugs. © 2012 The Author(s).

    東日本大震災では宮城県のほぼ全域で停電し,多くの在宅酸素療法(HOT)患者が酸素吸入の困難な状況に陥った.今後の災害発生時の適切な対応を検討するため,震災発生時におけるHOT患者の行動とその問題点を調査した.今回の大災害では電話による安否確認が困難であったため,本人または家族の直接来院や,HOTプロバイダーからの情報によって安否確認できたケースが多かった.早期に安否確認できたケースに酸素吸入の中断はなかったが,震災後に酸素供給に関する行動を起こさず,安否確認の遅れがある患者に,酸素中断の傾向がみられた.酸素吸入中断患者の一部は状態悪化につながり入院となった.停電や災害時にどう対応するべきなのかを,家族,病院およびHOTプロバイダーとの間で取り決めておくことが重要であり,特に酸素中断のハイリスク患者においては個別のアクションプランを設定する必要がある.

    目的:地区活動に参加している後期高齢者について,前期高齢者と比較検討し,ソーシャルサポートの現状と将来の介護ニーズに関する特性を把握し,地域における課題を明らかにする.方法:老人クラブ等に定期的に参加している高齢者152人を対象に,質問紙を用いた聞き取り調査を実施した.結果:前期,後期高齢者の2群で集計・分析を行った.その結果,後期高齢者のソーシャルサポートは「配偶者以外の同居家族」から受け取るサポートが「別居子・親族」「友人・近隣」より高い割合を示した.また,サポートの種類によりサポート提供者は限定的な傾向がみられた.将来の介護ニーズについては,地域に必要なものとして「状態悪化時の施設・病院」「同年代との交流」等が挙げられ,介護希望場所は「自分の家」が最も多かった.結論:後期高齢者にとって,家族のサポート源としての役割は大きく,手段的サポート,情緒的サポートの主な提供者であることが示された.また,友人や近隣との交流がサポートを得やすい環境づくりにつながることが示唆された.後期高齢者であっても前期高齢者とほぼ変わらない社会的支援の程度が維持されると推察された.後期高齢者にとって活動の場をもつことは,他者との交流によりサポートを受ける可能性を高め,そこで役割を担うことで心身の保持・増進意欲を高めることにつながる.

    The purpose of this study was to conduct a two-year program for the prevention of falling targeted at the elderly residing in rural areas and evaluate psychological and physical change among the elderly in two years. The following findings were obtained as a result of this study; 1) The subjects were consisted of 17males (40.5 %) and 25 females (59.5 %). 2) The number of persons who had experienced falling during the past year decreased to 13 persons (31.0 %) in 2000 and to 11 persons (26.2 %) in 2002. Among the 13 persons who experienced falling in 2000, there were 8 persons (61.5 %) who had not fallen (improvement) and 5 persons (38.5 %) who had fallen (recurrence of falling) in 2002. In addition, among the 23 persons who did not experience falling in 2000, 6 persons (26.1 %) experience falling for the first time in 2002. 3) Factors relating to the recurrence of repeated falling consisted of the four parameters Timed Up and Go test and functional reach.

    The purpose of this study was to evaluate the associations between subjective sleep disturbance, healthy lifestyle, and mental health among 180 elderly people in the community. The following findings were obtained; 1) 29.3% male and 60.2% of female reported that subjective sleep disturbance. There was a significant gender difference, with prevalence of subjective sleep disturbance being higher in females than males. 2) The factors related to subjective sleep disturbance in males included age and depression, but no factors were found for the female group. This suggested that we should carefully evaluate subjective sleep disturbance and need to evaluate associations with other factors.

    脳卒中後病的疲労(Post Stroke Fatigue:PSF)は発症率が高く、機能回復や社会生活復帰の阻害要因となり、さらにフレイルや寝たきりをもたらす重篤な後遺症の一つである。PSFの医学的判断は本人の主観的疲労と日常生活行動から評価する困難さががある。本研究計画は、今年度はこの主観的疲労を測定する尺度についてレビューした。 データベースの一つであるPubMedを用いて、keywordを”Post-stroke fatigue”として、タイトルまたは抄録に述べている研究141のうち、さらにoutcomeとしてPSFを測している研究は75件であった。最も多く用いられていた尺度は Fatigue Severity Scale(FSS)39件であり、次いで Fatigue Assessment Scale(FAS)17件、Multidimentional Fatigue Inventory (MFI)15件であった。 FSSはPSFを測定する尺度として広く知られ使用されているが、この尺度は脳卒中患者を対象として開発された尺度ではない。FSSは疲労の重症度や日常生活行動への影響に焦点があり、PSFを測定するには表面妥当性も十分ではないことが指摘されている。また、FASはよく用いられるもう一つの尺度であり、Fatigueにおける精神的側面と身体的側面の両側面が含まれている。本尺度はテストー再テスト法は十分な結果とされる一方、内的一貫性が低いことが指摘されている。 以上のことから、PSFの特徴を適切に測定する尺度は現時点で十分なものは存在せず、これらの限界と課題はPSFの独自とするFatigueの測定への影響と患者ケアに影響している可能性がある。 次年度からの本研究計画に、上記を結果を反映させて展開する。

    清拭は、対象者の清潔を保持し、温熱刺激等による気持ちよさをもたらす看護ケアである。清拭時間が同じであるならば、誰もが質の高いケアを求めることは当然のことである。しかし、皮膚が脆弱で、スキンテア発生リスクが高い高齢者において、タオル素材に関わらず、どのように清拭すると(清拭圧、回数、温タオル貼用など)、安全で、気持ちよさを提供できるか、清拭方法のエビデンスが十分とは言えず、清拭ケアプログラムは確立されていない。私たちは、気持ちよく、かつ皮膚バリア機能を保持する清拭ケアを目指して、これまで清拭方法に着目して基礎的研究を進めてきた。本研究ではこれまでの成果を基盤とし、追研究を行うとともに、高齢者における安全で、気持ちよさをもたらす清拭ケアプログラムを開発し、その効果を検証する。 2021年度は、高齢入院患者約50名を対象に 弱圧と通常圧による3回の清拭を行う単盲検ランダム化クロスオーバー 試験を実施した。皮膚バリア機能(経表皮水分蒸散量TEWL・角質水分量SCH)の 経時的変化と主観的評価を比較した。その結果、弱圧でも3回の拭き取りで十分に汚れを除去できるため皮膚を強く拭く必要性はないことを可視化した。 また、清拭前の 皮膚乾燥度を簡易的に測定するために、Overall dry skin score ODS 日本語版の信頼性および妥当性を検証し、ODS日本語版を開発した。さらに、これまで清拭における主観的効果は十分に可視化されていないことに着眼し、清潔ケアを提供する看護師の認識から清拭を含む清潔ケアの意味を記述し、構造化した。清拭の効果を生理学的視点から可視化することに加えて、心理・社会的効果を検証することにつながる研究を進めた。

    In Japan, stroke patients account for about 70% of all patients at home care setting. Nursing care to prevent patients at home from falling into a bedridden state is the most important and essential care for the improvement of QOL of patients. Though a large number of studies on standing balance and/or walking ability have been reported, they are few studies on a positional motion of "getting up from supine position and maintaining sitting position on the edge of the bed". Moreover, relatively large analytical devices used traditionally were difficult to apply to the practical site of home care. We conducted this study to develop a quantitative method to evaluated the efficacy of nursing intervention to prevent patients from falling into a bedridden state at home care site, and to verify its usefulness at care setting. The study had two stage:1) A device to measure the change in position that can be used at the site of homecare was developed. In order to verify the reliability and validity of this device experiment was performed where the following two motions(test motions) were measured and compared between normal subjects and mimic patients who emulated hemialgia.a) to maintain sitting position on the edge of the bed, and b) to stand up from sitting position on a wheelchair. 2) The test motions were measured at two points (in the interval of about one month) in stroke center. 1) Reliability and validity of a device to measure the change in position consisted of a piezoelectric vibrating gyro, amplifier and PC: Changes in the angle, angular speed and power were measured in two female subjects, and the results revealed significant differences in the angle, mean angular speed, power and motion time as compared with those in normal subjects. 2) The test motions were measured in 12 stroke patients, and longitudinal changes in the angle and angular speed in each case were determined, which revealed changes in position specific for cerebrovascular disease. Results of the basic and clinical experiments demonstrated that the measuring device developed in this study was effective for quantitative and visual determination of changes in position. It was also indicated that its applicability in the site of home care would allow visual presentation of the effect of training to stroke patients, their family and care providers at home, which may serve as the indicator for motivating the patients and setting the target of care for care providers.

    The purpose of this study was to compare and contrast services for demented elderly in communities of the elderly and of the young. We surveyed early intervention systems in the community and actual state of health, dementia and family caregivers. We compared the average ages of 134 people from an elderly community (25.8% elderly) and 137 people from a young community (8.9% elderly). There was no significant difference in age - 73.3 years and 74.6 years. Three-generation households were seen most in the elderly community and elderly couples in the young community. We found a high level of subjective satisfaction with life and health outlook in 80-90% of the elderly in both communities. The ability to carry on daily life activities was high in both communities, with no differences due to gender or household composition. We confirmed deterioration in those over age 85, but there were few cases of possible dementia. We analyzed the type of home care support available to 5 demented elderly in the elderly community and 6 in the young community. Care centered around the home helper in the elderly community, whereas family caregivers would pull together a care system themselves in the young community. This study suggests there is a need for understanding dementia and how to deal with it in all communities. We must find ways of supporting the demented elderly and their families in a way that will take root easily in all types of communities.

    This research aims at developing an acute care model for elderly patients hospitalized with delirium-like symptoms. The research was carried out in cooperation with nurses in a private general hospital. In the 1996 academic year, we summarized the results of a study (from the 1995 academic year) on the actual conditions of patients with delirium in a private general hospital. We analyzed the primary factors in the appearance and disappearance of symptoms of delirium. We also conducted a questionnaire survey of 160 nurses on their perception of delirium and the care they are delivering. This survey showed that important factors in the care of patients with delirium include securing safety, communication, observation, stimulation and the rhythm of daily life. The results were presented at the 1^ Scientific Meeting of the Japan Academy of Gerontological Nursing (January, 1996). In the 1997 academic year, we examined the literature on the assessment of and care plans for delirium. We considered the content and structure of a care model based on our studies. We developed "a Proposal for a Care Model of Delirium"to clarify how to determine the pressence of delirium, and how to deal with and distinguish the causes of delirium. This proposal includes a delirium rating scale, information on causes, and guidelines for acute and preventive care. The model is intended as a guideline for nurses in a clinical setting for making decisions and taking action. In order to increase the content validity of the care model, we analyzed case studies in which the model was actually used, and obtained evaluations of the model from specialists and clinical nurses. We revised the model based on the above results and devised the "Care Model for Delirium".

    Quality of Life (QOL) is a complex construct that must be defined clearly to be clinically useful. One instrument that captures the varying importance of quality dimensions in a very structured menner is the Ferrans and Powers Quality of Life Index (QLI). A Japanese version of the QLI has been developed, incorporating trends found in previous QOL studies. In the Japanese version, patients are first asked to rate their satisfaction with 35 aspects of life, and then to rate the importance of those same aspects. In this way, scores reflect how satisfied/dissatisfied patients are with the things that matter most to them. The research reported on here, which rests on an application of this instrument in a cross-sectional study, focused on the QOL structures of home care patients and the role of nursing services in supporting QOL.The research was undertaken in connection with the WHO/QOL development study, an international comparative study of QOL.A pilot study resulted in the following findings. 1.The Japanese version of the QLI covers eight of the ten evaluation items presented by Gill, and can be easily used in home care settings to identify problems and evaluate nursing sevices. 2.A cross-sectional study of home care patients shows the rate of satisfaction to be lower than the rate of importance for each and every aspect of QOL.Nursing intervention is required to improve QOL in areas such as usefulness to others, leisure activities, social activities, personal appearance, emotional support, and standard of living. 3.The WHO/QOL pilot study suggests that questionnaires developed by Japanese for use in Japan are not very sensitive, and underscores the fact that the same aspects of life are important for individual QOL in nations and cultures around the world.
