    Orthodontic treatment of patients with periodontal disease has been difficult due to the lack of a source of anchorage to which orthodontic force can be applied.However,osseointegrated implants have become popular as a safe and reliable tool,not only for the treatment of a missing tooth but also for other purposes,including the use of osseointegrated implants to reposition natural teeth.We report here two patients in whom an osseointegrated implant was placed into the alveolar bone and used in orthodontic treatment.The oral implant was immobile by definition.Case 1 had maxillary and mandibular protrusion,a mandibular molar defect,and periodontal disease.After treatment of her periodontal disease,an osseointegrated implant was placed.Seven months later,the patient received orthodontic treatment using the implant as the source of anchorage.After 29 months of orthodontic treatment,the patient is satisfied with the improvements in her initial complaints of difficulty in mastication, dysfunction of the masticatory system, difficulty in closing the lips,and aesthetic dissatisfaction.Case 2 was a patient with anterior crossbite reversed occlusion,a molar defect,and periodontal disease.After treatment of the periodontal disease and bite-raising by temporary restoration for 2 months,an oral implant was placed.After the 6-month healing period,the patient received orthodontic treatment for 24 months.After the 6-month retention,the final prosthesis was placed using hybrid ceramics.The problem with difficulty of mastication has improved,and the patient is greatly satisfied with the treatment.In both patients who received orthodontic treatment using an oral implant as the source of anchorage,the oral implant did not move or looser. Oral implants may be useful in orthodontic treatment of patients who have no source of anchorage.

    Orthodontic treatment is based on the remodeling of the alveolar bone in response to externally applied orthodontic force to a tooth. The rate of bone resorption is, therefore, one of the essential factors for understanding tooth movement during orthodontic treatment. There is the basic concept of differential force in orthodontics that the application of an optimal force enables the canine to be moved without moving the molar that is used as an anchor for retracting the canine. In this paper, the resorption rates of the alveolar bone around the molar and the canine were estimated from the clinical measurements of tooth movement and the stress analyses of tooth roots using a 3-D finite element model. The model consisted of an orthodontic spring, the canine, the second premolar, and the first and second molars. The bone resorption rate to a unit stress at the molar was found to be about 0.5 micron/(kPa • day) and almost the same as that of the canine. This result suggests that the concept of differential force can be explained simply by differences in the geometry of the roots of moved and fixed teeth.

    The long-term transition of the orthodontic patients for the past fifteen years in the orthodontic clinic at Hokkaido University Dental Hospital. An investigation of the longitudinal change was carried out on the orthodontic patients in the orthodontic clinic at Hokkaido University Dental Hospital during fifteen years from 1981 to 1996. The findings were as follows : 1. Total patients were 4, 559 for the past fifteen years. The number of the patients increased during 1981-1990, but later decreased gradually. 2. The ratio between males and females was 1 : 1.5, and females were more frequent than males in older age patients. 3. The mean age of the patients became higher every year. The number of the growing patients declined, but those of the adult patients augmented. 4. The aesthetic problem was the highest among the several patient's complaints, and secondary was the masticatory difficulty. TMD problem has been recently increased. 5. Regarding the routes for coming to the clinic, the number of voluntary patients was decreased, and those who were refered from other dental clinics or other medical institutions were increased. 6. On the basis of frequency of malocclusions, occlusal anomalies was 74.2%, space anomalies was 78.7%. The patients with anterior crossbite were decreased and those with anterior crowding were increased every year. 7. The patients with cleft lip and/or palate were the highest among congenital dentofacial anomalies. 8. Orthognatic surgery cases accounted for 16% of all patients and the skeletal class III patients were 75.5% of those cases. 9. Patients with TMD tended to increase every year.

    A polymerase chain reaction assay for a 278-nucleotide DNA fragment within aminopeptidase My gene of Mycoplasma salivarium was developed. The assay amplified M. salivarium DNA, but did not amplify DNAs of other mollicutes, bacteria and mammalian cells. The detection limit of the assay was 10 fg of DNA, approximately equivalent to 10 organisms.

    The purpose of this investigation was to clarify the process of periodontal tissue reaction on the pressure side caused by orthodontic tooth movement. The upper canine of adult cats were tipped distally with a initial force of 100 g. Histologic specimens were made at 7, 14, and 28 days after the initial force was applied. The following results were obtained by observations of microscopic images and three-dimensional computed images (COSMOZONE 2SA, Nikon, Tokyo). 1. Degenerating area composed of the cell free zone and the intra-degeneration zone became reduced three-dimensionally from 7 days to 14 days, because the recovery of degenerated tissue had already started. Tissue recovery was almost completed at 28 days, but the degenerating area of the alveolar crest still remained on the distal side. 2. Active resorption of alveolar bone facing degenerating areas occurred at 14 days. This resorption was observed at the open cleft from bone marrow. 3. Considering the morphological features of the above bone resorption, the rear resorption should be classified into two types. One was the rear resorption in the shallow portion near the open cleft, another was the rear resorption in the deep portion of the alveolar bone. 4. The locations of tissue responses, degeneration and bone resorption, were considered to have the correlation with the magnitude of pressure distributed on periodontal tissue.
