    BACKGROUND: Perceived instability is a primary symptom among individuals with chronic ankle instability. However, the relationship between joint kinematics during landing remains unclear. Therefore, we investigated the relationships between landing kinematics and perceived instability in individuals with chronic ankle instability. METHODS: In 32 individuals with chronic ankle instability, we recorded ankle, knee, and hip joint angles during a single-leg drop landing. Joint angle waveforms during 200 ms before and after initial contact were summarized into single values using two methods: peak joint angles and principal component scores via principal component analysis. Using Spearman's rank correlation coefficient (ρ), we examined the relationships of peak joint angles and principal component scores with the Cumberland Ankle Instability Tool score, with a lower score indicating a greater perceived instability (α = 0.05). FINDINGS: The second principal component scores of ankle angle in the horizontal and sagittal planes significantly correlated with the Cumberland Ankle Instability Tool score (Horizontal: ρ = 0.507, P = 0.003; Sagittal: ρ = -0.359, P = 0.044). These scores indicated the differences in the magnitude of angles before and after landing. Significant correlations indicated a greater perceived instability correlated with smaller internal rotation and plantarflexion before landing and smaller external rotation and dorsiflexion after landing. In contrast, no peak joint angles correlated with the Cumberland Ankle Instability Tool score (P > 0.05). INTERPRETATION: In individuals with chronic ankle instability, ankle movements during landing associated with perceived instability may be a protective strategy before landing and potentially cause ankle instability after landing.

    BACKGROUND: Lumbopelvic movement patterns during prone hip extension has been proposed as a clinical screening method for trunk muscle dysfunction in patients with chronic low back pain (CLBP). However, correlations between trunk muscle onset and pelvic kinematics have not been investigated. OBJECTIVE: To examine the correlation between trunk muscle onset and pelvic kinematics during prone hip extension in participants with CLBP. METHODS: Fifteen patients with CLBP and 15 healthy individuals participated. We evaluated the muscle activities of the lumbar multifidus, the longissimus, and the semitendinosus via electromyogram and the displacement angles of the pelvic tilt, oblique and rotation. RESULTS: The onset of the multifidus at the ipsilateral side of hip extension was significantly delayed in the patients with CLBP compared to the control group (P< 0.001). The onset of the ipsilateral multifidus in the control group was significantly correlated with increased anterior pelvic tilt angle (P= 0.019, r= 0.597), whereas no significant correlation was observed in the CLBP group (P= 0.810, r=-0.068). CONCLUSION: The results suggest that pelvic kinematics during prone hip extension does not predict the delayed trunk muscle onset in patients with CLBP.

    Postural stability is essential for performing daily activities and preventing falls, whereby suspensory strategy with knee flexion may play a role in postural control. However, the contribution of the suspensory strategy for postural control during sudden lateral perturbation remains unclear. We aimed to determine how suspensory strategy contributed to postural adjustment during sudden perturbation in the lateral direction and what knee flexion setting maximized its effect. Eighteen healthy young adults (10 male and 8 female) participated in this study. Kinematic data during lateral perturbation at three velocities (7, 15, and 20 cm/s) were collected under three knee flexion angle conditions (0°, 15°, and 65°) using motion capture technology. Postural adjustments to the external perturbation were assessed by four parameters related to the temporal aspects of the center of mass (COM): reaction time, peak displacement/time and reversal time, and minimum value of the margin of stability (minimum-MOS). Our results showed that the COM height before the perturbation significantly lowered with increasing knee flexion angle. The COM reaction times for low and mid perturbation velocities were delayed at 65° of knee flexion compared to 0° and 15°, and the COM reversal times were significantly shorter at 65° of knee flexion than at 0° and 15° across all perturbation velocities. The minimum-MOS at the high-velocity of perturbation was significantly smaller at 65° of knee flexion than at 0° and 15°. In conclusion, the adoption of a suspensory strategy with slight knee flexion induced enhanced stability during sudden external and lateral perturbations. However, excessive knee flexion induced instability.

    Static stretching (SS) and dynamic stretching (DS) are widely used as warm-ups before sports. However, whether stretching affects postural control remains unclear. We compared the effects of SS and DS on the plantar flexors and postural control during single-leg standing. Fifteen healthy young participants performed SS, DS, or no stretching (control). The stretch condition consisted of four sets lasting 30 s each. The control condition was a rest with standing for 210 s. Center of pressure (COP) displacement was measured using a force plate before and after each intervention to assess postural control during the single-leg standing task. The COP area, COP velocity, and anteroposterior (COPAP) and mediolateral (COPML) range were calculated. DS significantly decreased in the COPML range (21.5 ± 4.1 to 19.0 ± 2.5 mm; P = 0.02), COP velocity (33.8 ± 7.6 to 29.8 ± 6.5 mm/s; P < 0.01), and COP area (498.6 ± 148.3 to 393.3 ± 101.1 mm2; P < 0.01), whereas SS did not change in the COP parameters (COP area 457.2 ± 108.3 to 477.8 ± 106.1 mm2, P = .49; COP velocity 31.2 ± 4.2 to 30.7 ± 5.8 mm/s, P = 0.60; COPAP 25.4 ± 3.1 to 25.3 ± 3.2 mm, P = 0.02; COPML 20.7 ± 3.3 to 21.1 ± 2.5 mm, P = 0.94). Therefore, DS of the plantar flexors enhances postural control during single-leg standing and may be effective for both injury prevention and performance enhancement.

    Accuracy validation of gait analysis using pose estimation with artificial intelligence (AI) remains inadequate, particularly in objective assessments of absolute error and similarity of waveform patterns. This study aimed to clarify objective measures for absolute error and waveform pattern similarity in gait analysis using pose estimation AI (OpenPose). Additionally, we investigated the feasibility of simultaneous measuring both lower limbs using a single camera from one side. We compared motion analysis data from pose estimation AI using video footage that was synchronized with a three-dimensional motion analysis device. The comparisons involved mean absolute error (MAE) and the coefficient of multiple correlation (CMC) to compare the waveform pattern similarity. The MAE ranged from 2.3 to 3.1° on the camera side and from 3.1 to 4.1° on the opposite side, with slightly higher accuracy on the camera side. Moreover, the CMC ranged from 0.936 to 0.994 on the camera side and from 0.890 to 0.988 on the opposite side, indicating a “very good to excellent” waveform similarity. Gait analysis using a single camera revealed that the precision on both sides was sufficiently robust for clinical evaluation, while measurement accuracy was slightly superior on the camera side.

    Background and aim The suspensory strategy, a method for controlling postural balance in the vertical direction of the center of mass (COM), is considered by the elderly as a means of balance control. The vertical COM control might alter the sensory integration and regularity of postural sway, which in turn impacts balance. However, to date, this was not confirmed. Thus, this study aimed at investigating the influence of the suspensory strategy achieved through knee flexion on the static standing balance. Methods Nineteen participants were monitored at knee flexion angles of 0°, 15°, and 65°. Time-frequency analysis and sample entropy were employed to analyze the COM data. Time-frequency analysis was utilized to assess the energy content across various frequency bands and corresponding percentage of energy within each frequency band. The outcomes of time-frequency are hypothesized to reflect the balance-related sensory input and sensory weights. Sample entropy was applied to evaluate the regularity of the COM displacement patterns. Results Knee flexion led to a decreased COM height. The highest energy content was observed at 65° knee flexion, in contrast with the lowest energy observed at 0° in both the anterior–posterior (AP) and medial-lateral (ML) directions. Additionally, the ultra-low-frequency band was more pronounced at 65° than that at 0° or 15° in the ML direction. Furthermore, the COM amplitudes were notably higher at 65° than those at 0° and 15° in the AP and ML directions, respectively. The sample entropy values were lower at 65° and 15° than those at 0° in the ML direction, with the lowest value observed at 65° in the vertical direction. Conclusion The suspensory strategy could enhance the sensory input and cause sensory reweighting, culminating in a more regular balance control. Such suspensory strategy-induced postural control modifications may potentially provide balance benefits for people with declining balance-related sensory, central processing, and musculoskeletal system functions.

    PURPOSE: Jump-landing exercises are often performed during the rehabilitation of Achilles tendon (AT) injuries. However, the factors that affect the AT force (ATF) during landing are unclear. This study aimed to determine the kinematics and ground reaction force (GRF) variables associated with the peak ATF during a drop vertical jump (DVJ). METHODS: The landing phase of DVJ was evaluated in 101 healthy participants (46 males, age: 21.2 ± 1.4 years old) using a three-dimensional motion analysis system with two force plates. ATF was estimated from the ankle flexion angle and moment. Univariate and multivariate regression analyses were performed with the peak ATF as the dependent variable. The vertical GRF (VGRF), center of pressure (COP), forward trunk leaning, hip/knee/ankle joint angles at peak ATF, and sex were used as independent variables. RESULTS: In the univariate regression analysis, larger VGRF (β = 0.813), more anterior COP position (β = 0.214), smaller knee flexion (β = -0.251) and adduction (β = -0.252), smaller hip flexion (β = -0.407), smaller forward trunk lean (β = -0.492), and male sex (β = -0.282) were significantly associated with a larger peak ATF. Multivariate analysis revealed that larger VGRF (β = 1.018), more anterior COP position (β = 0.320), a larger knee (β = 0.442), and smaller hip flexion (β = -0.205) were associated with the larger peak ATF. CONCLUSIONS: The VGRF, COP position, and knee and hip flexion were independently associated with ATF. Modifying these factors may be useful in managing tendon loading during jump-landing exercises.

    OBJECTIVES: To determine the factors associated with and prevalence of persistent pain in college athletes with chronic ankle instability (CAI) and with previous lateral ankle sprain (LAS) without CAI. DESIGN: Cross-Sectional Study. SETTING: Online survey at one university. PARTICIPANTS: Of the 385 respondents surveyed online, 140 were identified as having experienced at least one LAS (CAI group: 69, coper group: 24, LAS group not classified as either: 47). MAIN OUTCOME MEASURES: Factors associated with the presence of pain during activities of daily living (ADL) and sports were identified using multivariate logistic regression analyses. Independent variables were demographics, injury characteristics, the Identification of Functional Ankle Instability (IdFAI) score, the Foot and Ankle Ability Measure Sports (FAAM-Sports) and ADL subscales, and the Tampa Scale for Kinesiophobia-11 score. RESULTS: The prevalence of pain during ADL and sports in the three groups was 16.7%-42.0% and 33.3%-56.5%, respectively. FAAM-Sports and IdFAI score were significantly associated with pain during ADL and sports in the CAI group (odd ratio: 0.923 and 1.145), respectively. No significant pain-related factors were found in the coper and LAS groups. CONCLUSIONS: Lower self-reported function and greater perceived ankle instability may be important factors in pain management in athletes with CAI.

    BACKGROUND: Although double-leg squatting is less dynamic and places less demand on the quadriceps compared with landing tasks, the relationship between double-leg squatting biomechanics and persistent quadriceps weakness after anterior cruciate ligament reconstruction (ACLR) is unknown. PURPOSE: To clarify the relationships between asymmetries in quadriceps strength and lower limb biomechanics during double-leg squatting >1 year after ACLR. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 26 participants (5.5 ± 3.8 years after ACLR) were enrolled. The limb symmetry index (LSI) of isokinetic quadriceps strength was used to divide participants into the high-quadriceps (HQ) group (LSI ≥90%; n = 18) and the low-quadriceps (LQ) group (LSI <90%; n = 8). The knee, hip, and ankle extension moment (relative to body weight and support moment [sum of knee, hip, and ankle moments]) and vertical ground-reaction force during double-leg squatting were analyzed using 3-dimensional motion analysis. The association of quadriceps strength and biomechanical variables was tested using 2-way analysis of variance and univariate regression analysis. RESULTS: A significant group-by-limb interaction was found for the peak knee extension moment and the ratios of knee and hip extension moment to support moment (P < .001, P = .015 and P < .001, respectively). The LQ group showed a significantly smaller peak knee extension moment and knee to support moment ratio but a larger hip to support moment ratio in the involved limb than in the uninvolved limb (95% CIs: knee extension moment, -0.273 to -0.088 N·m/kg; knee to support moment ratio, -10.7% to -2.2%; hip to support moment ratio, 3.2% to 8.5%). No interlimb difference was found for the HQ group. The LSI of quadriceps strength was significantly associated with the LSI of peak knee extension moment (R2 = 0.183), knee to support moment ratio (R2 = 0.256), and hip to support moment ratio (R2 = 0.233). The mean maximum isokinetic quadriceps strength and peak knee extension moment during squatting on the involved limb of the LQ group were 2.40 ± 0.39 and 0.90 ± 0.16 N·m/kg, respectively. CONCLUSION: Asymmetrical biomechanics during double-leg squatting was associated with persistent quadriceps weakness after ACLR. The LQ group had reduced knee extensor moment on the involved side during squatting despite loading at approximately half the maximum strength. CLINICAL RELEVANCE: Quadriceps strengthening exercises, together with interventions to improve neuromuscular control, may reduce asymmetrical biomechanics during double-leg squatting.

    This study aimed to determine the differences in trunk muscle activity during rowing at maximal effort between rowers with and without low back pain (LBP). Ten rowers with LBP and 12 rowers without LBP were enrolled in this study. All rowers performed a 500-m trial using a rowing ergometer at maximal effort. The amplitudes of the activities of the thoracic erector spinae (TES), lumbar erector spinae (LES), latissimus dorsi (LD), rectus abdominis (RA), and external oblique (EO) muscles were analyzed using a wireless surface electromyography (EMG) system. EMG data at each stroke were converted into 10-time series data by recording averages at every 10% in the 100% stroke cycle and normalized by maximum voluntary isometric contraction in each muscle. Two-way repeated measures analysis of variance was performed. Significant interactions were found in the activities of the TES and LES (P < 0.001 and P = 0.047, respectively). In the post hoc test, the TES activity in the LBP group was significantly higher than that in the control group at the 10% to 20% and 20% to 30% stroke cycles (P = 0.013 and P = 0.007, respectively). The LES activity in the LBP group was significantly higher than that in the control group at the 0% to 10% stroke cycle (P < 0.001). There was a main group effect on the LD activity, with significantly higher activity in the LBP group than in the control group (P = 0.023). There were no significant interactions or main effects in the EO and RA activities between the groups. The present study showed that rowers with LBP compared with those without LBP exhibited significantly higher TES, LES, and LD muscle activities. This indicates that rowers with LBP exhibit excessive back muscle activity during rowing under maximal effort.

    Single-leg landings with or without subsequent jumping are frequently used to evaluate landing biomechanics. The purpose of this study was to investigate the effects of subsequent jumping on the external knee abduction moment and trunk and hip biomechanics during single-leg landing. Thirty young adult female participants performed a single-leg drop vertical jumping (SDVJ; landing with subsequent jumping) and single-leg drop landing (SDL; landing without subsequent jumping). Trunk, hip, and knee biomechanics were evaluated using a 3-dimensional motion analysis system. The peak knee abduction moment was significantly larger during SDVJ than during SDL (SDVJ 0.08 [0.10] N·m·kg-1·m-1, SDL 0.05 [0.10] N·m·kg-1·m-1, P = .002). The trunk lateral tilt and rotation angles toward the support-leg side and external hip abduction moment were significantly larger during SDVJ than during SDL (P < .05). The difference in the peak hip abduction moment between SDVJ and SDL predicted the difference in the peak knee abduction moment (P = .003, R2 = .252). Landing tasks with subsequent jumping would have advantages for evaluating trunk and hip control as well as knee abduction moment. In particular, evaluating hip abduction moment may be important because of its association with the knee abduction moment.

    BACKGROUND: Baseball pitching induces a large elbow valgus load, stressing the ulnar collateral ligament. Flexor-pronator mass contraction contributes to valgus stability; however, repetitive baseball pitching may weaken the flexor-pronator mass contractile function. The present study investigated the effects of repetitive baseball pitching on the medial valgus stability measured using ultrasonography. We hypothesized that repetitive pitching would decrease elbow valgus stability. METHODS: This was a controlled laboratory study. Fifteen young male baseball players at the collegiate level (age: 23.0 ± 1.4 years) were enrolled. The medial elbow joint space was measured using ultrasonography (B-mode, 12-MHz linear array transducer) in the following three conditions: at rest (unloaded), under 3 kg valgus load (loaded), and under valgus load with maximal grip contraction to activate flexor-pronator mass (loaded-contracted). All measurements were performed before and after the pitching tasks, which comprised five sets of 20 pitches. Two-way repeated-measures analysis of variance was applied to determine changes in the medial elbow joint space. The post-hoc test with Bonferroni adjustment was applied to assess the changes within the time and condition. RESULTS: The medial elbow joint space was significantly greater under the loaded than the unloaded and loaded-contracted conditions both before- and after- pitching (p < 0.001). In the loaded-contracted condition, the medial elbow joint space significantly increased after repetitive baseball pitching (p < 0.001). CONCLUSIONS: The results of the present study indicated that repetitive baseball pitching reduced the elbow valgus stability. This reduction could be attributed to the decreased flexor-pronator mass contractile function. Insufficient contraction may increase the tensile load on the ulnar collateral ligament with pitching. Flexor-pronator mass contraction plays a role in narrowing the medial elbow joint space; however, repetitive baseball pitching reduced the elbow valgus stability. It has been suggested that sufficient rest and recovery of the flexor-pronator mass function are required to reduce the ulnar collateral ligament injury risk.

    Asymmetry in knee extensor moment during double-leg squatting was observed after anterior cruciate ligament reconstruction, even after the completion of the rehabilitation program for return to sports. The purpose of this study was to clarify the association between asymmetry in the knee extensor moment and pelvic rotation angle during double-leg squatting after anterior cruciate ligament reconstruction. Twenty-four participants performed double-leg squatting. Kinetics and kinematics during squatting were analyzed using a 3-dimensional motion analysis system with 2 force plates. The limb symmetry index of knee extensor moment was predicted by the pelvic rotation angle (R2 = .376, P = .001). In addition, the pelvic rotation and the limb symmetry index of the vertical ground reaction force independently explained the limb symmetry index of the knee extensor moment (R2 = .635, P < .001, β of pelvic rotation = -0.489, β of vertical ground reaction force = 0.524). Pelvic rotation toward the involved limb was associated with a smaller knee extensor moment in the involved limb than in the uninvolved limb. The assessment of pelvic rotation would be useful for partially predicting asymmetry in the knee extensor moment during double-leg squatting. Minimizing pelvic rotation may improve the asymmetry in the knee extensor moment during double-leg squatting after anterior cruciate ligament reconstruction.

    It is well-known that multitasking impairs the performance of one or both of the concomitant ongoing tasks. Previous studies have mainly focused on how a secondary task can compromise visual or auditory information processing. However, despite dual tasking being critical to motor performance, the effects of dual-task performance on proprioceptive information processing have not been studied yet. The purpose of the present study was, therefore, to investigate whether sensorimotor task performance would be affected by the dual task and if so, in which phase of the sensorimotor task performance would this negative effect occur. The kinematic variables of passive and active knee movements elicited by the leg drop test were analyzed. Thirteen young adults participated in the study. The dual task consisted of performing serial subtractions. The results showed that the dual task increased both the reaction time to counteract passive knee-joint movements in the leg drop test and the threshold to detect those movements. The dual task did not affect the speed and time during the active knee movement and the absolute angle error between the final and the target knee angles. Furthermore, the results showed that the time to complete the sensorimotor task was prolonged in dual tasking. Our findings suggest that dual tasking reduces motor performance due to slowing down proprioceptive information processing without affecting movement execution.

    The Balance Error Scoring System (BESS), a subjective examiner-based assessment, is often employed to assess postural balance in individuals with chronic ankle instability (CAI); however, inertial sensors may enhance the detection of balance deficits. This study aimed to compare the BESS results between the CAI and healthy groups using conventional BESS scores and inertial sensor data. The BESS test (six conditions: double-leg, single-leg, and tandem stances on firm and foam surfaces, respectively) was performed for the CAI (n = 16) and healthy control (n = 16) groups with inertial sensors mounted on the sacrum and anterior shank. The BESS score was calculated visually by the examiner by counting postural sway as an error based on the recorded video. The root mean square for resultant acceleration (RMSacc) in the anteroposterior, mediolateral, and vertical directions was calculated from each inertial sensor affixed to the sacral and shank surfaces during the BESS test. The mixed-effects analysis of variance and unpaired t-test were used to assess the effects of group and condition on the BESS scores and RMSacc. No significant between-group differences were found in the RMSacc of the sacral and shank surfaces, and the BESS scores (P > 0.05), except for the total BESS score in the foam condition (CAI: 14.4 ± 3.7, control: 11.7 ± 3.4; P = 0.039). Significant main effects of the conditions were found with respect to the BESS scores and RMSacc for the sacral and anterior shank (P < 0.05). The BESS test with inertial sensors can detect differences in the BESS conditions for athletes with CAI. However, our method could not detect any differences between the CAI and healthy groups.

    Abstract Background Squatting exercises are commonly used in rehabilitation for knee joint disorders; in these exercises, control of knee extensor moment is important to enhance training effects and to avoid adverse effects. Ankle dorsiflexion and trunk flexion are widely used to reduce knee extensor moments during squatting, but the increased load on the low back is a concern. The purpose of this study was to determine whether the anterior–posterior (AP) center-of-pressure (COP) position and the AP-COP position in combination with ankle dorsiflexion and trunk flexion angles can predict the contribution of the knee extensor moment during double-leg squatting. Methods Twenty-eight healthy individuals (14 female and 14 male participants, age 22.8 ± 1.3 years) performed three sets of five consecutive double-leg squats. Kinematics and kinetics were analyzed using a three-dimensional motion analysis system with force plates. Univariate and multivariate regression analyses were performed to predict the contribution of the knee extensor moment (% total support moment) from AP-COP position, ankle dorsiflexion, and trunk flexion. Results The AP-COP position was a significant predictor of the knee extensor moment contribution (R2 = 0.168, P = 0.030). Multivariate analysis showed that the ankle dorsiflexion angle (ΔR2 = 0.561, β = 0.842) and AP-COP position (ΔR2 = 0.296, β =  − 0.499) predicted the knee extensor moment contribution (model R2 = 0.857, P < 0.001). Additionally, the combination of trunk flexion (ΔR2 = 0.429, β =  − 0.613) and AP-COP position (ΔR2 = 0.109, β =  − 0.332) predicted the knee extensor moment contribution (model R2 = 0.538, P < 0.001). The limb symmetry index of the knee extensor moment was significantly associated with that of the AP-COP position (R2 = 0.493, P < 0.001) but not with that of the ankle dorsiflexion angle (P = 0.057). Conclusions The AP-COP position can predict the contribution of the knee extensor moment and improve the prediction when combined with ankle dorsiflexion and trunk flexion. The present findings suggest that intervention focusing on the AP-COP position in combination with ankle dorsiflexion or trunk flexion would be useful to coordinate the contribution of the knee extensor moment during double-leg squatting.

    In baseball pitching, suppressing trunk rotation while rotating the pelvis in the early phase of arm cocking is important for throwing a fast ball. However, quantitative evaluation of trunk rotation during pitching has not been established, and its associations with elbow and shoulder torques are unclear. The purpose of this study was to examine the correlation of a new measure of trunk rotation suppression with ball speed and elbow and shoulder torques during pitching. Eighteen adult male baseball pitchers (21.7 ± 1.2 years old) participated. Three qualified pitches were analysed using a three-dimensional motion capture system. Trunk rotation velocity, normalised to the peak velocity, was derived at the time of peak pelvic velocity. Pearson's correlation coefficient was used to determine correlations. The normalised trunk rotation velocity at the peak pelvic velocity was significantly correlated with elbow valgus torque (R = -0.508, P = 0.032), shoulder external rotation torque (R = -0.507, P = 0.032) and ball speed (R = -0.504, P = 0.033). A smaller normalised trunk rotation angular velocity at the time of peak pelvic rotation velocity could increase ball speed but may also increase elbow and shoulder torques among pitchers who demonstrate trunk rotation after foot contact.

    The effects of changes in the anterior-posterior center of pressure (AP-COP) position on the lower limb joint moments during double-leg squatting remain unclear. The purpose of this study was to determine the effects of AP-COP positional changes on the hip, knee, and ankle extensor moments during double-leg squatting. Sixteen male participants (22.1 ± 1.5 years) performed double-leg squatting under two conditions (anterior and posterior COP conditions) with visual feedback on their COP positions. Kinematics and kinetics were analyzed using a three-dimensional motion analysis system and force plates. The hip, knee and ankle flexion angles and extensor moments at peak vertical ground reaction force were compared between the two conditions using paired t tests. The COP position was 53.5 ± 2.4% of the foot length, starting from the heel, under the anterior condition and 44.4 ± 2.1% under the posterior condition (P < 0.001). The knee extensor moment was significantly smaller under the anterior than the posterior COP condition (P = 0.003, 95% confidence interval (CI) -0.087 to -0.021 Nm/kg/m), while the ankle extensor moment significantly larger under the anterior COP condition than under the posterior COP condition (P < 0.001, 95% CI 0.113 to 0.147 Nm/kg/m). There was no significant difference in hip extensor moment (P = 0.431). The ankle dorsiflexion angle was significantly larger under the anterior than the posterior COP condition (P = 0.003, 95% CI 0.6 to 2.6°), while there was no difference in trunk, hip, or knee flexion angle. The present results indicate that changes in the AP-COP position mainly affect the ankle and knee extensor moments during double-leg squatting, while the effect on the lower limb joint and trunk flexion angles was limited. Visual feedback on the AP-COP position could be useful for modifying the ankle and knee extensor moments during double-leg squatting.

    With this study, we aimed to investigate the effects of different warm-up intensities on counter-movement jump (CMJ) performance over time under cold conditions. Eleven male collegiate athletes volunteered. The participants performed high-intensity warm-up (HWU) at 80% VO2max and moderate-intensity warm-up (MWU) at 60% VO2max for 15 min on a bicycle ergometer in a laboratory room at 10 °C. CMJ height, vastus lateralis muscle temperature, heart rate, and perceived fatigue were measured before warm-up (Pre), immediately after (Post 0), 10 min after (Post 10), and 20 min after (Post 20). Significant main effects and interactions were found for CMJ height (time, p < 0.001 and ηp2 = 0.859; interaction, p = 0.007 and ηp2 = 0.327). HWU significantly increased CMJ height at Post 0 to Post 20 compared to that at Pre (p < 0.01), whereas MWU increased CMJ height at Post 0 only compared to that at Pre (p < 0.001). The results indicate that HWU achieved an increase in CMJ height for 20 min. MWU changed CMJ height instantly, but the change did not last compared to HWU in a cold environment.

    The influence of low energy availability (LEA) on bone mineral density (BMD) and trabecular bone microarchitecture in pubescent female athletes is unclear. This study aimed to investigate the influence of LEA on BMD and trabecular bone microarchitecture in 21 pubescent female athletes (age, 12-15 years; 11 track and field athletes, 10 gymnasts). We used two indices to assess LEA: energy availability and the percent of ideal body weight. Dual-energy X-ray absorptiometry was used to obtain total body less head, lumbar spine BMD Z-scores, and lumbar trabecular bone scores (TBS). Pearson's or Spearman's correlation coefficients were used to assess the relationship among EA, percent of ideal body weight, and bone parameters. The threshold for statistical significance was set at p < 0.05. The percent of ideal body weight was significantly correlated with the BMD Z-scores of the total body less head (r = 0.61; p < 0.01), lumbar spine (r = 0.55; p < 0.01), and lumbar TBS (r = 0.47; p = 0.03). However, energy availability was not correlated with bone parameters. These findings suggest that screening for low ideal body weight may be a useful predictor of low BMD and insufficient trabecular bone microarchitecture in pubescent female athletes.

    BACKGROUND: Lateral trunk obliquity during landing is a characteristic of anterior cruciate ligament (ACL) injuries in female athletes and affects their knee and hip kinetics and kinematics. However, it is unclear whether these effects differ between females and males. The purpose of this study was to compare the effects of lateral trunk obliquity on knee and hip kinetics and kinematics in females and males during single-leg landing. METHODS: Eighteen female (aged 22.1 ± 1.5 years) and 18 male participants (aged 21.8 ± 1.1 years) performed single-leg landings under two conditions: (1) without any instructions about trunk position (natural) and (2) with leaning their trunks laterally 15° from the vertical line (trunk obliquity). The kinetics and kinematics of their hip and knee were analyzed using a three-dimensional motion analysis with a force plate. Two-way repeated-measures ANOVA (sex × trunk obliquity) and Bonferroni pairwise comparisons were conducted. RESULTS: The trunk obliquity angle at initial contact was significantly greater in the trunk-obliquity landing condition than in the natural landing condition (natural 4.0 ± 2.2°, trunk-obliquity 15.1 ± 3.6°, P < 0.001) with no sex difference (95% CI - 1.2 to 2.2°, P = 0.555). The peak knee abduction moment was significantly larger in the trunk-obliquity landing condition than in the natural landing condition (trunk-obliquity, 0.09 ± 0.07 Nm/kg/m; natural, 0.04 ± 0.06 Nm/kg/m; P < 0.001), though there was no sex or interaction effect. A significant interaction between sex and landing condition was found for the peak hip abduction moment (P = 0.021). Males showed a significantly larger peak hip abduction moment in the trunk-obliquity landing condition than in the natural landing condition (95% CI 0.05 to 0.13 Nm/kg/m, P < 0.001), while females showed no difference in the peak hip abduction moment between the two landing conditions (95% CI - 0.02 to 0.06 Nm/kg/m, P = 0.355). CONCLUSIONS: The knee abduction moment increased with a laterally inclined trunk for both female and male participants, while the hip abduction moment increased in males but not in females. It may be beneficial for females to focus on frontal plane hip joint control under lateral trunk-obliquity conditions during single-leg landing.

    OBJECTIVE: To investigate the effects of knee flexion angle on peak torque, rate of torque development (RTD) during isometric contraction and hamstring flexibility after hamstring strain injury (HSI). DESIGN: Cross-sectional. SETTING: Controlled laboratory research. PARTICIPANTS: Fourteen male athletes with a history of HSI and 14 athletes without HSI (controls). MAIN OUTCOME MEASURES: Hamstring flexibility was evaluated using active knee extension test. Isometric knee flexion peak torque and RTD were determined at 30°, 60°, and 90° of knee flexion measured by an isokinetic dynamometer. RESULTS: Individuals with a history of HSI had statistically significant, moderate deficits in isometric peak torque at 30° of knee flexion (P = 0.037; effect size = 0.55) in the HSI limb than in the uninjured limb, but not at 60° and 90° of knee flexion. In the control group, no significant differences in isometric peak torque at any angle were found between limbs. No differences in peak RTD and flexibility were found between limbs in both groups. CONCLUSIONS: Isometric peak torque at 30° of knee flexion was lower in the injured limb than in the uninjured limb. Isometric strength deficits after HSI tended to be affected by lengthened hamstring angles.

    OBJECTIVES: Falls in older adults generally occur during unpredictable situations. Controlling posture through fine-tuned muscle force before and after falls is necessary to avoid serious injuries. However, details regarding force control among older adults during unpredictable situations are unclear. This study determined the features of force control in a random force-tracking task among older adults. METHODS: Ten healthy older adults (67-76 years) and eight healthy young adults (20-23 years) participated in three force-tracking tasks with ankle plantar flexion: low-range (LR), high-range (HR), and pseudo-random (PR) force tasks. Force control ability was assessed using the root mean square error (RMSE) between the target and muscle forces produced by the participants. Muscle activities from the lateral head of the gastrocnemius and the tibialis anterior during each task were measured using surface electromyography to calculate the co-contraction index (CCI). RESULTS: In all tasks, older adults (RMSEs: 1.09-3.70, CCIs: 29.4-56.4) had a significantly greater RMSEs and CCIs than young adults (RMSEs: 0.49-1.83, CCIs: 11.7-20.6; all, p < 0.05). The RMSEs during force generation were significantly greater than those during force release (LR: p < 0.01, HR: p < 0.05), except for the random force-tracking task in older adults. CCIs during the force release phase in both groups (older adults: 27.8-56.4, young adults: 15.0-20.6) were consistently greater than those during force generation (older adults: 24.5-50.4, young adults: 11.7-17.2). CCIs in force-tracing tasks differed in older adults, whereas those in the random force-tracing task increased. RMSEs and CCIs in the random and LR force-tracing tasks were significantly negatively correlated with the functional reach test (all: r > 0.5, p < 0.05). CONCLUSION: Force control in older adults declines in low-band and random muscle force output. Moreover, increased CCIs in older adults are particularly pronounced during unpredictable situations.

    The different effects of intermittent and continuous stretching on the mechanical properties of the musculotendinous complex have been unclear. This study aimed to compare the effects of intermittent and continuous stretching for the same duration on the range of motion (ROM), passive resistive torque (PRT), and musculotendinous stiffness (MTS) of ankle plantar flexors. Eighteen healthy young men participated in the study. Intermittent (four sets × 30 s) and continuous stretching (one set × 120 s) were performed in random orders on two separate days. Both stretching protocols were conducted using a dynamometer with a constant torque applied. ROM and PRT were determined using a dynamometer, and MTS was calculated using the torque-angle relationship measured before and after stretching. Two-way repeated measures analysis of variance was performed for all parameters. Both intermittent and continuous stretching significantly increased ROM and decreased PRT and MTS (p < 0.05). Intermittent stretching led to greater changes in ROM and PRT than continuous stretching. However, the reduction in MTS did not differ between the two conditions. These results suggest that intermittent stretching is more effective in increasing ROM and changing the mechanical properties of the musculotendinous complex.

The standard deviation of the center of mass (COM) and lower limb joint motions in the anterior-posterior (AP) and medial-lateral (ML) planes were calculated from three-dimensional marker trajectories. Pearson's correlation analysis and independent t-tests were conducted to investigate the relationship between COM and lower limb joint motion and to compare group difference, respectively.RESULTS: The AP hip angular velocity alone in the knee OA group and the AP hip and knee angular velocity in the control group were significantly correlated with the AP COM velocity. The ML hip angular velocity was significantly correlated with the ML COM velocity in both groups. The knee OA group exhibited a significantly larger standard deviation of AP COM velocity than the control group.CONCLUSIONS: Individuals with knee OA depended solely on the contribution of the hip to the AP COM velocity, which could not be successfully controlled by the knee.

    OBJECTIVES: To investigate the effects of pelvic and trunk lateral tilt-focused landing instructions on the knee abduction moment during the single-leg drop vertical jump task. DESIGN: Descriptive laboratory study. SETTING: Motion analysis laboratory. PARTICIPANTS: Fifteen young, healthy female participants. MAIN OUTCOME MEASURES: The participants performed 15 single-leg drop vertical jumps. Landing instructions with self-video recordings were provided so that the participants' pelvis and trunk remained horizontal in the frontal plane. Pelvic, trunk and knee kinematics and kinetics were evaluated using a three-dimensional motion analysis system before and after the landing instructions. RESULTS: The peak knee abduction moment significantly decreased postinstruction (preinstruction 22.6 ± 15.3 Nm, postinstruction 17.9 ± 15.4 Nm, P = 0.004), as did pelvic and trunk lateral tilt (P < 0.01). The knee abduction and internal rotation angles at initial contact significantly decreased postinstruction (P = 0.037, P = 0.007), with no significant change in the peak knee abduction and internal rotation angles from pre-to postinstruction. CONCLUSIONS: Landing instructions focused on pelvic and trunk lateral tilt are effective in decreasing the knee abduction moment during the single-leg drop vertical jump. Pelvic and trunk lateral tilt should be controlled to decrease the knee abduction moment during single-leg landing.

    OBJECTIVES: To investigate the prevalence and characteristics of chronic ankle instability (CAI) and copers among collegiate athletes as identified by criteria for research (CAI-R and copers-R) and for clinical practice (CAI-C and copers-C). DESIGN: Cross-sectional study. SETTING: University. PARTICIPANTS: Collegiate athletes (n = 507). MAIN OUTCOME MEASURES: Participants were assessed by questionnaires based on the International Ankle Consortium guidelines. The percentages of participants with CAI-R, CAI-C, copers-R and copers-C were calculated, respectively. Demographic and injury data were statistically compared between CAI-R and copers-R groups. RESULTS: The data of 470 participants was retained after exclusions. Of these, the prevalence of CAI-R (10.0%) was only half of that of CAI-C (19.8%), and that of copers-R and copers-C was about 5%. Seventy percent of unclassifiable participants had recurrent ankle sprains. CAI was most common in basketball, while copers were less prevalent in basketball, judo, rugby and gymnastics. The age at the initial injury was significantly younger in the CAI-R participants than in the copers-R. CONCLUSIONS: The type of sport and the age at the initial injury may be associated with developing CAI. The standard criteria may not capture the entire clinical CAI population, therefore, care should be taken when applying the research to clinical practice.

  • 北海道の全市における特定保健指導の運動指導の現状と今後の課題

    目的:北海道で行われている特定保健指導の運動指導の現状について明らかにし、他職種の関わりついて明らかにする。方法:北海道の全市役所と委託を受けた施設の特定保健指導を担当する部署へアンケート用紙を郵送にて送付し、郵送式またはインターネットによる回答を依頼した。結果:回答率79.4%、有効回答数は143名であった。実務者は保健師(77.6%)と管理栄養師(21.0%)の2つの職種で98%を占めた。全体の指導時間に占める運動指導の時間は少なく、「食習慣」を最も重要視していた。理学療法士による運動指導が必要と回答した人は、初回支援47.6%、継続支援62.9%であった。理学療法士への相談内容として、「疾患を持つ方への運動指導」と「支援対象者に合った適切な運動指導」が多かった。北海道の冬期の運動指導の困難さについて、「活動量の低下・運動習慣の継続困難」と「冬道での転倒リスクの増大、ヒートショックの恐れ」を挙げていた。結論:北海道での特定保健指導は保健師や管理栄養士により行われており、運動習慣への指導は他の生活習慣に比べて優先されていなかった。実務者は、膝や腰に痛みを抱えるなど、すでに運動障害を抱える支援対象者に合わせた運動指導やリスク管理に困難さを抱えており、運動指導への理学療法士の参加の必要性が示唆された。北海道の地域特性は、他の積雪寒冷地域と同様に、冬期の活動量の維持が課題であることが明らかとなった。(著者抄録)

    BACKGROUND: Abdominal draw-in maneuver (ADIM) has been recommended to achieve appropriate trunk muscle response for patients with non-specific chronic low back pain (CLBP). However, it has remained unclear whether the intervention with ADIM could change the trunk muscle response to sudden release from loading, which is considered to contribute mechanical circumstances to low back pain. The purpose of the present study was to investigate the effects of the intervention with ADIM on electromyography (EMG) activities of trunk muscles following sudden release from loading. METHODS: Seventeen subjects with non-specific CLBP participated. Subjects resisted trunk flexion or extension loading in semi-seated position, and then the loading was suddenly released. EMG recordings of 6 trunk muscles were acquired using a wireless surface EMG system. Onset and offset times were calculated from the EMG data. The intervention with ADIM was provided for 4 weeks. The onset and offset times were compared between pre- and post-intervention with ADIM. RESULTS: At the post-intervention, the onset of trunk flexors following release from trunk flexion loading became significantly earlier than pre-intervention (P = 0.028). The offset of flexors following release from trunk extension loading of post-intervention was significantly earlier than that of pre-intervention (P = 0.001). CONCLUSIONS: We showed that the intervention with ADIM changed the EMG activity of trunk flexors in response to sudden release from loading. These results suggest a possibility that ADIM might be effective to improve the neuromuscular control of trunk flexors for the treatment of young patients with non-specific CLBP.

    【はじめに】高齢者の健康上の問題として,骨折があげられる。骨折の原因の多くは,姿勢調節の障害に関係する転倒である。高齢者の姿勢調節は,視覚情報への高い依存度,股関節戦略の利用,反応時間の遅れ,運動自由度の低下などを特徴とする。また,高齢者の立位バランス時の下肢筋活動の同時収縮も特徴の一つである。これまでの姿勢制御に関する研究報告では,下肢筋の活動(興奮性)に着目して同時収縮を主に評価している。しかしながら,動筋の作用を効率よくするためには,拮抗筋の抑制も重要である。本研究では,拮抗筋の抑制性に着目し,動的運動課題時の反応時間と下肢筋活動の加齢の影響について調べた。

    【方法】対象は健康若年者29名,健康高齢者26名とし,1年以内に転倒歴のない者であった。測定肢位は安静直立位とし,床反力計上に立たせた。運動課題は視覚誘導型の足圧中心(COP)を用いた追跡課題とし,被験者に「眼前に設置したコンピュータ・モニター内に提示された目標にCOPを一致して下さい」と指示した。被験者は課題を10試行行った。モニター内の目標の動きは上方へのステップ状の離散運動とし(振幅5.0cm),実際の運動はCOPの前方移動とした。COPデータから目標に対する運動反応時間,前後の最大速度,運動停止時間を算出した。導出筋は,利き足側の脊柱起立筋,腹直筋,大腿直筋,大腿二頭筋,前脛骨筋,腓腹筋とした。各筋の活動と抑制の発現率,活動時間,抑制時間を計測した。COPのデータのグループ間と筋の各時間のグループ間の比較はt検定,各筋の発現率はカイ二乗検定を用いて統計解析を行った。有意水準は0.05未満とした。

    【結果】高齢群のCOPの運動反応時間は若年群に比べて有意に遅延し(若年群341.3±5.4 msec,高齢群363.9±9.2 msec,p<0.05),高齢群の運動停止時間は有意に延長していた(若年群2078.0±163.3 msec,高齢群4828.3±201.4 msec,p<0.01)。COPの前方および後方最大速度に年齢差はなかった。高齢群の大腿四頭筋の活動の発現率が有意に高く(大腿四頭筋:若年群77.3%,高齢群100.0%,p<0.05),高齢群の大腿二頭筋と腓腹筋の抑制の発現率は有意に低かった(大腿二頭筋:若年群90.9%,高齢群64.7%,p<0.05,腓腹筋:若年群100.0%,高齢群88.2%)。高齢群の前脛骨筋,脊柱起立筋,大腿二頭筋,腓腹筋の活動時間は若年群より有意に延長し,高齢群の脊柱起立筋,大腿二頭筋,腓腹筋の抑制時間は有意に短縮していた。

    【結論】本研究より,高齢者の運動開始前の拮抗筋の抑制と運動停止時の動筋の抑制は若年者に比べて低いことが明らかとなった。円滑な足圧中心や重心移動には筋の抑制も重要であるこを示唆する。

    [Purpose] This study investigated the effects of changes in ankle angle on standing balance. [Subjects] Twelve healthy young individuals. [Methods] The subjects performed static and dynamic balance tasks on a 20˚ sloping platform (ankle plantarflexion or dorsiflexion) and a flat platform (control, 0˚). The position of the center of mass and the joint angles of the lower limbs were measured with a three-dimensional motion analysis system. [Results] The vertical position of the center of mass in plantarflexion was the highest among three ankle conditions. There were no significant differences in hip and knee joint angles among three ankle conditions during the static balance. Dynamic balance ability during ankle dorsiflexion was significantly lower than during plantarflexion and control positions. During the dynamic task with dorsiflexion, hip motion increased and knee motion was antiphase compared with the control angle. [Conclusion] The results suggest changes in ankle angle influence dynamic postural control during the standing posture.

    【はじめに,目的】立位において足関節が背屈する方向に急に床面が傾斜すると,下腿三頭筋が伸張され,傾斜後100-120msのタイミングで下腿三頭筋に急激な収縮が起こる(Melvill & Watt, 1971)。これは機能的伸張反射と呼ばれ,この反射により身体は大きく後方に動揺する。しかしながら,この足関節背屈方向への床面傾斜を数試行繰り返すだけで機能的伸張反射は抑制されることが知られており,後方への身体動揺も減少する(Nashner, 1976)。筋緊張の異常を呈する疾患では機能的伸張反射が亢進し転倒の危険性が高まる恐れがあるため,この反射を抑制し,その抑制を持続させる必要がある。そこで今回,我々は床面を傾斜させることで足関節背屈刺激を与え,その刺激の繰り返しによる機能的伸張反射の抑制効果と身体動揺への影響を調べた。【方法】研究趣旨に同意し,神経学的,及び整形外科学的既往のない健常成人15名(22±1歳)を対象とした。被験者は足関節軸に合わせて背屈方向に傾斜する床面に立ち,傾斜に対しできるだけ動揺しないよう要求された。傾斜は水平位から振幅10°,角速度10°/secで台形波状に与えられた。傾斜はプレテストとして10試行,適応テストとして100試行,ポストテストとして10分後と30分後に10試行ずつ繰り返された。腓腹筋の積分筋活動量と三次元動作解析装置(Motion analysis社製)により記録した体重心の移動距離を算出した。床面が傾斜した直後の腓腹筋活動量(0-250ms)と体重心の最大移動距離についてプレテストと各テストにおける10試行の平均値を比較した。統計検定として反復測定一元配置分散分析と多重比較(Bonferroni)を行った。危険率は5%とした。【結果】プレテストにおいて出現した腓腹筋の機能的伸張反射は足関節背屈刺激の繰り返しにより全ての被験者で有意に抑制され消失した(0.49±0.12 vs 0.16±0.08,p<0.01)。また,その抑制効果は10分後(0.24±0.09,p<0.01)と30分後(0.28±0.12,p<0.01)も持続した。体重心の動揺距離は有意に減少した(41.8±11.2mm vs 26.6±8.3mm,p<0.01)。その減少は10分後(30.1±10.6mm,p<0.01)と30分後(28.4±8.7mm,p<0.01)も保持された。【結論】床面を傾斜させ足関節背屈刺激を繰り返し与えることにより機能的伸張反射は抑制され,少なくとも30分間は抑制効果が持続した。この結果は,反射レベルの筋活動においても適切な刺激の繰り返しにより抑制の適応が起こること,その抑制を持続させることが可能であることを示唆している。また,このような筋活動の抑制の持続により身体動揺が減少し,姿勢の安定性が向上すると考える。本研究を発展させ,適切な繰り返しの回数や刺激強度を調べて機能的伸張反射抑制の持続効果を高めることは,機能的伸張反射が亢進した患者の転倒防止に役立つであろうし,将来のリハビリテーションにとって重要と考える。

    Preventing fall-related injuries is becoming a priority as the world population ages. This study's purpose was to examine the effect of aging on vertical postural control in the community-dwelling elderly. Thirty-six elderly individuals and twenty-two healthy young adults were asked to shift their centers of pressure (COPs) as far as possible while standing. The COP position, angle of each lower leg joint, and postural muscle activities were measured using a force plate, three-dimensional motion analyzer, and electromyogram, respectively. The vertical position of the center of mass (COM) was also measured to assess the change in vertical postural control. The backward COP shift in the elderly group was significantly smaller than that in the young group, and both the forward and backward COM shifts were significantly smaller in elders relative to those in youths. The COM position in the elderly group during the backward COP shift was also significantly lower than that in the young group. Knee and ankle joint movements differed between the two groups during the backward COP shift. Factor analysis indicated that dorsal and ventral muscle groups were involved in the COP shift. Specifically, the relationship between the biceps femoris muscle and the voluntary COP shift was reinforced in the elderly group.     Purpose: This study investigated leg joint movements of the elderly during the squat motion. Subjects: Nineteen healthy elderly individuals in their 60s and 70s participated. Methods: The reaction time, maximum joint angle, distance between the knees foot angle, and the center of mass during the squat task with eyes closed recorded and analyzed using a force plate and a three-dimensional motion analysis system. Results: There were no significant differences in the reaction times between joints or ages. The maximum knee angle of the 70s group was significant smaller than that of the 60s group. Although the distance between the knees of all participants increased during the squat motion, there was no difference between the two age groups. Both the hip and the knee joints of the 70s group showed significant correlations with the vertical component of the center of mass. Conclusion: The results suggest that the joint motion of the lower limbs of the elderly changes during the squat motion with aging.

    【目的】随意的に荷重を移動させる動的バランスの課題を用いて,聴覚フィードバックと視覚フィードバックの学習効果を比較することだった。【方法】被験者は健常若年者20 名とし,無作為に10 名ずつ2 群に割りあてた。被験者にはモニター上に写し出されたターゲットに,自身の足圧中心を前後に移動させて一致させるように指示した。練習ではターゲットの他に,足圧中心位置と連動して聴覚フィードバック(以下,聴覚群)または視覚フィードバック(以下,視覚群)が与えられた。正確性の指標として,足圧中心とターゲットとの間の距離の二乗平均平方根(以下,RMS)を算出した。【結果】両群ともに練習直後は,練習前と比較してRMS が有意に減少した。一方,練習後から1 日経過後では,聴覚群でRMS の減少が持続していたが,視覚群では有意な増加がみられた。【結論】断続的な感覚フィードバックを用いた動的バランスにおいては,聴覚が視覚よりも運動学習に優れている。

    Controlling weight shift (WS) is essential to performing motions safely and smoothly during daily and athletic activities. This study investigated the impact of loading parameters on the motor performance and difficulty level of a dynamic WS task performed while standing. Twelve healthy young adults (21.2 +/- 0.9 years, 53.5 +/- 7.4 kg) were asked to match the target and their weight loads using visual feedback displayed on a computer monitor. Motor performance was estimated by assessment of loading accuracy and pace of motor skill acquisition, measured as a proxy of difficulty level, was estimated by assessment of learning rate. As predicted, both loading accuracy values decreased with increasing target frequency. Notably, the interaction of loading size and frequency had a significant effect on loading accuracy, which was increasingly impaired as the weight load increased at frequencies greater than 0.5 Hz. Moreover, the pace of motor skill acquisition in a dynamic WS task while standing was affected by the weight-load speed independently of the weight-load size. These results indicate that loading accuracy is affected by both the weight-load size and frequency and that 0.5 Hz is a critical frequency at which the difficulty level increases during dynamic WS tasks while standing in healthy youths. These findings suggest that the adequacy of the initial settings used regarding loading size and frequency is an important consideration in rehabilitative and athletic training aimed at evaluating and improving WS while standing.     Anticipatory postural adjustments (APAs) provide postural stability and play an important role in ensuring appropriate motor performance. APAs also change in various situations. However, it is unknown whether changes in APAs during repetitive movement training contribute to improvement in motor performance. This study aimed to investigate the relationship between improvement in motor performance and changes in APAs during repeated reaching training, as well as the learning effects on APA changes. Sixteen healthy subjects (23 +/- 2 years of age) stood barefoot on a force platform and reached as quickly and accurately as possible to a target placed at their maximum reach distance immediately following a beep signal in a reaction time condition. Whole-body reaching training with the right arm was repeated 100 times for three consecutive days. Motor performance and APAs were evaluated on the first day, after discontinuation of training for one day, and again at three months. In addition, reaching with the left arm (untrained limb) was tested on the first and the fifth training day. Body position segments were measured using three-dimensional motion analysis. Surface electromyography of eight postural muscles in both lower limbs was recorded. Kinetics data were recorded using the force platform. Whole-body reaching training induced not only improvements in motor performance (e.g., increased peak hand velocity), but also changes in APAs (e.g., earlier APA onset and increased amplitude). These changes were strongly correlated with and occurred earlier than improvements in motor performance. The learning effects on APAs were retained after the discontinuation of training and were generalized to the untrained limb.     【はじめに,目的】立位バランスの改善を目的とする練習方法として感覚フィードバックを用いた練習が一般的に行われている。感覚フィードバックには主として視覚・聴覚・体性感覚が使用されており,その効果は多くの先行研究で示されている。しかし,その効果は姿勢動揺を減少させるための静的バランスを課題としており,随意的に体重心を移動させるための動的バランスについての報告は極めて少ない。加えて,用いられている感覚は視覚が多く,聴覚など他の感覚フィードバックによる学習効果と比較検討した立位バランスに関する研究は筆者らが知る限り見当たらない。従って,本研究はフィードバック練習に用いる感覚の違いが動的バランスの学習効果に与える影響について明らかにすることを目的とした。【方法】被験者は,健常若年者18名(平均年齢22.3±1.9歳,平均身長165.8±7.5cm,平均体重56.4±7.9kg)であり,無作為に聴覚刺激をフィードバックに用いた9名(聴覚群)と視覚刺激をフィードバックに用いた9名(視覚群)に割り当てられた。被験者には,床反力計上に裸足で立ち,前方に設置したモニター画面上に表示された前後方向に移動するターゲットに自身の足圧中心(COP)を一致させるように指示した(動的バランス課題)。ターゲットは被験者のCOP最大移動距離の前方80%から後方70%を周波数0.23Hzで移動するようにLabVIEWソフトウェアを用いて作成した。練習課題ではターゲットの他にCOP最大移動距離の前方70~80%,後方60~70%の範囲に被験者のCOPが位置したときに各群に合わせたフィードバックを与えた(聴覚群:スピーカーからの音,視覚群:モニター画面上に赤い点)。課題は1施行30秒とし,1日目には練習前と練習後に動的バランス課題を5施行ずつと練習課題を20施行で合わせて30施行を行い,1日介入のない日を設けて,3日目に動的バランス課題5施行を行った。また,動作開始時の影響を取り除くために,1施行の7周期から最初の1周期を除外した6周期を解析対象とした。動作の正確性の指標として,床反力計から算出された被験者のCOPとターゲットとの間の距離の二乗平均平方根(RMS)を使用した。フィードバック練習の適応および学習効果について調べるために,練習前・練習後・3日目の3群間比較には一元配置分散分析および多重比較を行った。また,両群間(視覚,聴覚)の比較をするために,練習前のRMS値,練習前と練習後のRMS値の差および練習後と3日目のRMS値の差による変化量を算出し,対応のないt検定を行った。検定の有意水準は5%未満とした。【倫理的配慮,説明と同意】被験者には事前に口頭と書面で本研究の目的や実験手順,考えられる危険性などを十分に説明し,書面にて同意を得た。本研究は,当大学院倫理委員会の承認を得て行った。【結果】課題間での比較については,聴覚群は練習前と比較して練習後および3日目で有意な低下を示した(p<0.01)。一方,視覚群では練習前と比較して練習後に有意な低下を示したが(p<0.05),3日目は有意差がみられなかった。群間の比較では,練習前のRMS値に有意差はみられなかった。変化量の比較では,練習前後で両群間に有意差はみられなかったが,練習後から3日目を引いた値では視覚群(-4.19±3.62)は聴覚群(0.78±2.38)と比較して有意に小さい値を示した(p<0.01)。【考察】練習前の群間比較でRMS値に有意差がみられず,練習前後では両群ともに有意な低下が示された。さらに,その変化量には有意差がみられなかった。従って,動的バランスにおいて聴覚および視覚のフィードバック練習によって同程度の適応効果が示されたと考えられる。しかしながら,聴覚群では3日目においてもその効果の保続が示されたが,視覚群では一日あけることによって練習前の正確性に戻り,変化量の群間比較においても有意差が示された。従って,聴覚フィードバック練習によって学習効果が示されたが,視覚フィードバック練習では学習されにくいことが示唆される。Ronsseらは,両手の協調運動を用いて視覚フィードバック練習では視覚刺激に頼りがちになるが,聴覚フィードバックを用いた場合は刺激に頼らない神経回路が構築されやすいことを明らかにした。よって,随意運動を伴う動的バランス練習においても聴覚刺激の方が運動学習には有効であると考えられる。【理学療法学研究としての意義】立位バランスにおいて,異なる感覚フィードバックによる学習効果の違いを初めて明らかにした。本研究成果は,視覚に頼らない有効な感覚フィードバック練習を提案するための科学的根拠となる。

    [Purpose] The purpose of this study was to investigate the effects of aging on the squat task. [Subjects] Nineteen healthy elderly individuals participated (mean age: 70.5 years). [Methods] The squat task with eyes closed was performed on a force platform. The center of mass was recorded with a three-dimensional motion system and the reaction time, amplitude, and downward velocity of the center of mass were analyzed. [Results] There was no significant difference in the reaction times of the participants in their 60s and 70s. The downward amplitude and velocity of those in their 70s were lower than those in their 60s. Also, elderly participants in their 70s showed a tendency of increased anterior-posterior amplitude compared with those in their 60s. [Conclusion] The results suggest that the squat task becomes slower and more shallow with aging. There is a possibility that change in the motion of the center of mass in the sagittal plane is influenced by aging.

    [Purpose] The purpose of this study was to investigate the effect of the amplitude and velocity on the weight shift task. [Subjects] Six healthy young adults participated. [Method] The visually-guided weight shift task was performed on one force platform. Motor directions were the anteroposterior and mediolateral directions, load amplitudes were 50 N and 1/3 of subject's weight, and motor frequencies were 0.1-1.0 Hz. The spatial and temporal errors were analyzed to assess the motor performance. [Results] There was no significant difference between motor directions. The spatial errors for the light weight were larger than those for the heavy weight. The temporal errors were not affected by load amplitudes and increased with increases in motor frequency. [Conclusion] During the weight shift task, spatial and temporal controls were affected by the load amplitude and the motor frequency, respectively. Balance exercise with this task should be reconsidered in clinical settings.

    [Purpose] The aim of this study was to quantitatively investigate gender differences in trunk muscle strength and activity and to verify the two proposed field tests for screening the muscle strength of trunk flexors by use of surface EMG (sEMG). [Subjects and Methods] Fifty-four healthy young volunteers participated in this study and RA strength was evaluated using the curl-up (CU) and double-leg-lower (DLL) tests. All muscle activity that occurred in the upper and lower RA sites during the two tests was recorded using sEMG. The sEMG data were then normalized using the maximum voluntary contraction. To investigate the validity of the two tests, the ratio of muscle activity between the upper and lower RA sites was calculated. [Results] The reliability of the CU and the DLL tests were estimated using the intraclass correlation coefficient and the values ranged from 0.84-0.96 and 0.93-0.98, respectively. Muscle activity was significantly higher in females than in males in all tests except one of the DLL tests. The activity ratio with respect to each muscle grade was constant between the CU and the DLL tests. [Conclusion] Our results indicate that trunk muscle activity is affected by gender, and the tests should be selected according to purpose.

    In this study, to clarify the effects of the intervention of somatosensory information on standing posture and performance, we investigated the center of pressure (COP) during standing on different floor surfaces in seven healthy young subjects. Subjects were asked to stand on three different floor surfaces including a hard surface, a patterned indented surface and a foam cushion surface and to perform static and dynamic balance tasks with their eyes open or closed on one leg or both legs. The postural stability was assessed with the COP sway by calculating the standard deviation of the ordinate data of the COP. The postural mobility was assessed with forward, backward and total voluntary maximum displacements from the center of the force plate. The COP sway during the eye-closed condition was increased more than that during the eye-open condition. The total and backward COP displacement decreased significantly during the eye-closed conditions compared with the eye-open conditions. The performance of the static and dynamic balance tasks on the foam cushion was significantly lower than on the other surfaces. Our findings show that changes in the floor surface lead to differences in the stability and mobility of the standing posture in healthy young subjects.<和文要約>健常若年者7名を対象に足底接地面、つまり、床面の違いが立位バランス能力に及ぼす影響について調べた。被験者は立位での静的バランス課題と動的バランス課題を開眼または閉眼、両脚または片脚立位の状況で行った。計測は床反力計上で行われ、床面条件は床反力計の上に何も置かない、足底刺激板、発泡クッションを床反力計の上に設置した3条件とした。静的バランス課題時の動揺量をCOP前後方向の座標データの標準偏差値で算出した。動的バランス課題時の床反力計の中心から前後方向へのCOPの最大移動距離と総移動距離を求めた。閉眼時のCOP動揺量は開眼時に比べ増大し、前方向を除くCOP移動距離は閉眼時で開眼時に比べ有意に低下した。発泡クッション上での成績が静的および動的バランス課題とも最も低かった。今回の研究から、床面の違いが健康若年者の立位バランスに影響を与えることが明らかとなった。

    The application of motor learning is important for the rehabilitation of patients with motor disorders. Although the exercise method of motor learning consists of constant exercise, variable exercise or both, these methods have not yet been verified adequately with regard to the postural control during standing. To investigate the effect of different exercises with visual feed-back information on postural control, we examined the motor performance with regard to the center of pressure (COP) during the standing target-tracking task in six healthy young males. The visual feed-back information of COP from the force plate was provided to subjects by the monitor display. Subjects were asked to move their body in a forward or backward direction to fit and track their own projected COP to the target on the display. The constant exercise group had a scale with the same rate between the COP displacement on the force plate and the COP displacement displayed in the monitor, and the variable exercise group had two scales with different rates. The retention tests were performed after completing the exercises, on the following day, and one week later. The COP data acquired during the tracking task in retention tests were measured, and the gain, the root-mean-square error (RMSE) and the phase shift for the tracking target were computed. In comparison to the constant exercise group, the motor performance of the variable exercise group was better and maintained these effects for a week after the exercise. The findings of this study suggest that differences in exercise methods affect motor performance and/or the retention of motor performance, and that the application of variable exercise for balance training may therefore be beneficial for fall prevention.<和文要約>運動障害をもつ患者のリハビリテーションに運動学習の応用は重要である。運動学習の練習方法には一定練習と多様性練習があるが、異なる練習方法がその後の立位時の姿勢制御に与える影響については十分に明らかにされていない。本研究は視覚的フィードバック情報を用いた練習方法の違いが立位での目標追跡課題の成績に与える影響について健常若年者6名を対象に行なった。目標追跡課題は床反力計上で直立位をとり、モニター画面上に表示された自身の足圧中心(COP)と追跡目標を一致させる視覚誘導型の随意的追跡運動とした。練習方法の違いは床反力計の情報からモニター画面上に表示されるフィードバック情報への割合の違いとした。一定練習は床反力計上のCOP移動量をモニター画面上に同じ大きさで表示し、多様性練習は2種類の異なる割合でモニター画面上にCOPの移動量を表示した。多様性練習群の成績は一定練習群に比べ良好であり、その練習効果は1週間後においても維持されていた。本研究より、練習方法の違いは立位での随意的運動課題の成績及び効果の持続に影響を与え、転倒予防を目的としたバランス訓練への多様性の応用は有効である可能性が示唆される。

    [Purpose] To quantitatively investigate whether the muscle grades of the Curl-up (CU) and Double-leg-lowering (DLL) test show any electromyographic changes in the upper and lower rectus abdominis (RA) as trunk flexors. [Methods] The muscle activities of both the upper and lower RA in thirty healthy young males were recorded during each muscle grade task of two tests. The percentages of maximum voluntary contractions (%MVC) of the upper and lower RA were compared between each muscle grade and test. The %MVC ratios of the upper and lower RA were calculated to investigate the contribution of each muscle site in two tests. [Results] Although there were significant electromyographic differences among muscle grades in the CU test, there was no significant difference in the DLL test. The %MVC from both sites in the CU test were significantly larger in all muscle grades than in the DLL test. However, the %MVC ratio was consistently similar to 1.0 in both tests and the contribution of each muscle site did not differ. [Conclusion] These findings suggest that the three grading tasks used in this manual muscle testing procedure may be insufficient to objectively detect muscle weakness in the trunk flexors. The grading criterion for trunk flexors may need remediation.

    [Purpose] The purpose of this study was to examine the flexion-relaxation phenomenon (FRP) of the lumbar erector spinae muscles (ES) during asymmetric forward bending with lumbar flexion and rotation in healthy young males. [Subjects] Ten healthy young males (mean age 22.0 ± 0.8 [SD] years) participated in this study. [Methods] Subjects were asked to perform three tasks: forward bending without lumbar rotation (Task 1), forward bending with lumbar rotation from the standing position with rotation (Task 2) and forward bending with lumbar rotation from the standing position without rotation (Task 3). Muscle activities from both ES and the lumbar motion were recorded by electromyography and an electromagnetic system. The onset of the FRP was identified with the data from these devices. [Results] The FRP of bilateral ES occurred nearly simultaneously during Task 1. There was a significant difference between the sides of ES for the onset of FRP during Task 2 (p< 0.01). The FRP of the rotated side did not occur during Task 3. [Conclusion] Our findings suggest that the onset and appearance of the FRP of the ES differ between symmetric and asymmetric forward bending in healthy young males. Also, the FRP of the ES is influenced by the rotation direction during asymmetric forward bending.

    Eye and head movements are coordinated during head-free pursuit. To examine whether pursuit neurons in frontal eye fields (FEF) carry gaze-pursuit commands that drive both eye-pursuit and head-pursuit, monkeys whose heads were free to rotate about a vertical axis were trained to pursue a juice feeder with their head and a target with their eyes. Initially the feeder and target moved synchronously with the same visual angle. FEF neurons responding to this gaze-pursuit were tested for eye-pursuit of target motion while the feeder was stationary and for head-pursuit while the target was stationary. The majority of pursuit neurons exhibited modulation during head-pursuit, but their preferred directions during eye-pursuit and head-pursuit were different. Although peak modulation occurred during head movements, the onset of discharge usually was not aligned with the head movement onset. The minority of neurons whose discharge onset was so aligned discharged after the head movement onset. These results do not support the idea that the head-pursuit-related modulation reflects head-pursuit commands. Furthermore, modulation similar to that during head-pursuit was obtained by passive head rotation on stationary trunk. Our results suggest that FEF pursuit neurons issue gaze or eye movement commands during gaze-pursuit and that the head-pursuit-related modulation primarily reflects reafferent signals resulting from head movements.

    Purpose: To understand stair ascent and descent motions, we examined the muscle activities of the vastus medialis (VM) and vastus lateralis (VL) and the ground reaction force (GRF) in the supporting leg of eight young healthy males. We also compared these data with level walking data. Result: The muscle activities of VM and VL during the ascent motion were the highest among the three tasks. The muscle activity ratios of VM and VL during ascending and descending differed from that during level walking. The VM contribution increased in the ascent and descent tasks compared with level walking. There was no significant difference for the vertical GRF among the tasks. Although the anterior-posterior GRF during the ascent motion was similar to that during level walking, the anterior-posterior GRF during the descent motion was different from that of the ascent motion and level walking. Further, the medial-lateral GRF during ascending and descending were different from that during level walking. Conclusion: The results of this study suggest that the increased VM contribution of the supporting leg may be related to the change of medial-lateral GRF during ascent and descent. This finding will add knowledge to the rehabilitation and physical therapy of ascent and descent motions for elderly people and patients with motor disorders.

    The smooth-pursuit system is important to precisely track a slowly moving object and maintain its image on the foveae during movement. During whole-body rotation, the smooth-pursuit system interacts with the vestibular system. The caudal part of the frontal eye fields (FEF) contains smooth pursuit-related neurons that signal eye velocity during pursuit. The majority of them receives vestibular inputs and signal gaze-velocity during passive whole-body rotation. It was asked whether discharge modulation of FEF pursuit neurons during head rotation on the stationary trunk could be accounted for by vestibular inputs only or if both vestibular and neck proprioceptive inputs contributed to the modulation. Discharge modulation during active head pursuit, passive head rotation on the stationary trunk, passive whole-body rotation, and passive trunk rotation against the stationary head were compared. The results indicate that both vestibular and neck proprioceptive inputs contributed to the discharge modulation of FEF pursuit neurons during head movements.

    This study assessed the relationship of each movement direction on the lumbar spine and on the pelvis in regards to lumbar-pelvic coordination in the sitting position. Lumbar and pelvic motions were recorded using a flexible electrogoniometer in 12 healthy subjects during two different tasks in the sitting position. The coordination of the lumbar spine and pelvis was evaluated using the ratio of lumbar and pelvic angles (L/P ratio hereafter) in three motion phases. There were significant differences in the UP ratio values between during the "forward bending" and "rising from a forward flexed position" phases (P < 0.01). The UP ratio value (similar to-0.7) was consistent during the movement from an erect to a slumped sitting position as the lumbar spine and pelvis moved in opposing directions. This study shows that lumbar-pelvic coordination occurs in the sitting position. Lumbar-pelvic coordination is confirmed even if the movement tasks are different in the sitting position, and these findings show that aspects of a particular movement are dependent on the given movement task. These findings may provide greater insight into the kinematic changes involved in lumber-pelvic coordination, and help clinicians implement sitting exercises conducive to the independence of the patient.     [Purpose] To study the effects of chest or lumbar corset restrictions on pulmonary function. [Subjects] Six healthy men aged 28.2 ± 5.3 [SE] yrs participated in this study. [Methods] Pulmonary function tests were perfomed with subjects wearing a plastic chest or a lumbar corset which could regulate corset restriction pressure from 0 mmHg to 60 mmHg. [Results] The chest corset restriction decreased both vital capacity (VC) and expiratory reserve volume (ERV), but not residual volume (RV), when the restrictive pressure of the corset increased to over 40 mmHg. In contrast, the lumbar corset had no effect on VC, ERV or RV. [Conclusion] These findings suggest that the use of the chest corset may suppress respiration.

    The purpose of this study was to evaluate the repeated one-leg heel-rise test method of ankle plantar flexors in MMT with kinetic measuring devices and electromyograms (EMG). Seven healthy young males (age, 23 +/- 2 y) with no prior history of fractures or surgery involving the lower limbs participated in this study. The ankle and knee motions from flexible electrogoniometers (EGM), ground reaction forces (GRF) and center of pressure (COP) from one force plate and the root-mean-square (RMS) and mean-power-frequency (MPF) from EMG in the medial head of the gastrocnemius and soleus muscles during the repeated one-leg heel-rise test. Kinetic and kinematic data from EGM, GRF and COP were unchanged during the tests. Although the RMS in both muscles and the MPF in the soleus muscles were unchanged during this test, the MPF in the gastrocnemius muscles decreased with the number of iterations (r=-0.79, p < 0.001). The MPF of the gastrocnemius muscles in the late phase was significantly lower that in the early and middle phases (respectively, p < 0.05, p < 0.05). Our results show that this repeated motion method estimates muscle endurance rather than the muscle power.

    It is still unclear how rehabilitative intervention assists recovery from strokes. In order to address this question, we made an animal model to examine the recovery process after experimental strokes. The forelimb movements in 2 monkeys retrieving food from a well were analyzed with a videotape recording. A small infarction by coagulation of blood vessels was made in the forelimb, areas after mapping of the primary motor areas. The recovery process was evaluated by measuring the time required for taking food. We used the modified Kluver board with the small diameter in order to restrict the monkeys' compensatory movements such as pronation or supination. After the infarction, the monkeys could not use their impaired hands for the first 2 days. Failure to retrieve food and compensatory movements such as involvement of other joints were often observed during the first week. However, the forelimb function of both monkeys showed no significant impairments on the 6th week, compared to the results before the infarction. Throughout the experiments, the monkeys showed no supination or pronation of the forearm. These results suggest that this animal model with the modified Mover board may be useful for the evaluation of motor deficit.

    The smooth-pursuit system moves the eyes in space accurately to track slowly moving objects of interest despite visual inputs from the moving background and/or vestibular inputs during head movements. Recently, our laboratory has shown that young primates exhibit asymmetric eye movements during vertical pursuit across a textured background; upward eye velocity gain is reduced. To further understand the nature of this asymmetry, we performed three series of experiments in young monkeys. In Experiment 1, we examined whether this asymmetry was due to an unlcompensated downward optokinetic reflex induced by the textured background as it moves across the retina in the opposite direction of the pursuit eye movements. For this, we examined the monkeys' ability to fixate a stationary spot in space during movement of the textured background and compared it with vertical pursuit across the stationary textured background. We also examined gains of optokinetic eye movements induced by downward motion of the textured background during upward pursuit. In both task conditions, gains of downward eye velocity induced by the textured background were too small to explain reduced upward eye velocity gains. In Experiment 2, we examined whether the frame of reference for low-velocity, upward pursuit was orbital or earth vertical. To test this, we first applied static tilt in the roll plane until the animals were nearly positioned on their side in order to dissociate vertical or horizontal eye movements in the orbit from those in space. Deficits were observed for upward pursuit in the orbit but not in space. In Experiment 3, we tested whether asymmetry was observed during head-free pursuit that requires coordination between eye and head movements. Asymmetry in vertical eye velocity gains was still observed during head-free pursuit although it was not observed in vertical head velocity. These results, taken together, suggest that the asymmetric eye movements during vertical pursuit are specific for upward, primarily eye pursuit in the orbit.

    【はじめに】我々は,前回の本学会において足関節底屈筋群の筋力検査法について運動学的解析を行い,正常の判定基準である反復踵上げ動作中の腓腹筋に疲労が起こること,さらに前後方向のCOP(圧中心)移動と床反力の結果から底屈筋力が分散することを明らかにし,その妥当性や問題点について報告した.そこで,今回我々は,反復踵上げ動作による判定に代わり,徒手抵抗による判定を考案し,その妥当性について検討したので報告する.
    【方法】対象は健康若年男子10名(平均年齢22±2歳,平均身長171±6cm,平均体重59±6kg ).全員,過去に下肢の機能傷害を持たないものであった.被検者に書面をもって十分な説明を行い,同意を得た.本研究では,特に重力と徒手による抵抗を明確にして調査した.踵上げ動作に代わる方法として,被検者に完全底屈位での踵上げ肢位を保持させ,検査者は被検者の正面の位置に立った.抵抗を加える手は両骨盤部(骨盤稜)にあてがうようにした.抵抗は下方に向かう方向(床に向かって)に,足関節が底屈するように加えた(改MMT5).このとき,体幹の動揺や股及び膝関節が屈曲しないようにした.底屈筋群測定の腹臥位での段階2プラス(以下MMT2プラス)と踵上げ肢位保持(踵上げ保持)との比較を行った.さらに,他の筋の各段階との比較のため,膝関節伸筋群のMMT5及びMMT3についても調査した.
    足関節底屈筋(腓腹筋)と膝関節伸展筋(大腿直筋)の筋活動をマイオシステム1200(NORAXON社製)を用い,サンプリングタイム1kHzで記録した.さらに,BiodexSystem3を用いてMVC(最大随意収縮)を引き出し,各試行の%MVCを算出し,比較した.それぞれ3試行の平均±sdを求め,統計解析は有意水準0.05以下とした.
    【結果】腓腹筋の%MVCは,改MMT5(113.5±21.0 %)で踵上げ保持(75.2±16.1 %)とMMT2プラス(60.3±16.7 %)より有意に大きかった(P<0.05).しかし,踵上げ保持とMMT2プラスの間に有意な差はみられなかった.大腿直筋のMMT5の%MVC (71.1±25.7 %)はMMT3(14.0±14.3 %)より有意に大きかった(P<0.05).改MMT5での腓腹筋の%MVCは大腿直筋のMMT5のものより有意に大きかった(P<0.05).
    【考察とまとめ】踵上げ動作の反復回数による判定方法は,腓腹筋など底屈筋群に筋肉疲労を引き起こすことが問題であった.そこで,今回我々は足関節底屈筋の筋力評価として徒手抵抗による方法を試行し,最も大きな筋活動を得ることができ,他の段階と区別できることを確認した.今後は,徒手抵抗についてはさらに検討を加え,検者内での比較も行ってゆきたい.

    We studied the influence of hand breaking on sharing effective mass and impact force at sideways falls, since hip fractures in elderly are mainly caused by sideways falls. When the hand contacted the ground before the great trochanter, the effective mass and impact force of the great trochanter were shared on the hand in the both case of one-hand and both-hands contacts. However, when the great trochanter contacted before the hand, the effective mass and impact force were not shared. Consequently, we suggest that hand contact before the great trochanter at sideways fall is shared the effective mass cause of impact force, and contributes to prevent the hip fracture.

    簡易酸素マスク,リザーバー付酸素マスク,簡易酸素マスクと鼻カニュラ併用など,吸入酸素濃度調節ができない器具で酸素吸入を行う場合の酸素流量と吸入酸素濃度の関係を,人工顔モデルを用いて検討した.1回吸気量は400 mlと800 ml,吸気時間は1秒,呼気時間は2秒とした.どの酸素マスクを用いても酸素流量が一定の場合は1回吸気量が少ないほど,1回吸気量が一定の場合は酸素流量が多いほど,吸入酸素濃度は高かった.リザーバー付酸素マスクの吸入酸素濃度は,酸素を高流量流しても,製品カタログに記載されている数値よりかなり低かった.しかし,同マスクを使用すると,簡易酸素マスクと鼻カニュラ併用に比べ,より少ない酸素流量で,同程度の高濃度酸素を吸入させることができた.吸入酸素濃度調節ができない器具で酸素を投与する場合,患者の動脈血分析を行いながら慎重に酸素流量を決めるべきである.

    【目的】
    歩行訓練の前段階として横歩きが少なからず取り入れられており、日常生活においても側方移動は狭い通路や台所作業時などで行われている。また、パーキンソン病患者のすくみ足は、歩行開始時のみならず側方移動時においても顕著にみられる。歩行分析の多くは矢状面上であり、前額面に着目した運動コントロールについては十分に知られていない。歩行開始時の予測的姿勢制御 (APA) に関する研究は足圧中心 (COP) の遊脚肢側および後方への軌跡に着目して報告されているが、遊脚肢側へのCOP変移の機序について筋活動から検討したものはほとんど報告がみられない。従って、本研究の目的は、側方移動の初期相である側方ステップの運動コントロールについて、APAに着目して筋電図学的に検討することである。
    【方法】
    対象は、健常な成人男性9名で、年齢、身長、体重はそれぞれ22.2±1.9歳、171.5±3.3cm、63.5±6.2Kgだった。全被験者右利きだった。足圧の水平分力と垂直分力およびCOPを求めるために2枚の床反力計 (200Hz) を用いた。8チャンネル筋電計 (1KHz) を用いて、両側の中殿筋、長内転筋、長腓骨筋、後脛骨筋から表面電極によって筋活動を計測した。床反力計と筋電計からのデータは同期してA/D変換後パーソナルコンピューターに保存された。被験者は、各足を各床反力計上に乗せ(踵骨中心間距離;身長の11%、両足角14°)、安静立位後随意的に右足を外側方へ前方を注視したままステップした。ステップ幅(身長の13%)が変動しなくなるまで予め練習を行った。ステップ速度は3段階とし、各速度3回ずつランダムに施行した。ステップ時間は、右足部の離床から再び接地するまでとし垂直分力から求めた。離床以前に見られるCOP軌跡の左右成分から、APA amplitude(安静時位置から右方への最大変移)およびAPA duration(右方へ変移して再び安静時位置に戻るまでの時間)を算出した。筋活動の開始時間は、積分筋電図(filter;20-500Hz)から求めた(安静時平均値+2SD)。統計学的検討は、相関係数とt検定を用い、危険率1%未満を有意とした
    【結果および考察】
    COP左右成分に変動がみられたためAPAの判別が困難だったデータを除いて検討した。有効データ数は66だった。COP軌跡から91%にAPAがみられた。ステップ時間が長くなるに従って有意にAPA amplitude、APA duration共に大きくなった。両長内転筋の筋活動はほとんどみられなかった。APA開始時には右後脛骨筋と左長腓骨筋の筋活動がみられ、APA終了時には両中殿筋の筋活動がみられた。従って、遅いステップ程APAが大きく働き、それは足関節の回内および回外筋群によって発生し、ステップ下肢の荷重減少時から股関節外転筋群が活動することが示唆される。この研究で得られた結果は、側方移動の運動療法を考案するための基礎資料となる。

    To investigate the role of protective movements with regard to the head and hip during sideways falling, 40 healthy young males fell to the left onto a sports-mat from a standing position after application of force against the right side of the hip. All falls were recorded on videotapes and subsequently played back to analyze protective movements. During sideways falling, some protective movements were frequently observed. The appearance ratio of non-contact of head to the ground, indicating the extent of the head protection, was 60.5%. The non-contact of the head significantly correlated with rotation of the body and, the motion of the arm to touch the ground, but the first contact near the hip to the ground had no significant correlation with any protective movements. The most likely protective movements' pattern during the sideways falling was rotating to the abdominal position and flexing the body, and reaching the arm to the ground to protect the head during the fall.

    我が国の理学療法士における筋骨格系職業性傷害の実態を把握するとともに傷害発生に関わる要因やその対策を検討した。無作為に抽出した臨床経験2年以上の理学療法士1000名に筋骨格系の職業性傷害に関する質問用紙を送付し,有効回答者637名を対象とした。過去2年以内で業務に支障をきたすほどの筋骨格系職業性傷害を経験した理学療法士は42%と高率であった。特に,傷害を経験した理学療法士の男女別比較では女性で有意に高かった。受傷部位は腰部が69%と最も多く,その要因として移乗介助(34%)の影響が大きかった。また,それは年齢や経験年数に関係なく起こっていた。傷害後,多くの人が身体の使い方を工夫するなど自分自身で対処し,管理者に報告したり,同僚に仕事を依頼する人は少なかった。再発率は受傷者全体の41%と高かった。腰痛などの傷害に対する知識を持っているはずの理学療法士でも筋骨格系の職業性傷害を経験する人は多く,これらの職業性傷害に対する対策の検討が今後必要となろう。

    Fractures in the elderly occur mostly as a consequence of a fall, and are represented by the reduction of the bone strength. Principal sites of fracture are the proximal femur and the spine. A useful parameter in the study of the mechanism of fracture is the factor of risk, being defined as the ratio of the applied load on the skeletal element to the failure load of the bone itself. The failure load of the proximal femur is strongly correlated with bone density, and is influenced by the direction and frequency of the load and the rate of strain. High risk of bone fracture in the elderly is due to low bone density of the femur and spine. The risk of fracture of vertebrae is largely dependent on the strength of the trabecular bone. Fall severity from standing height can be reduced by soft tissue overlying the impact site as a cushion, and by protective mechanisms such as outstretching the arm, rotation of the trunk and muscle-relax. A dominant factor in susceptibility to pathological fracture is metastatic defect. Two biomechanical models are available to estimate the strength of bone exhibiting metastatic defect. First, the failure load may be estimated from the relationship between the size of defects and the reduction of bone strength. Second, the failure load may be predicted by the measurement of the structural geometry of an intact region of the bone using noninvasive imaging, even if the bone displays metastatic defects.

    我々は,高齢者におけるバランス能力改善を効率的に達成するためのトレーニング方法の開発を目指し,健常若年者と健常高齢者を対象として立位における急速運動停止課題,足関節底背屈調節課題,足趾課題を行い,バランス能力への加齢の影響と加齢に関与するバランス要素を調べた.研究の結果,バランス能力改善に欠かせない要素として前脛骨筋と下腿三頭筋の筋出力調整能力,同時収縮と相反性収縮の切り替え能力,足趾やそれに関わる筋の働きがあり,それらは加齢の影響を受けやすいことを明らかにした.それらの能力を改善するためのトレーニングに体性感覚系入力が必要だと考えられる.

    我々は,健常若年成人と健常高齢者を対象としてリーチ動作の繰り返し課題,反復重錘捕球課題,体性感覚系刺激課題を行い,ヒトの姿勢制御がどのようなメカニズムで適応および学習されるのか,加齢が姿勢制御のどの側面に影響を及ぼすかを検討した.研究の結果,運動パフォーマンスの学習に姿勢制御の変化が影響を与える可能性,足関節の運動と下腿三頭筋の筋活動が加齢により影響を受けること,体性感覚系がバランス能力の改善に強く影響を及ぼすことを明らかにした.

    高齢者の立位時の姿勢および運動の不安定性について,健康な若年者と高齢者を対象に視覚誘導型の足圧中心を用いた追跡運動課題を行った.高齢者は与えられえるフィードバック情報に立位バランスは影響を受け,運動の開始だけでなく運動の停止も遅延し,大きな変動性を持つことが明らかとなった.このとき,高齢者の立位バランスへの二関節筋の作用や垂直方向への姿勢戦略が重要な役割を果たすことが示唆された.

    立位時の運動制御と下肢筋活動及び加齢変化について、健康な若年者及び高齢者を対象に足圧中心を用いた視覚誘導型追跡運動課題を行った。運動成績は運動方向、大きさ及び速度の影響を受けることが明らかとなった。特に、運動成績と運動速度の間にU字型の曲線関係がみられ、最適運動速度を持つことが確認された。筋活動の年齢差は膝関節屈筋群にみられ、加齢による姿勢戦略の選択の低下と二(多)関節筋群の活動低下が関与していることが示唆された。

    (1)2頭のサルに対する訓練と手指機能パフォーマンステストの有用性について検討を行い,(2)サルの大脳皮質第一次運動野の上肢領域(主に手指領域)梗塞作成前の上肢動作をVTRにより,(3)第一次運動野の上肢領域を皮質内微小刺激によってマッピング、(4)さらに梗塞作成後の麻痺の状態ならびに機能回復過程について,上肢運動障害,代償動作を比較した.(1)アクリル板で深さの異なる5つの円柱状の穴(Kluver board)を作成した.訓練前のテスト結果では,2頭のサルとも深さが徐々に増加するにつれ所要時間は増加した.(2)サルの把握動作は,浅い穴では栂指とII指の間にリンゴを挟み,穴が深くなるとIIからIV指を穴の中に差込み,屈曲して掻き上げる方法であった.これは2匹のサルで同様であった.(3)GOFとネンブタール麻酔下で開頭し優位半球の第一次運動野(A12L18)に微小電極を用いて刺激し,マッピングを行った。その結果、中心溝の吻側に内側から外側にかけて,肩領域、肘〜前腕領域,手〜手指領域の順序で筋収縮が観察された.手指領域は,運動野の内側から外側にかけて第Vから第II指の屈曲、伸展が順に誘発された.これらの結果は,従来の運動野上肢領域の体部位局在に一致していた.(4)マッピング終了後,手指領域に梗塞を作成した.両サルとも術後翌日に対側上肢に麻痺を認めた.術後は麻痺側を使うも失敗が多く非麻痺側を用い,数日間は麻痺側を使わなかった.麻痺側を使う際,隣接する肘関節と肩関節に麻痺による運動障害を代償する動作が確認され,特に,肘や肩関節を用いて手先を前後に動かす,身体を傾けるまたは移動する行為が見られた.運動障害からの機能回復が見られると,所要時間の減少とともに代償動作の頻度も減少し,術後1ヶ月と術前でほぼ変わらない成績であった.
