English, Scientific journal, Ultraviolet B (UVB) exposure induces epidermal hyperplasia, wrinkle formation, and stress-associated responses; however, practical topical countermeasures remain limited. This exploratory proof-of-concept study evaluated whether topical amnion-derived mesenchymal stem cell conditioned medium (AMSC-CM) attenuates chronic UVB-induced photodamage in hairless mice. Male Hos:HR-1 hairless mice were exposed to escalating UVB doses three times per week for 10 weeks and received no topical treatment, α-minimum essential medium (αMEM) vehicle gel, or AMSC-CM gel immediately after each irradiation session; a nonirradiated group served as a control. Wrinkle severity was graded on endpoint dorsal photographs by a masked evaluator, and skin sections were analyzed for epidermal and dermal thickness and epidermal p21 immunoreactivity. UVB exposure increased wrinkle scores, epidermal thickness, and epidermal p21 positivity relative to nonirradiated controls. Compared with UVB alone, AMSC-CM significantly reduced wrinkle severity (mean score 2.00 vs. 4.50; Holm-adjusted p = 0.035). By contrast, epidermal thickness and epidermal p21 positivity showed directional but nonsignificant reductions after Holm correction. These findings support wrinkle attenuation as the primary statistically supported outcome of this exploratory proof-of-concept study and suggest that topical AMSC-CM may modulate chronic UVB-induced photodamage under controlled dosimetry, while secondary histological and p21-related observations remain preliminary. - Flap Selection in Pressure Ulcer Reconstruction Using a Longitudinal Archive of Ten Representative Cases for the Board Certification Examination of the Japan Society of Plastic and Reconstructive Surgery

Wounds in the elderly are frequently recalcitrant and chronic as a result of the effects of skin aging and associated complications. The objective of this study is to utilize an α‐Klotho knockout (KO) mice wound model to assess the capacity of amnion‐derived mesenchymal stem cells (AMSCs) to facilitate wound healing in aging skin.
Methods
AMSCs were applied topically to the wound after extraction and gelatinization of the conditioned medium (CM). Animal experiments were performed with two distinct mouse strains: α‐Klotho KO mice and wild‐type mice. Full‐thickness skin defect models with a diameter of 8 mm were created by incising the skin on the left and right sides of the dorsum. On day 8 after wound creation, the mice were sacrificed, and wound tissue was collected for analysis through histological and immunohistochemical evaluations, as well as through quantitative polymerase chain reaction.
Results
The topical application of CM gel to wounds of α‐Klotho KO mice demonstrated that wound healing was significantly higher than that observed in control, reaching the wound closure rate of wild‐type mice on day 8. Additionally, gene expression analysis of wound tissue indicated that AMSC‐CM may regulate extracellular matrix formation and fibrosis. Moreover, histological analysis indicated that AMSC‐CM may facilitate wound contraction of aging skin wounds of α‐Klotho KO mice by inducing myofibroblast differentiation and promoting granulation and collagen formation, which are the primary components of the extracellular matrix.
Conclusions

English, Scientific journal, Objective: Drugs regulating hypoxia-inducible factor (HIF)-1α have not been investigated for wound healing in lymphedema. Therefore, we examined the effects of drug modulation of HIF-1α activity for wound healing in our previously developed mouse model of nonirradiated hind limb lymphedema. Approach: Mouse hind limb lymphedema models (n = 17) and a sham group (n = 6) were created using 8- to 10-week-old male C57BL/6N mice. Mice with hind limb lymphedema were randomized into experimental groups receiving roxadustat, 3-(5'-hydroxymethyl-2'-furyl)-1-benzylindazole (YC-1), or dimethyl sulfoxide and were given intraperitoneal injections every 2 days for up to 2 weeks. Four days after the surgery, an 8-mm diameter full-thickness skin wound was created in the hind limb. The number of days required for wound closure and the percentage of wounds closed were measured. Skin samples taken at wound creation were evaluated by histological and molecular analysis. Results: Administration of roxadustat accelerated wound healing, whereas YC-1 delayed it, with a significant decrease and increase in skin thickness, respectively. The relative mRNA expression of Hif1α, matrix metalloproteinase-3, and interleukin-6 was significantly higher in the roxadustat group and that of metalloproteinase-9 was significantly lower in the roxadustat group compared with the control group. Innovation: This study is the first to demonstrate delayed wound healing in a mouse model of hind limb lymphedema and the first to demonstrate the promotion of significant wound healing through the use of roxadustat. Background
Klippel–Trenaunay syndrome (KTS) is a rare slow-flow combined vascular malformation with limb hypertrophy. KTS is thought to lie on the PIK3CA-related overgrowth spectrum, but reports are limited. PIK3CA encodes p110α, a catalytic subunit of phosphatidylinositol 3-kinase (PI3K) that plays an essential role in the PI3K/AKT/mammalian target of rapamycin (mTOR) signaling pathway. We aimed to demonstrate the clinical utility of targeted next-generation sequencing (NGS) in identifying PIK3CA mosaicism in archival formalin-fixed paraffin-embedded (FFPE) tissues from patients with KTS.
Results
Participants were 9 female and 5 male patients with KTS diagnosed as capillaro-venous malformation (CVM) or capillaro-lymphatico-venous malformation (CLVM). Median age at resection was 14 years (range, 5–57 years). Median archival period before DNA extraction from FFPE tissues was 5.4 years (range, 3–7 years). NGS-based sequencing of PIK3CA achieved an amplicon mean coverage of 119,000x. PIK3CA missense mutations were found in 12 of 14 patients (85.7%; 6/8 CVM and 6/6 CLVM), with 8 patients showing the hotspot variants E542K, E545K, H1047R, and H1047L. The non-hotspot PIK3CA variants C420R, Q546K, and Q546R were identified in 4 patients. Overall, the mean variant allele frequency for identified PIK3CA variants was 6.9% (range, 1.6–17.4%). All patients with geographic capillary malformation, histopathological lymphatic malformation or macrodactyly of the foot had PIK3CA variants. No genotype–phenotype association between hotspot and non-hotspot PIK3CA variants was found. Histologically, the vessels and adipose tissues of the lesions showed phosphorylation of the proteins in the PI3K/AKT/mTOR signaling pathway, including p-AKT, p-mTOR, and p-4EBP1.
Conclusions
The PI3K/AKT/mTOR pathway in mesenchymal tissues was activated in patients with KTS. Amplicon-based targeted NGS could identify low-level mosaicism from low-input DNA extracted from FFPE tissues, potentially providing a diagnostic option for personalized medicine with inhibitors of the PI3K/AKT/mTOR signaling pathway. - 皮膚欠損用創傷被覆材Sorbactコンプレスを用いた広範囲熱傷デブリードマン後の局所療法

English, Scientific journal, ABSTRACT: Arteriovenous malformation (AVM) sometimes causes hemorrhage that can be fatal. We report a case of AVM of the tongue with bleeding that was treated by semi-emergent sclerotherapy with polidocanol. A 33-year-old woman presented with Schobinger stage III AVM of the tongue. Sclerotherapy with 3% polidocanol foam was performed under general anesthesia using curved intestinal forceps to clamp the root of the tongue for control of blood flow. Postoperatively, there was no further bleeding from the lesion. Three subsequent sclerotherapy sessions with polidocanol were performed, and there was a marked reduction in the size of the lesion. The lesion has remained well controlled in the year since the last sclerotherapy session.

遊離筋皮弁移植を行った糖尿病足潰瘍24例(男23例、女1例、平均年齢57.9歳)について検討した。初診時のHbA1c値は平均9.3%で、使用した皮弁は遊離前外側大腿皮弁15例、遊離広背筋弁9例であり、CT、血管造影による血流評価で虚血を認めた12例のうち、3例は血管内治療を、他の9例はバイパス手術を先行し、動脈では端側吻合を行った。その結果、皮弁は虚血のない12例、血管内治療後に再建した3例、バイパス術後に再建した7例の計22例(92%)で生着した。残りの2例は糖尿病性腎症による透析例で、バイパス血管を移植床にして遊離筋弁移植術を行い、いずれも一旦は生着したもののバイパス血管の閉塞により広背筋弁が壊死した。皮弁の壊死には石灰化よりも虚血が深く関与していると考えられ、術前には血流評価を行い、虚血例では筋皮弁手術に先行して血行再建を行うことが重要であることが示唆された。, 克誠堂出版(株), Japanese - 膝周囲の虚血性潰瘍に対する自家静脈移植と遊離広背筋移植の同時再建術に際して術中血流評価を行った1例

症例は71歳男性で、当科受診7ヵ月前、打撲を契機に右下腿瘢痕に潰瘍が出現した。近医整形外科を経て、前医形成外科を受診した。同医での造影CTにてバイパス血管の血流は保たれているものの、膝窩動脈の閉塞、膝周囲の血流不全を認めた。大腿切断によるADLの著明な低下が危惧されたため下肢温存を目的に当科を紹介された。膝周囲の血流改善と潰瘍閉鎖目的に、自家静脈移植術と遊離広背筋弁手術を計画した。また、本術式により遊離筋弁を含めた膝周囲の血流が増加することで足部への血流が低下する可能性があるため、血流計を用いて術中に血流評価を行う方針とした。自家静脈移植後、移植自家静脈、胸背動脈、膝窩動脈の血流が保たれていることを確認し、遊離広背筋弁手術を行った。術後2年3ヵ月現在、筋弁・分層植皮とも良好に経過し、自力歩行で外来通院している。, (一社)日本形成外科学会, Japanese - 当院における洗浄液周期的自動注入機能付き陰圧創傷治療システムの使用経験
