Background

Primary prevention of depressive symptoms among independent older adults is a critical public health challenge. Lifestyle factors and social support networks are increasingly recognized as pivotal determinants of mental health in this population. However, few longitudinal studies have examined these relationships, and little attention has been directed to gender-based differences. We investigated the associations between lifestyle factors and social support networks and the depressive symptoms of community-dwelling independent older adults in a sex-stratified 3-year longitudinal study.

Methods

This study used data from the Japan Gerontological Evaluation Study. Participants were functionally independent individuals aged 65 years and older. Our final analysis included data for 6,929 individuals collected between 2019 and 2022. A logistic regression analysis was conducted with depressive symptoms in 2022 as the dependent variable. Lifestyle factors and social support networks were treated as independent variables, with age, chronic diseases, and household income included as covariates. The effects of interactions between lifestyle factors and social support networks on depressive symptoms were also analyzed.

Results

The prevalence of depressive symptoms was 10.7% in men and 11.6% in women in the 3-year follow-up. The incidence rate of newly developed depressive symptoms (per 1,000 person-years) was 35.6 for men and 38.8 for women. Sex-based differences were found in lifestyle and social support network factors associated with depressive symptoms. In men, walking for more than 30 min per day, insomnia, and emotional support were significantly associated with the risk for depressive symptoms. In women, walking for more than 30 min per day, insomnia, and instrumental support were significantly associated with the risk for depressive symptoms. No interaction effects between lifestyle factors and social support networks on depressive symptoms were observed in either men or women.

Conclusion

These findings underscore the importance of preventing depressive symptoms among older adults, particularly through the lens of gender-specific differences in lifestyle factors and social support networks associated with depressive symptoms. This study highlights the potential for developing targeted and effective primary prevention strategies by identifying modifiable determinants of depressive symptoms.

Examining Loneliness: A Comparative Analysis of Face-to-Face, Telephone, and Online Communication among Japanese Young Adults

Scientific journal, (1) Background: Loneliness is inherently linked to social connections, with interpersonal communication playing a pivotal role. Despite this connection, limited research exists on the relationship between loneliness and communication among young adults. This study investigates the correlation between face-to-face, telephone, and online communication frequencies and loneliness among individuals in their 20s. (2) Methods: Using a dataset from a nationwide survey conducted by the Japanese Cabinet Office, this study focuses on 1812 respondents aged 20–30, selected from a random sample of 20,000 individuals aged 16 and older across Japan. (3) Results: A Multivariate Logistic Regression Analysis reveals a significant association between communication frequency and loneliness, even after adjusting for demographic characteristics. Notably, decreased communication frequency across all modalities correlated with increased loneliness. Online communication exhibited the highest impact, followed by face-to-face communication, with phone call communication ranking last. (4) Conclusions: This study emphasizes the importance of seamlessly integrating social networking service (SNS)-based communication with various forms of social interaction. A well-balanced integration of these approaches is crucial for mitigating loneliness among young individuals and promoting positive mental health outcomes.

Empowering Community Health Workers in Japan: Determinants of Non-Communicable Disease Prevention Competency

Scientific journal, Background: Community health workers (CHWs), hailing from the general populace, play a pivotal role in fortifying healthcare systems, with a primary focus on mitigating non-communicable diseases (NCDs) and elevating overall life expectancy. To assess the aptitude of CHWs in NCD prevention, we introduced the Community Health Workers Perceptual and Behavioral Competency Scale for preventing non-communicable diseases (COCS-N). This study examines the multifaceted interplay of individual and community factors that influence CHWs’ COCS-N scores. Methods: The research design is a secondary analysis using data from a self-administered questionnaire survey of 6480 CHWs residing in municipalities across Japan, which obtained 3120 valid responses, between September to November 2020. The COCS-N was employed as the dependent variable, while the independent variables were individual-related factors, including years of community health work, health literacy, and community-related factors, such as CHWs’ sense of community. To ascertain the significance of associations between individual and community factors and CHWs’ competency, an analysis of covariance (ANCOVA) was utilized to compare the three groups Q1/Q2/Q3 by low, medium, and high scores on the COCS-N scale. Statistical significance was considered to be indicated by a p-value of less than 0.05. Results: The ANCOVA analysis revealed that three factors were significantly linked to CHWs’ competence. These comprised individual factors: “years of CHWs” (mean ± SD Q1: 6.0 ± 6.0, Q2: 7.8 ± 7.0, Q3: 8.2 ± 7.7, p < 0.001) and “health literacy” (Q1: 27.7 ± 6.6, Q2: 30.4 ± 6.9, Q3: 33.8 ± 7.8, p < 0.001), as well as a community factor: “Sense of community” (Q1: 14.8 ± 3.7, Q2: 16.5 ± 3.5, Q3: 18.2 ± 3.6, p < 0.001). Conclusions: Our finding is that a positive association was derived between COCS-N scores and certain determinants. Notably, “years of CHWs” and “health literacy” in the individual domain, along with the “Sense of community” in the communal context, were firmly established as being significantly associated with CHWs’ competency. Consequently, CHWs need training to increase their “health literacy” and “sense of community”, to acquire high competency in NCD prevention, which will lead to the empowerment of CHWs and maintain their motivation to continue.

Determinants of Self-Rated Health Disparities among Independent Community-Dwelling Older Adults: An Age-Stratified Analysis

Scientific journal, In response to the distinctive healthcare requirements of independent, healthy, community-dwelling older adults in Japan and other developed countries with aging populations, the current study examined the differences in factors associated with self-rated health (SRH) between the following two age groups: young–old (65–74) and old–old (75 and above). Age-stratified analysis was used to provide a comprehensive understanding of the unique health challenges faced by these demographic segments and to inform the development of targeted interventions and health policies to improve their well-being. The results of a cross-sectional study of 846 older adults in Yokohama, Japan, who completed self-administered questionnaires, revealed that high SRH was consistently linked with the low prevalence of concurrent medical issues in both age groups (<75 and ≥75) (β: −0.323, p < 0.001 in the <75 group; β: −0.232, p < 0.001 in the ≥75 group) and increased subjective well-being (β: 0.357, p < 0.001 in the <75 group; β: 0.244, p < 0.001 in the ≥75 group). Within the ≥75 age group, higher SRH was associated with more favorable economic status (β: 0.164, p < 0.001) and increased engagement in social activities (β: 0.117, p = 0.008), even after adjusting for age, sex, and economic status. These findings may inform the development of targeted interventions and policies to enhance the well-being of this growing population in Japan and other developed countries.

本研究は、地域包括支援センター（以下、地域包括）の保健師の地域づくりに関する専門能力評価尺度を開発し、信頼性、妥当性、有用性を検証するものである。地域包括においては、保健師、社会福祉士、主任介護支援専門員が配置され、三職種協働で地域づくりに取り組んでいる。三職種の協働が求められる地域包括において、保健師は専門性を強調する以前に地域包括の職員として共同活動を重視すること、専門性があいまいになり、専門性が発揮されていても、専門性の自覚やプライドが高められないことが指摘されており、保健師が専門能力を認識する困難さが生じていることが指摘されている。そこで、本研究は、地域包括という三職種の協働が求められる職場において、保健師の地域づくりに関する専門能力を構成しているものは何か、専門能力を何によって評価するのかを明らかにする。
今年度は、昨年に引き続き、地域包括の地域づくりに関する文献検討を継続した。また、地域包括の管理者を含めた地域包括の保健師経験10年以上の実践能力の高い保健師及び市町村主管部門保健師を対象にしたインタビュー調査を行うにあたり、協力が可能な対象者の把握に努めた。結果、直営型、委託型の地域包括に所属する約5名の対象者を把握することができた。
今後は、委託型地域包括の主管部門の保健師にインタビューについて、協力可能な対象者を探す。インタビューの実施に向けては、地域の新型コロナウィルス感染症の流行状況に応じて、Zoomを用いるなど方法を検討して実施する。

本研究の目的は、経口免疫療法をうける食物アレルギー（Food Allergy:以下FA）をもつ子どもの療養行動の習得をねらいとして、情報通信技術（Information and Communication Technology、以下ICT）を活用したFAをもつ子どもの体調管理アプリ「わたし・ぼくのカルテ」の開発と検証を行うことである。本研究において、療養行動の習得とは、原因食物の摂取に伴う誘発症状の内容と重症度、シックデイの対応、エピペンの管理及び手順の知識・技術を身につけ、自らの判断で正しい行動が取れるようになることと定義する。
本研究の課題は4つである。
課題１：体調管理アプリ作成に向けた文献検討。課題２：体調管理ツール作成に向けた子どもへの面接調査。課題３：子どものためのICTを活用した体調管理アプリの試作作成。課題４：修正版体調管理アプリの検証。
2021年度は,課題1について取り組んだ.FAの療養支援の実態が特定されると,親が行っている療養行動のうち,子どものセルフケア能力に応じた体調管理ツールを検討するための示唆が得られると考えた.そこで,課題2の前に,FAの療養支援の内容を特定する研究課題に修正を行った.本邦におけるFAの療養支援に関する研究のほか,FAに関するテキストやFAに関する正しい情報を発信しているホームページやリーフレットの内容,諸外国の諸外国の研究も含め,FAの療養支援の項目の特定に取り組んだ.

The contents of a crisis on the life and risk management among older people living alone

The purpose of this research was to clear the contents of a crisis on the life and the risk management of senior citizens living alone. A mail survey in 2 autonomous bodies in 2016 was put into effect and collected 4,292 cases were analyzed.
The items many senior citizens feel a crisis were "accident" "fall at outdoor and accident" "decline of a mind and body" "the living obstacle by which it's for no water and a blackout" "being dementia". There were a lot of items which feel that senior citizens living alone was worried compared with person of the other household forms. More than 70 % of person was putting health care, crime prevention and fire prevention into effect for handle to a crisis. Senior citizens living alone were putting the risk management for which a human network was utilized into effect, but they had problems of shortage of physical environment setting for disaster or fires.
