English, Scientific journal, INTRODUCTION: Reading disorders caused by homonymous visual field defects (HVFDs) have a significant impact on a patient's quality of life. However, no review has been conducted to evaluate the available evidence on the effects of rehabilitative interventions on reading disorders caused by HVFDs. Thus, the aim of this study was to systematically evaluate the effects of rehabilitative interventions on reading disorders caused by HVFDs. METHODS: We searched the MEDLINE/PubMed, Cochrane Library, ClinicalTrials.gov, CINAHL, and ScienceDirect databases for relevant articles. Relevant search terms were used to identify reports of randomized controlled trials or randomized crossover trials published between January 1990 and December 2021. Only studies that included reading-speed-related outcomes were analyzed. Risk of bias was assessed using the PEDro scale. Meta-analysis was conducted using a random-effects model, and standardized mean differences (SMD) and 95% confidence intervals (CIs) were calculated. Heterogeneity was assessed using the Ι2 statistic. RESULTS: Nine studies were included in the meta-analysis. The results showed that rehabilitative interventions significantly improved reading disorders caused by HVFDs (SMD = 0.30; 95% CI 0.08-0.51; P < 0.01; Ι2 = 0.0%). Subgroup analysis showed that reading training significantly improved reading disorders (SMD = 0.35; 95% CI 0.05-0.66; P = 0.02; Ι2 = 0.0%). CONCLUSION: Reading disorders caused by HVFDs can be improved through rehabilitation. In addition, reading training for the improvement of eye movement and fixation to compensate for foveal and parafoveal visual field defects may improve reading speed. - Prefrontal activation during simulated driving in people with schizophrenia: A functional near-infrared spectroscopy study.

English, Scientific journal, People with schizophrenia (PWS) could be at risk when driving, yet this remains to be confirmed. In this study, we used functional near-infrared spectroscopy (fNIRS) and a driving simulator to assess potential driving skill difficulties as reflected by brain activity in PWS and compared them with those of healthy controls (HCs). Twenty PWS and 20 HCs were evaluated. Four tasks were performed: 50-kph and 100-kph sudden braking and 50-kph left and right curve tasks. The hemodynamic activity and driving performance of the two groups were compared. No significant differences were found in the performance of the four tasks. However, significant differences in hemodynamic activity were observed in the left and right dorsolateral prefrontal cortex (DLPFC) during the 100-kph sudden braking task. In addition, a significant negative correlation was found between brake reaction time and brain activity in the left DLPFC during the 100-kph sudden braking task in both groups. The brain mechanisms involved in processing the mental load associated with driving a car are possibly similar in PWS and HCs. Our results suggest that PWS may be able to drive their vehicles safely in the community. - Identifying subgroups based on self-assessment of ability in patients with schizophrenia and its relationship with vocational outcomes.

English, Scientific journal, Background/Objectives: People with schizophrenia have defective self-assessment of ability (i.e., loss of introspective accuracy [IA]). Although previous studies grouped people according to the degree of IA, the clinical features of these subgroups have not been clarified. Additionally, the determinants of outcomes depending on self-assessment remain unknown. We aimed to identify the clinical features that can help distinguish these subgroups and whether the determinants of vocational outcomes differed between the groups. Methods: The self-assessment ability of 100 people with schizophrenia was examined and categorized as accurate, over-, or under-estimators. Multiple discriminant analysis was also performed. After demonstrating statistical validity, the relative effects of positive and negative symptoms, cognitive function, and level of IA on vocational outcomes were also examined for each subgroup. Results: The symptoms that particularly explained the differences between these subgroups were positive and negative (expressing factors) symptoms: p < .001. Using the determinants of vocational outcomes in each subgroup, the over-estimator subgroup was characterized by positive symptoms: p = .025, the under-estimator group, by the underestimation of their own ability: p = .042, and the accurate estimator group, by cognitive function: p = .006. Conclusion: Reduced IA can be a core mediator of various symptoms. Thus, tailoring the target and strategy of interventions for vocational outcomes according to the accuracy and quality of IA is important in clinical settings. - Association of negative symptom domains and other clinical characteristics of schizophrenia on long-term hospitalization

English, Scientific journal, Background: Recent studies suggest five domains for negative schizophrenia symptoms: anhedonia, asociality, avolition, blunted affect, and alogia. Avolition has been considered a characteristic symptom in community-dwelling patients. However, few studies have explored the association of these symptoms with long-term hospitalization.Purpose: This study explored the relative association of each of the five domains of negative symptoms in two groups of patients with schizophrenia: long-term hospitalized and community-dwelling patients.Methods: Participants included 56 long-term inpatients and 111 community-dwelling patients at Nasukougen Hospital in Japan. The nearest neighbor matching within caliper was used. After matching participants by age, sex, disease duration, and years of education, each group was assigned 30 participants. Model 1 was analyzed with a logistic regression analysis with 5 subdomains as independent variables. Model 2 was analyzed after adding positive symptoms, cognitive function, functional skills, and functional outcomes to the subdomains that were significant in model 1.Results: The results indicated that asociality was significantly associated with long-term hospitalization. When the characteristic clinical factors of schizophrenia were added, asociality, daily living skills, and social and role functions were found to be characteristic of long-term hospitalization. Among the negative symptoms, lack of social motivation was more characteristic among the group with a poor prognosis.Conclusions: Of the negative symptoms associated with long-term hospitalization, asociality, lack of social motivation, rather than avolition was found to be most strongly associated with community-dwelling patients' functional outcomes. Further studies are required to establish a causal association as it may have therapeutic implications. - Determinants of Recreational Outcomes in Schizophrenia: An Exploratory Study Focusing on Gender Differences.

English, Scientific journal, While recreation is impactful for recovery in schizophrenia, no study has examined the effects of schizophrenic symptoms on recreational outcomes. This study examined the determinants of recreational outcomes based on gender. We investigated the relationship between recreational outcomes, positive and negative symptoms, cognitive function, and factors such as negative self-evaluation and evaluation by others. Motivation had a significant impact on both men and women. However, participation in recreational activities was impacted by negative self-assessment for men and evaluations from others for women. English, Scientific journal, Background: The functional outcomes for many patients with schizophrenia remain poor, and the specific determinants for and pathways to functional outcomes are not well understood to date. It is unknown whether major determinants of outcomes are achieved via a motivated single pathway or by the motivation and capacity defined in dual pathways. This study investigated whether different aspects of functional outcomes, such as residential, social, and vocational outcomes, are the main determinants of the experience factors for negative symptoms or whether the experience factors and cognitive function are the determinants. Method: We enrolled 107 patients with schizophrenia. The Social Functioning Scale domains were used to examine whether a single or dual pathway is appropriate for each domain based on the model fit using structural equation modeling. Results: The model goodness of fit criterion showed a dual pathway for residential and vocational outcomes. In contrast, social and recreational outcomes showed a single pathway. Conclusion: The major determinants were clearly different for each outcome. Therefore, we emphasize the importance of using different treatment strategies for each outcome. Irrespective of the factors approached, social and recreational outcomes should ultimately focus on motivation. The findings also suggest that interventions should be combined for vulnerable cognitive functions and motivational interventions for residential and vocational outcomes. - Impact of Negative Symptom Domains and Other Clinical Characteristics on Functional Outcomes in Patients with Schizophrenia

English, Scientific journal, Negative symptoms of schizophrenia have generally been defined using five factors; however, few studies have examined the relationship between these five factors and functional outcomes. In addition, there is no definitive conclusion regarding the association between negative symptoms and various aspects of functional outcomes (daily living, social, and vocational). This study is aimed at examining the relationship between these five domains of negative symptoms and different functional outcomes. Patients diagnosed with chronic schizophrenia ( ) were selected for the evaluation. We used the Brief Negative Symptom Scale to assess negative symptoms, the Brief Psychiatric Rating Scale to assess positive symptoms, the Schizophrenia Cognition Rating Scale to assess cognition, and the Evaluative Beliefs Scale (negative self-assessment) to assess psychological factors. We analyzed their relative impact on Social Functioning Scale domains using hierarchical multiple regression analysis. Concerning the relationship between daily living and negative symptoms, cognitive function showed the highest association with residential outcomes, such as self-care and shopping, while avolition appeared to show an additional contribution; however, for recreational outcomes, avolition showed the main association, whereas cognitive function showed no additional contribution. For social outcomes, asociality and negative self-assessment showed the main associations, while vocational outcomes were determined by both cognitive function and multiple negative symptoms, such as avolition, anhedonia, asociality, and alogia. Since negative symptom domains appear to differentially impact each outcome, specifically daily living outcome, it is important to evaluate the residential outcomes and recreational outcomes separately. Overall, the present study points to the importance of formulating psychosocial treatment strategies specific for each type of preferred outcome in patients with schizophrenia.

English, Scientific journal, Negative symptoms in schizophrenia can be explained by two factors, experience and expression, which should be considered as separate symptoms. However, their relative role in real world functioning has not been clarified. This study aimed to clarify the relative role of experience and expression in real world functioning, and their associations with positive symptoms and neurocognitive function. In total 107 patients with schizophrenia were enrolled. The Social Function and the Brief Negative Symptom Scales were used to evaluate the real world functioning and negative symptoms, respectively. The analysis showed that the expression factor influenced real world functioning through the experience factor. In addition, neurocognitive function affected real world functioning independent of expression and experience. These results suggested that interventions that enhance the communicative expression, such as oxytocin therapy and SST, could successfully be combined with motivational interventions, such as the cognitive behavioral therapy. In addition, real world functioning could be improved by separately performing interventions to compensate for vulnerabilities in neurocognitive function and motivation. - 長期入院統合失調症者の対人関係機能に陰性症状が与える影響 対人関係機能の障害と陰性症状の重症度に関する探索的研究

Japanese, 長期入院者を精神障害者社会生活評価尺度(LASMI)で評価した対人関係機能の障害とBrief Negative Symptom Scale(BNSS)で評価した陰性症状の重症度との関連を検討し、障害の有無を判別するBNSS得点カットオフ値を探索的に算出した。対象者は1年以上精神科病院に入院する統合失調症者で、分析には順序ロジスティック回帰分析、ROC曲線を使用した。結果、分析対象は53名となりBNSSと関連が認められカットオフ値が20以下であった項目は自主的な付き合い、30〜40の範囲内にあった項目は自発性、友人付き合い、40〜50にあった項目は主張、応答、発話の明瞭さであった。障害の有無を判別する陰性症状の重症度は、各対人機能によって異なる可能性がある。(著者抄録) - 長期入院統合失調症者の社会機能に陰性症状が与える影響 失快楽症、非社会性、意欲の低下、感情鈍麻、言語の貧困と社会機能との関連

Japanese, 本研究の目的は、陰性症状の5つの因子である失快楽症、非社会性、意欲の低下、感情鈍麻、言語の貧困と社会機能との関連を検討することである。分析対象者は当院に入院する長期入院統合失調症者51名であった。従属変数を精神障害者社会生活評価尺度下位項目、独立変数をBrief Negative Symptom Scaleの下位項目とし、Spearmanの順位相関係数および重回帰分析にて分析した。結果、日常生活、労働には意欲の低下、対人関係には非社会性、感情鈍麻が有意に寄与していた。自己認識についてはどの因子とも関連していなかった。陰性症状の中でも意欲の低下、非社会性、感情鈍麻の改善に取り組むことが、退院支援に向けて有用であることが示唆された。(著者抄録) - 統合失調症の社会機能と陰性症状サブドメインとの関連 MEDLINEおよび医学中央雑誌に基づくシステマティックレビュー

Japanese, 多くの統合失調症者の社会機能は依然貧弱なものである。近年、新たな治療法の開発に向け陰性症状が重要視されつつある。本研究の目的は陰性症状のサブドメインと社会機能との関連について研究動向を把握することである。MEDLINE、医学中央雑誌にて2010〜2017年に発行された医学文献を検索した結果、17本の研究が対象となった。発症初期や前駆期の段階でさえも失快楽症(anhedoniaアンヘドニア)、非社会性、意欲の低下からなる体験症状因子が社会機能の回復を阻害する要因になっていた。体験症状因子に比べると感情鈍麻や会話の貧困からなる感情表出因子と社会機能との関連は弱いことが明らかとなった。しかし、感情表出因子が社会機能と関連していないわけではなく、(1)感情表出因子は社会機能に間接的に影響を与えること、(2)社会機能を日常生活機能や家族機能など個別に検討すると感情表出因子との関連が強い機能が存在することが示された。これら(1)、(2)の結果は部分的な解析に留まるため、経路分析や多変量解析などの適切なデザインを組んだ上でさらなる報告を積み重ねていく必要がある。(著者抄録) - Characteristics of negative symptoms and relationship with factors of discharge difficulty in long-term hospitalized person with schizophrenia : Comparison with community dwelling person with schizophrenia

Japanese, 統合失調症の社会機能と、陰性症状の5つの症状(失快楽症、非社会性、意欲の低下、感情鈍麻、会話の貧困:サブドメイン)の関連を明らかにすることを目的に、精神科単科病院である当院へ外来通院していた当該患者83名(男性35名、女性48名。平均年齢49.8±14.1歳)を対象に、社会機能については精神障害者社会生活評価尺度(LASMI)、陰性症状についてはBNSS(Brief Negative Sympton Scale)をそれぞれ用い、評価を行った。その結果、社会機能の「日常生活」「労働」「持続性・安定性」にはサブドメインの「意欲の低下」、「対人関係」には「非社会性」「感情鈍麻」が、それぞれ有意に影響していた。一方、社会機能の「自己認識」については、いずれのサブドメインとも関連は認められなかった。 - 統合失調症者の認知機能障害と家族負担との関連

Japanese, 統合失調症者の認知機能障害が家族のどのような負担と関連するのかを明らかにすることを目的に、精神科単科病院の外来に通院する、退院後6ヵ月以上在宅生活を続ける本人(男性19名、女性14名、平均年齢43±11.7歳)および家族33組を対象に、SCoRS-J(統合失調症認知機能評価尺度日本語版)、NFBAS(那須高原病院版家族負担評価尺度)を用いた調査を行った。その結果、NFBASの因子1(健康、日常生活)、因子2(問題行動、対人関係)、因子4(支援、サポート)とSCoRS-Jに有意な相関が認められ、統合失調症者の認知機能は家族負担の関連因子となることが示唆された。

【目的】現行の道路交通法では、統合失調症者(以下、対象者)に対し「自動車等の安全な運転に必要な能力を欠くこととなるおそれがある症状を呈しないもの」に限って免許を認めるという規定を設けている。しかし、実際には対象者のどのような特徴が安全に運転する能力を障害しているかを判断する法的な根拠は乏しい。よって、本研究では、①Functional Near-Infrared Spectroscopy(以下、fNIRS)を用いて、自動車教習所内コースを運転した際の脳活動を計測し、健常者との差異を明らかにする、②実世界で対象者が運転している自動車にドライブレコーダーを設置し、実際の運転状況を解析した上で、机上での認知機能検査時の脳活動との関連を検討する。本研究の結果は、運転能力を客観的に評価する際の判断基準の作成、エビデンスに基づいた統合失調症者の自動車運転支援体制の構築に向けた一助につながる可能性がある。
【方法】両課題とも対象者50名、健常者50名を対象にする。①実車での脳活動の評価については、自動車教習所内の直線走行課題、ブレーキ課題、左右カーブ課題などfNIRS計測時に体動の影響を受けにくい課題を中心に健常者と前頭連合野の脳活動の差を検討する。②ドライビングレコーダー課題については、1000km分の走行データを解析し、違反運転/危険運転行動の特徴ごとに分類し、机上での認知機能検査時の脳活動の特徴との関連性を検討する。
【実施状況】対象者の選定を行い、ドライビングレコーダーの課題を中心に予備実験を行った。来年度より、両課題ともコロナ禍という現状に合わせて本研究を実施する。

社会的孤立・孤独は、さまざまな疾患のリスク因子となっており、孤立・孤独を生まない体制や社会の構築は喫緊の課題であるが、十分な体制が構築されているとはいえず、社会の理解も進んでいるとは言い難い現状がある。このため、当事者は自身の気持ちを表出することに抵抗感を感じてしまう。また、孤立・孤独予防を目的とした活動を含め、高齢者が集う地域の場で男性が少ないことが、全国共通の課題になっている。その背景として、既存の地域の場が「男らしさやニーズに十分な配慮をしないまま運営されてきた」ことが挙げられる。
本プロジェクトでは、「シチズンサポートプロジェクト」として、a)高齢男性向けの新たな居場所(コミュニティー・シェッド)の立ち上げ、b)孤立リスクを簡便に可視化できるツール(社会的つながり可視化ツール)の開発を通し、コミュニティー・シェッドを運営するとともに、その効果を検証する。具体的には、プロジェクト期間内に熊本県水上村および北海道札幌市においてコミュニティー・シェッドを立ち上げ、社会的孤立・孤独の一次予防システムとして有効に機能するか、フィールド調査および心理・脳・健康の調査から検証する。また、「社会的つながり可視化ツール」をコミュニティー・シェッドの効果検証に活用するとともに、地域の支援者や医療専門職が住民の孤立・孤独リスクのスクリーニングや地域診断に活用できるツールとして社会実装することを目指す。これらを通じて、日本独自のコミュニティー・シェッドの在り方を提案し、実践と研究の両輪で高齢者の社会的孤立・孤独に立ち向かい、望まない孤立・孤独を未然に防ぐ取り組み、すなわち一次予防システムの構築につなげる。
