An epidemiological study on behavioral and psychological symptoms of dementia (BPSD) was conducted among long-term care facilities- and home-dwelling older people with dementia. I found that the participants in facilities had any symptoms of BPSD in about 60% at any of three annual surveys. Agitation/aggression and irritability/lability were the most common of the individual BPSD symptoms. Appetite/eating disturbances were annually changeable. Prevalence of any symptoms of BPSD was about 90% among home-dwelling older people in the baseline survey. Apathy/indifference was the most common symptom.

A prospective cohort study over one year was conducted among older residents with dementia (n=312) using a questionnaire for care staff in 10 selected long-term care (LTC) facilities in Hokkaido. The baseline study revealed that the prevalence of BPSD in LTC facilities (percentage of people exhibiting at least one BPSD) in residents with dementia or similar symptoms was 64%. Agitation/aggression was the most common of the individual BPSD symptoms. Also, the factors related to the point prevalence of BPSD were found. The one-year period prevalence, cumulative incidence, persistence, and resolution of BPSD were estimated from 237 participants with complete follow-up.

Firstly, we verified the reliability of the Japanese version of the family caregiver’s self-efficacy scale regarding driving issues related to discontinuation of automobile driving in patients with dementia. Secondly, we performed a psycho-educational intervention using our manual targeting 43 subjects at the psychiatric outpatient clinic at a hospital. Three months after the intervention, our survey revealed a significantly lower traffic accident incidence in the early intervention group, suggesting the effectiveness of the present psycho-educational intervention.

The purpose of this study was twofold : (1) to illustrate the current employment situation for foreign careworkers at caregiving facilities in Japan and (2) to examine the related measures on the acceptance of foreign careworkers under Economic Partnership Agreements. For the first purpose, mail surveys and interviews were conducted and analyzed. For the second purpose, interviews with relevant authorities and other organizations were conducted to address issues within the current system. The findings of this study may contribute to quality assurance of care service at caregiving facilities in Japan.
