English, Scientific journal, BACKGROUND: Major disruptions and changes in education have occurred worldwide as a result of the coronavirus disease (COVID-19) pandemic and the ensuing shift from in-person to online education. However, the effect of such changes on medical education, its magnitude, and the learning domains impacted by such rapid changes have not been adequately addressed, particularly with regard to objective assessment approaches. METHODS: Second-year medical students enrolled in our Medical English Course between 2019 and 2021 were recruited from Hokkaido University, Japan (N = 321) to participate in this study. We evaluated the potential impact of teaching styles on the academic performance of students before (2019; face-to-face) and during (2020; online; 2021; in-person and online) the pandemic. We examined the potential effect of three teaching styles--in-person (2019), online (2020), and a combination of these (2021) on the academic performance of medical students using: (i) subjective assessment of self-reported general English skills, including reading, writing, listening, and speaking; and (ii) objective assessment of medical terminology scores, evidence-based medicine (EBM) skills, and final written exam scores. RESULTS: In-person education significantly improved listening and speaking skills in 2019 (p < 0.001). This trend was observed for writing skills in an online course in 2020 (p = 0.001). With the combined teaching method, students reported significant improvements in all four English skills. In our objective assessments, medical terminology improved significantly post-test versus pre-test for all three teaching styles, and we found that the online course did not adversely affect the gain in medical terminology knowledge during the course. Additionally, we did not find any significant differences across the three applied teaching styles regarding EBM skill levels. It is noteworthy that the students taking online courses had a significantly higher final exam score (mean ± SD; 82.8 ± 8.2) than in in-person (78.6 ± 8.8) and combined (79.7 ± 12.1) teaching styles. CONCLUSIONS: In our study, the online/combined courses showed better academic outcomes compared to the face-to-face course in the preclinical clerkship. Although the current results need to be replicated on a larger scale, online/combined courses can continue and evolve in the post-pandemic education of medical students. Medical schools and institutions should consider incorporating such courses, especially combined courses, into their curricula in the future to improve the effectiveness, accessibility, and flexibility of medical education. - Future career plans of medical students and the COVID‐19 pandemic: Time to recover?

However, in 2020 and during the pandemic, this percentage fell to 35%. English, Scientific journal,Background: The aim of this study was to assess the impact of the coronavirus disease 2019 (COVID-19) pandemic on the current study methods and future plans of medical students compared to those in the pre-pandemic period.
Methods: Second-grade medical students reported their academic experiences, study methods, and future career plans before (between 2016 and 2019) and during the pandemic (2020) using a questionnaire-based survey at Hokkaido University, Japan (n = 534).
Results: From 2016 to 2019, we found an increasing trend for participation in short-term international exchange programs, taking the United States Medical Licensing Examination (USMLE), clinical training, and undertaking research abroad among the students. However, these percentages significantly declined (to 35.5%) during the COVID-19 pandemic in 2020 for all the assessed future plans, including short-term exchange programs (-27.9%), taking USMLE (-19.8%), clinical training (-24.5%), and undertaking research abroad (-13.2%) compared to 2019, wherein 67.9% of the students wished to have at least one of these four above-mentioned academic activities.
Conclusions: The COVID-19 pandemic adversely and significantly influenced our medical students’ plans to go abroad for clinical and research training. Future studies are warranted to assess the long-term influence of this pandemic on the career planning of medical students.

BackgroundMisdiagnoses are associated with various cognitive biases, which are difficult to reduce even if physicians think about clinical cases deliberately. We propose a new "2-Dimensional Approach" that combines two perspectives in diagnostic reasoning: organs (brain, heart, lung, intestine/liver, kidney) and systems (metabolic, endocrine, blood/tumor, infection, immune, circulatory, nervous systems). Systems involve throughout the whole body and can be pathogenesis of diseases. As a result of diseases, organs show abnormal vital signs and symptoms. We investigated: (1) whether each individual resident has a bias for either organs or systems when they diagnose clinical cases, and (2) whether the 2D Approach can reduce such a bias.MethodsWe randomly assigned PGY1 residents (n=105) to either the 2D Approach group (n=45) or a control group (n=60). After attending either a lecture on the 2D Approach or a non-related lecture, residents were asked to diagnose two clinical cases. We divided each diagnosis into one of the two categories, either "organs" or "systems". We investigated whether each resident would diagnose the two cases into the same category, either organs or systems (i.e., a bias for either organs or systems).ResultsThe participants in the control group tended to diagnose the two cases into the same category, either organs or systems (OR: 5.63, 95% CI: 1.62-21.7, p=0.0030, Fisher's exact test). In the 2D Approach group, the category of diagnoses for the two cases were not related to each other (OR: 2.14, 95% CI: 0.50-9.81, p=0.33).ConclusionThere is a bias for either organs or systems when residents diagnose clinical cases, suggesting that organs and systems are different perspectives in diagnostic reasoning. By combining these 2 different perspectives in a 2-dimensional matrix, the 2D Approach reduces this bias.

Particularly, pathophysiological understanding is the key for novices to organize their knowledge and improve clinical reasoning. To this end, we propose a "2-Dimensional (2D) Approach" that visualizes pathophysiology by using a matrix of organs and systems. Methods: This study was a prospective observational study of 100 residents (PGY-1), who attended a lecture on the 2D Approach. They underwent a pre- and post-test that assessed their ability to list differentials for various symptoms and to choose the most likely diagnosis for a case vignette. We stratified the participants according to their baseline knowledge level on the pretest into 2 groups (high-knowledge and low-knowledge). In each of the 2 groups, we compared the change in the pre- and post-test scores between participants who used the 2D Approach during the posttest and those who did not. Results: In the high-knowledge group (n=59), the change in the pre- and post-test scores was significantly larger in the 2D Approach users than non-users (p=0.046), while in the low-knowledge group (n=41), there was no significant difference in the change of the scores between the 2D Approach users and non-users. Conclusions: On condition that residents have sufficient knowledge, the 2D Approach helps them organize their knowledge and improve their clinical reasoning skills, through visualization of pathophysiology on a matrix of organs and systems.

近年、医療ベンチャー主導の技術革新により、疾病予防の推進や健康寿命の延伸をはかることが重要と考えられ、医師によるベンチャー起業が注目されている。 また、各省庁主導の医療ベンチャーサポートプログラムも開発されている。 本研究の目的は、医師が起業する際の促進因子や阻害因子を明らかにし、データ及び理論に基づいた医師の起業教育プログラムを開発すること、起業の自己効力 感及び意図を測定する尺度を開発することである。2020年度は、in-depth interviewを医師起業家、行政官(厚生労働省、経済産業省)、東京医科歯科大学のカリ キュラム開発に関わる教員に対して行い、focus group discussionを東京医科歯科大学の医学部学生に対して2回実施した。約一時間の各セッションは 事前に作成したインタビューガイドを用いて行い、医師が起業をする際の促進因子・阻害因子について調査を行なった。インタビューは同意を得た後に録音し、 逐語的に一語一語文字起こしを行った後、主題分析(Thematic Analysis)を行った。主題分析に用いたコードは、CDCのSocial Eco logical Modelをもとに、個人 の知識、スキルに加え、周囲の友人や家族のサポート、所属するコミュニティーの因子(医局や専門科)、社会的因子(政策等)をコードとするdeductive coding法 を用いて行った。これらの調査を行った後、医師が起業をする際の促進因子・阻害因子について調査した結果を論文にまとめ、Facilitators and barriers to physicians’ entrepreneurial ventures in major Japanese cities: A qualitative studyとして国際学術誌に掲載された。

2021年度には、2020年度に開発ができなかった機能について、引き続き、システム開発を行い、本研究のシステムの運用に必要な基本的な機能をすべて完成させることができた。このために、具体的に下記を実施した。
(1)運用による機能・要望の取りまとめ: 4月より研究分担者等によるテスト運用を行い、8月より本番運用を開始して、機能・要望事項の収集と取りまとめをおこなった。
(2)機能・要望のとりまとめ結果に基づき、システム概要設計及びシステム開発を担当する研究分担者の協議により、システム概要設計を行った。
(3)システム詳細設計とプログラミング: システム詳細設計は、システム開発と並行して行った。つまり最も基本的なシステムの大枠をプログラムのプロトタイプモデルとして開発し、これに順次必要な機能を、スパイラルモデルを活用して追加していった。従来一般的であった硬直的なウォーターフォール型での開発を避け、プロトタイプモデル、スパイラルモデルの併用による柔軟な開発を図った。システム開発手法としては、UMINで長年システム開発を行う中で、効率・柔軟性が高いと認められてきた手法を用いた。その特徴は、開発モデルとしてプロトタイプモデルとスパイラルモデルの両方を組み合わせ、開発方法論として、データ中心アプローチを用いることにある。またシステム開発に当たっては、既存のEPOC及びEPOC2のプログラムを可能な範囲で再利用して、必要な労力・コストの削減を図った。

This study developed computer simulation materials in e-learning (English/Japanese) to learn basic knowledge about Japanese caregiving and experience the technique and situation for international students in their home country before coming to Japan.
In order to evaluate the validity of the developed simulation materials, they were provided to medical, nursing, dental, dental hygiene, and graduate students in several courses of the e-learning system (WebClass). As a result, they were generally highly evaluated. Furthermore, it was indicated that the learning method using these materials was effective and useful when the English versions of teaching materials were provided to the Japanese and international learners.

The purpose of this study was to develop an educational system to learn new advanced medical techniques for medical students and to examine the usefulness of clinical anatomy practice consisting of simulated surgery using donated cadavers. The contents of the research were a questionnaire survey of medical students and academics, and a pilot study of simulated endoscopic surgery. The simulated surgery consisted of surgical lectures, demonstrations by surgeons, and hands-on or observational practice by students.
These studies showed that both conventional systematic anatomy and newly developed clinical anatomy were useful. It is desirable to introduce clinical anatomy as part of the clinical training curriculum other than conventional anatomy.

The aim of this study was to develop a teaching system for highly-skilled surgical procedures. We developed a simulator for anterior cervical spinal procedures, i.e. the floating method of the ossification of posterior longitudinal ligament (OPLL). This simulator with anatomical characteristics is modeling with salt using a 3-D printer. Also, we created an evaluation system of safety management during operation and mastery level of the anterior cervical procedures. Our simulator and evaluation system would provide high quality and safety teaching methods for highly-skilled surgical procedures.

We exhaustively analyzed risk factors when medical students perform phlebotomy or urethral catheterization which are basic clinical procedures. It was suggested that safety management abilities of a teacher could be assessed by exhaustiveness of risk factors which he/she recalls beforehand. We developed a faculty training workshop for safety management on the clinical training, and conducted it on a trial basis. We assigned a task of planning of urethral catheterization training, and requested using process hazard analysis. The participants could list exhaustively the risk factors when medical students urethral catheterization. This workshop seems to be beneficial for faculty development for safety management on the clinical training,

We developed assisting software for Hazard and Operability Study of medicine, which is one of risk analysis methods using in an industrial field. But it was not feasible because of time consuming process. On the other hand, importance of the concept of HAZOP was verified by exercise for medical students, in which a newly-developed bone-drilling simulator was utilized. To evaluate effectiveness of HAZOP exercise of medical students, we compared incident reports written by residents between those who have received HAZOP exercise and those who have not. The results showed no difference between two groups. Patient safety exercise and verification of its educational effectiveness should be continued.

1. A questionnaire was administered to 240 persons in order to collect and analyze basic data regarding medical communication education for clinical situations. Subsequently, we analyzed the data consisting of multiple-choice and open-ended questions related to doctor-patient communication.
2. Another survey was conducted of foreign medical students who would be using the e-learning materials in order to understand their needs for such study aids, and we were able to get a grasp of how well that they would be able to work independently in conjunction with a computer using the e-learning materials developed in this project.
3. An investigation and content analysis was carried out on existing communication education materials as part of the basic study to create new ones. We invited Prof. Dennis Novak, a specialist in medical communication from Drexel University's medical school (in the U.S.A.), and held a public lecture.
4. Scripts for interactions in clinical situations were created for independent study materials, and these were used to develop video materials using a digital camera. The titles and scenarios were "Patient Confirmation"-- a dentist/doctor confirms a patient's identity, "An Intravenous Drip Mistake"--an interaction between an inexperienced doctor and a patient during the application of an intravenous drip, "A Patient with Cold Hands and Feet"--dealing with a foreign patient with cold hands and feet who is nervous due to previous examinations, "An Elderly Patient with Back Pain in the Emergency Room"--A young doctor's interaction using young people's language with an elderly patient and an accompanying family member in an emergency room.
5. The e-learning materials were subsequently used by foreign and Japanese medical students who later evaluated the contents by computer. The materials generally were highly evaluated by the students.
6. The results and analysis of the administration of these materials were presented at the 3rd International Wireless Ready Symposium and Leiden University's (The Netherlands) Japanese Linguistics Day, and other conferences. The need for sympathetic communication between doctors and patients is growing. Educational materials that deal with a variety of problematic communication situations such as these are essential, and the use of these independent study materials by foreign medical students as well as Japanese students is very meaningful for the field of medical communication as a whole.
7. All the results of the research described in the above including Dr. Novack's lecture and e-learning materials with videos were published on a website.

Human bone marrow stromal cells (hBMSCs) are an attractive source for bone tissue engineering applications because of their differentiation capability and their proliferation capability. However, despite the potential of BMSCs, they lose their differentiation potential after multiple doubling processes under standard culture conditions
1. We hypothesized that dexamethasone could augment the responsiveness of BMSCs to differentiation reagents, and continuous dexamethasone treatment would establish BMSCs with higher differentiation potentials. The results of our studies indicated that the dexamethasone-treated cells showed higher osteogenic capability and bone formation capability than the control group.
2. To develop efficient bone regeneration using cells and porous scaffolds, it is essential to introduce an adequate number of cells into porous scaffolds. To improve cell seeding efficiency, we modified the previously reported cell seeding techniques using low-pressure conditions and devised a simple device for this method. The procedures are as follows. In the first step, porous β-TCP blocks are placed into a glass chamber, and in the second step, the air in the chamber is removed by a syringe to create a vacuum. In the third step, the cell suspension is slowly infused into the chamber, maintaining the low-pressure condition, and the blocks are soaked in the suspension. In the final step, the valve is released so that the pressure in the chamber normalizes. Compared to spontaneous penetration (control), cell seeding efficiencies by this method were three fold higher, and the implants prepared by the method showed bone formation more than three folds of the control.
3. We also developed and tested a cell seeding system into porous implants using plasma, expecting that the fibrin network formed from fibrinogen would hold introduced cells within minutes and promote bone formation. BMSCs were suspended in plasma and introduced into porous scaffolds, and fibrin gel was formed within minutes. Then the implants were transplanted. The implants prepared using plasma showed significantly more bone formation than that by the implants using BMSCs suspended in culture medium.

今年度は、前回報告した移植細胞となる胎児海馬由来神経幹細胞(神経幹細胞)と骨髄由来幹細胞(骨髄細胞)のRT-PCR法による神経栄養因子発現の比較から1)NGF・CNTF・GDNF遺伝子およびそれぞれのレセプター遺伝子ベクターを培養神経幹細胞に導入しその分化傾向を確認した。in vivo実験では2)脊髄損傷モデルに移植した細胞の生存期間を改善するような遺伝子導入を行った。
1)NGF・CNTF・GDNFおよびそれぞれの受容体遺伝子を培養神経幹細胞にさまざまな組み合わせで導入実験を行った。その結果、GDNFとそのレセプターGFRα1の組み合わせでもっと高い神経細胞へ分化が認められた。本結果から骨髄細胞が脊髄損傷部に移植された場合、損傷後の機能回復を促進する一因としてGDNFの関与が示唆される。今後は、脊髄損傷後にGDNFを持続的に発現させるモデルを作製し、損傷急性期の炎症の抑制と組み合わせて研究を行っていく予定である。
2)前回、培養神経幹細胞を脊髄損傷1週後に移植した結果、ホスト脊髄内で細胞の生着は確認されたもの正常脊髄に移植したものの約半数しか生着できなかったことを報告した。今回、細胞骨格とアポトーシスに関与するRhoファミリー遺伝子に注目し、神経幹細胞に導入し移植実験を行った。その結果、RacおよびRho遺伝子のdominant negative typeを感染させた場合、対照群と比較して2.5倍の生着細胞が観察された。これら遺伝子は、移植後の細胞死を抑制するための有効な手段と考えられた。
1)2)では損傷脊髄に移植する神経幹細胞、骨髄幹細胞についてその神経栄養因子的な働きと細胞死の抑制方法について明らかにした。骨髄幹細胞の脊髄損傷に対する働きはいまだ不明であり、移植細胞の多くが細胞死を受け十分に生着せず、一過性の働きしか得られないという報告もあることから本研究で明らかにしたことは意義が大きい。いずれの実験結果も論文への投稿準備中である。研究計画にあるような抗炎症作用とダリア瘢痕の抑制環境の作製には時間が必要であるが、本研究で明らかにした手法を組み合わせることで脊髄損傷のメカニズムおよび治療の解明に役立つと思われる。
