- Pregnant nurses' occupational stress and associated factors: A comparative cross-sectional study with other healthcare workers, physical workers, and desk workers.

English, Scientific journal, AIM: To examine occupational stress levels and associated factors among pregnant nurses through a comparison with pregnant non-nurses. METHODS: This cross-sectional study included 1060 working pregnant women. Participants completed questionnaires including the Brief Job Stress Questionnaire, work conditions, obstetric conditions, Sense of Coherence Scale, and self-management behaviors questionnaire. The participants were categorized into four groups: nurses, other healthcare workers, physical workers, and desk workers. Comparisons were made between the four groups and by gestational ages using Dunnett's test and chi-squared test, and associated factors were examined using ordinal logistic regression analysis. RESULTS: Analysis of 847 valid responses showed that the nurse group had a higher occupational stress, higher job demand, and lower job control at any gestational age than the other profession groups. Nurses' occupational stress was associated with factors such as frequent overtime work, fewer break times, lower manager support, and lower prioritizing the fetus. Particularly, fewer break times and lower prioritizing the fetus were nurse-specific associated factors. CONCLUSIONS: The study showed that pregnant nurses are exposed to higher occupational stress throughout the entire pregnancy than other occupations. The stress was related to modifiable factors such as fewer break times and lower prioritizing the fetus. To manage occupational stress, pregnant nurses should avoid overtime, take breaks, and prioritize the fetus in the workplace. Hospital administrators would need to provide work coordination support and consider certain regulations regarding hazardous work restrictions for pregnant nurses. Future cohort studies are required to better understand occupational stress among pregnant nurses. - The transition process of perceptions of hygiene care practices among nurses in acute‐care wards: A grounded theory study

Aim
To explore how perceptions of hygiene care practices among nurses in acute‐care wards change over their clinical experience.
Methods
Based on symbolic interactionism, we employed a grounded theory methodology to explore interrelationships of meaning arising from actors' perceptions and interactions. Thirty‐three nurses working in acute‐care wards for >3 years were selected by purposive snowball and theoretical sampling; semi‐structured individual interviews were conducted from October 2023 to February 2024. Data were analyzed for 32 participants who provided consent.
Results
The core category was “adjusting care channels”; seven other categories also emerged. Care channels refer to options for hygiene care practices. When nurses are first assigned to a ward, they regard hygiene care as the “bare task under organization rules.” However, through repeated processes of “emerging caring with patient resonance” and “internalizing the meaning of care,” they can “form care channels.” Subsequently, nurses become practitioners who can transform practices of hygiene care into “care to be restricted” or “wealth of ‘life behind the curtain’” by “adjusting care channels” according to the patient and ward organization situation. In contrast, if nurses fail to internalize the meaning of care, they end up performing “pro forma activities.”
Conclusions
Our findings highlight the importance of nurses to improve their ability to “adjust care channels,” which would allow them to provide hygiene care in a manner appropriate to the patient and organizational context. These results contribute to the development of educational strategies that foster nurses' values of hygiene care and support strategies by nursing managers. - Quantity and quality of napping to mitigate fatigue and sleepiness among nurses working long night shifts: a prospective observational study

Background
Napping during night shifts is a countermeasure against fatigue and sleepiness, which both impact patient safety. However, there is insufficient evidence on how nurses nap, especially concerning their napping quality. This study explored night-shift napping and its associated factors among nurses, considering napping quantity and quality, to mitigate fatigue and sleepiness.
Methods
This month-long prospective observational study included 32 nurses working 16-h night shifts in a general ward. All nurses responded to questions on individual factors, while fatigue and sleepiness were checked four times during night shifts. Night-shift napping was measured using a wearable device and classified into six groups: time in bed [TIB] > 180 min and sleep efficiency [SE] ≥ 70%, TIB > 180 min and SE < 70%, TIB 120–180 min and SE ≥ 70%, TIB 120–180 min and SE < 70%, TIB < 120 min and SE ≥ 70%, and TIB < 120 min and SE < 70%.
Results
Most nurses (81.2%) worked four night shifts per month, and 105 night shifts in which nurses intended to nap were analyzed. The two nap conditions (TIB 120–180 min and SE ≥ 70%, TIB > 180 min and SE ≥ 70%) were not worse than other nap conditions in fatigue and sleepiness at the end of the night shift and change in fatigue from the start to the end of the night shift. Sleep reactivity, pre-nap time on electronic devices, and prophylactic naps taken before the night shift were each the common factors related to napping for TIB ≥ 120 min and SE ≥ 70%.
Conclusions
Nurses working long night shifts should consider both sufficient napping quantity and good napping quality. We suggest aiming for a TIB of at least 120 min and a SE of at least 70% to mitigate fatigue and sleepiness at the end of a night shift. Assessing sleep reactivity, pre-nap time on electronic devices, and prophylactic naps may be useful in achieving both quantity and quality effectively. Nurses and their managers should have a better understanding of napping and consider strategically taking naps during night shifts. - A nomogram to predict skin barrier dysfunction induced by mechanical irritation during skincare in hospitalized Japanese older adults

Scientific journal, The high rate of perinatal abnormality among pregnant nurses and midwives attributable to their working environment is a serious international occupational health concern. Self-management behaviours can prevent abnormalities, but nurses and midwives have difficulty carrying out these behaviours during pregnancy. This cross-sectional study aimed to examine self-management behaviours and associated factors during pregnancy among nurses and midwives. The participants were 232 pregnant or postpartum registered nurses and midwives from four Japanese general hospitals. Participant recruitment and questionnaire distribution were undertaken through hospital management. Data were collected from July to November 2022. Instruments included the Personal Demographic and Obstetric Conditions Questionnaire, the Self-Management Behaviours Questionnaire, the Working Conditions during Pregnancy Questionnaire, the Social and Workplace Support Scale, the Sense of Coherence Scale, and Self-Management Difficulties Scale. Data analysis used univariate and multiple classification analysis.Results indicated that participants demonstrated good compliance with contraindications among self-management behaviours but scored lower in knowledge and self-monitoring, abnormalities prevention, and avoiding burden movements. Multiple classification analyses revealed that parity, profession, overtime, acquisition of maternity protection provisions, managers support, family support, and prioritising the foetus were significantly associated with self-management behaviors. Nursing administrators must provide adequate information and adjust the work of pregnant staff. Policymakers need to propose measures to reinforce maternity protection provisions for pregnant nurses and midwives. Pregnant nurses and midwives need to be proactively educated on occupational health and safety. Learning the importance of acquiring maternity protection provisions, receiving support and prioritising the foetus will contribute to effective self-management practices in them. Aim
To explore the recognition of pregnant nurses on how they managed their health conditions to examine safe working strategies.
Design
A qualitative study with a grounded theory approach.
Methods
Twenty‐one nurses engaged in work during their pregnancy were recruited and interviewed using a semi‐structured questionnaire from January to June 2021. The data were analysed using a constant comparative method.
Results
The core category ‘duelling roles’ and the four other categories emerged. Pregnant nurses understand the ‘weight of one’ of being a professional in the workplace. Therefore, despite their health concerns, they struggle to complete their work as one team member to avoid inconveniencing others. However, through experiencing various nursing situations, they ‘perceive one's limits’ of working as they had done before pregnancy and protect their health and patients. Nevertheless, interactions with patients and their colleagues bring ‘delight in nursing’, which encourages them to continue working. Pregnant nurses thus develop a ‘prioritizing the foetus’ working style to continue being nurses while protecting their health.
Implications for the Profession and/or Patient Care
These results provide meaningful guidance in considering safe job retention strategies for pregnant nurses. Sharing and developing the ‘prioritizing the foetus’ mindset and management skills gained by the participants may be beneficial for the appropriate health management of pregnant nurses. The study may also facilitate nursing managers' understanding of the experiences of pregnant nurses and encourage them to consider reviewing nursing practices.

Aim
Skin barrier dysfunction can trigger various skin disorders in older adults. Skin barrier assessment is essential for nurses and caregivers to prevent skin disorders; however, the evidence available for clinical assessment is limited. This systematic review aimed to clarify the risk factors of skin barrier dysfunction in older adults.
Methods
This review adhered to the Preferred Reporting Items for Systematic reviews and Meta‐Analyses (PRISMA) guidelines. The four databases were searched using multiple terms related to “aged” and “skin barrier.” The search was initially run on April 19, 2023, and rerun on October 12, 2023. Peer‐reviewed quantitative studies in English were included, with no publication time limit being set. Two reviewers assessed the risk of bias in a blinded and independent manner using JBI tools. Owing to the heterogeneity of the results, a narrative synthesis was performed.
Results
Among the database‐identified 4833 studies, 20 studies were included. The extracted factors were categorized as demographic characteristics, functional characteristics, chronic diseases, nutritional status, skin condition, and environmental factors. However, owing to high risk of bias and inconsistent results across studies, only chronic kidney disease and dry skin were considered risk factors for skin barrier dysfunction in older adults.
Conclusions
Assessment of chronic kidney disease and dry skin in daily skin care may guide the development of personalized skincare programs to maintain skin integrity in older adults. Furthermore, cohort studies that consider confounding factors and the reliability of measurements are needed for an in‐depth investigation into skin barrier dysfunction and more risk factors. - Correction: Venous dilation effect of hot towel (moist and dry heat) versus hot pack for peripheral intravenous catheterization: a quasi-experimental study.

English - Utility of salivary cortisol profile as a predictive biomarker in nurses’ turnover risk: a preliminary study

Background
Predicting nurse turnover risk is crucial due to the global nursing shortage; however, existing predictors, such as fatigue and burnout, lack objectivity. Salivary cortisol is a non-invasive marker of stress and fatigue, but its utility in predicting nurse turnover risk is unknown. We examined whether salivary cortisol profiles across three different day shifts in a month are predictors of the extent of nurses’ reluctance to stay in their current jobs.
Methods
This preliminary longitudinal study followed forty female nurses who engaged in shift work at a university hospital for 3 months. Data at enrollment were collected including demographics, working conditions, chronic fatigue (the Japanese version of the Occupational Fatigue/Exhaustion Recovery Scale), and burnout (Japanese Burnout scale). Salivary cortisol was measured before the three different day shifts (after awakening) during the first month, and the means of these measurements were used as the cortisol profile. The extent of reluctance to stay was assessed using the numerical rating scale at 3 months.
Results
Among the forty female nurses (mean [SD] age, 28.3 [5.1]), all completed follow-up and were included in the analysis. The cortisol profile was associated with the extent of reluctance to stay (P = 0.017), and this association was significant despite adjustments for chronic fatigue and burnout (P = 0.005). A multiple regression model with chronic fatigue, burnout, and job tenure explained 41.5% of the variation in reluctance to stay. When the cortisol profile was added to this model, the association of the cortisol profile was significant (P = 0.006) with an R2 of 0.529 (ΔR2 = 0.114).
Conclusions
This preliminary study conducted in an actual clinical setting indicated the potential of the salivary cortisol profile across three different day shifts in a month to predict nurses’ reluctance to stay in their current jobs. The combination of subjective indicators and the cortisol profile would be useful in predicting nurses' turnover risk. - Chronic fatigue and recovery among nurses working two-shift and three-shift rotations

English, Scientific journal, BACKGROUND: Heat application before peripheral intravenous catheterization is recommended for venous dilation. Hot pack application enlarges the venous diameter in healthy adults; however, hot towels (moist and dry heat) are used often in some medical cases. However, it is unclear whether hot towel application promotes venous dilation better than hot pack application. This study compared the venous dilation effect of using a hot towel (moist and dry heat) to a hot pack before applying the tourniquet at an access site for peripheral intravenous catheterization. METHODS: Eighty-eight healthy females aged 18-29 years were recruited for this quasi-experimental study. They underwent three types of heat applications (hot pack, moist hot towel, and dry hot towel [moist hot towel wrapped in a dry plastic bag], all of which were warmed to 40 ± 2 °C and performed for 7 min) to their forearm and tourniquet application for 30 s after each heating. Venous diameter and depth were measured using ultrasonography, and venous palpability and visibility (venous assessment score) was observed as venous dilatation effects. In addition, the skin temperature, stratum corneum hydration, and subjective evaluation of the warmth were measured. RESULTS: There were no significant differences in venous diameter and assessment scores after intervention between the dry hot towel and the hot pack groups, and the effect size was negligible (Cohen's d < 0.20). However, these measurements were significantly lower for the moist hot towel than for the other two heat applications (P < .001). Although there was no significant difference in skin temperature and warmth rating score between the dry hot towel and the hot pack, these were significantly lower for the moist hot towel than for the other two heat applications (P < .001). The amount of change in stratum corneum hydration of the dry hot towel was not significantly different from that of the hot pack; however, that of the moist hot towel was significantly larger than that of the other two heat applications (P < . 001.) CONCLUSIONS: A method in which a towel warmed in hot water is wrapped in a dry barrier may be an alternative to a hot pack. Scientific journal, Successful insertion of a peripheral intravenous catheterization requires that veins be sufficiently dilated. This study aimed to clarify the venous dilation effect of including tapping or massaging to the application of a tourniquet at the cutaneous veins of healthy adults’ forearms. This was a quasi-experimental study of 30 healthy adult volunteers. Each participant underwent all three venous dilation procedures, which included the tourniquet application (Control condition), the tourniquet application and tapping the participant’s forearm (Tapping condition), as well as the tourniquet application combined with massaging the participant’s forearm (Massage condition). To clarify the venous dilation effects, venous indices were measured, namely the venous diameter (mm), depth (mm), and palpation score. After applying all venous dilation procedures, the venous diameter and palpation score significantly increased. However, no significant difference was observed between the control condition and each intervention condition. The depth in the control and tapping conditions decreased significantly in contrast to the Massage condition. Moreover, a subgroup (nine participants with a venous diameter less than 3 mm after the control condition) had similar results. This study found that additional tapping or massaging after tourniquet application could be less effective in promoting dilation in the forearm veins of healthy adults. Future studies should examine the efficacy and effectiveness of venous dilation in a wide target population while considering intervention methods. - Minimum wiping pressure and number of wipes that can remove dirt during bed baths using disposable towels: a multi-study approach

Background
Friction irritation by wiping increases the risk of skin problems. In bed baths with cotton towels, wiping three times with weak pressure (10–20 mmHg ≈ 1333–2666 Pa) can remove dirt while maintaining skin barrier function. However, few studies have examined the appropriate frictional irritation with disposable towels. This study aimed to analyse the wiping pressure and number of wipes currently applied by nurses when using disposable towels during bed baths and propose the minimum values for removing dirt from the skin.
Methods
This multi-study approach consisted of cross-sectional and crossover design components. In Study 1, 101 nurses in two hospitals were observed by recording the wiping pressure and number of wipes when using both disposable (nonwoven) and cotton (woven) towels. Wiping pressure and number of wipes by towel materials were analysed using a linear mixed model. In Study 2, 50 adults received oily and aqueous dirt on their forearms, which were wiped six-times with disposable towels, applying randomly assigned pressure categories. We used colour image analysis and a linear mixed model to estimate the dirt removal rate for each combination of wiping pressure and number of wipes.
Results
Study 1 showed that although wiping pressure did not differ by towel material, the number of wipes was significantly higher for disposable wipes than cotton wipes. Approximately 5% of nurses applied strong wiping pressure or wiped too often. In Study 2, wiping three times with disposable towels at least 5–10 mmHg achieved dirt removal rates of ≥80%.
Conclusions
Some nurses excessively wiped using disposable towels, which might cause skin problems. However, excessive wiping is not required to adequately remove dirt, regardless of the towel material used in various clinical situations. We recommend wiping at 10–20 mmHg of pressure (just like stroking gently) at least three times to improve the quality of bed baths. These findings highlight the need to develop skin-friendly bed bath educational programmes, particularly using appropriate frictional irritation to reduce the risk of skin problems. - Human pathogenic bacteria on high-touch dry surfaces can be controlled by warming to human-skin temperature under moderate humidity.

One route of transmission of pathogenic bacteria is through contact with "high-touch" dry surfaces, such as handrails. Regular cleaning of surfaces with disinfectant chemicals is insufficient against pathogenic bacteria and alternative control methods are therefore required. We previously showed that warming to human-skin temperature affected the survival of pathogenic bacteria on dry surfaces, but humidity was not considered in that study. Here, we investigated environmental factors that affect the number of live bacteria on dry surfaces in hospitals by principal component analysis of previously-collected data (n = 576, for CFU counts), and experimentally verified the effect of warming to human-skin temperature on the survival of pathogenic bacteria on dry surfaces under humidity control. The results revealed that PCA divided hospital dry surfaces into four groups (Group 1~4) and hospital dry surfaces at low temperature and low humidity (Group 3) had much higher bacterial counts as compared to the others (Group 1 and 4) (p<0.05). Experimentally, warming to human-skin temperature (37°C with 90% humidity) for 18~72h significantly suppressed the survival of pathogenic bacteria on dry surfaces, such as plastic surfaces [p<0.05 vs. 15°C (Escherichia coli DH5α, Staphylococcus aureus, Pseudomonas aeruginosa, Acinetobacter baumannii, and blaNDM-5 E. coli)] or handrails [p<0.05 vs. 15~25°C (E. coli DH5α, S. aureus, P. aeruginosa, A. baumannii)], under moderate 55% humidity. Furthermore, intermittent heating to human-skin temperature reduced the survival of spore-forming bacteria (Bacillus subtilis) (p<0.01 vs. continuous heating to human-skin temperature). NhaA, an Na+/H+ antiporter, was found to regulate the survival of bacteria on dry surfaces, and the inhibitor 2-aminoperimidine enhanced the effect of warming at human-skin temperature on the survival of pathogenic bacteria (E. coli DH5α, S. aureus, A. baumannii) on dry surfaces. Thus, warming to human-skin temperature under moderate humidity is a useful method for impairing live pathogenic bacteria on high-touch surfaces, thereby helping to prevent the spread of healthcare-associated infections. - Validity and reliability of the Japanese version of the Nursing Performance Instrument.

English, Scientific journal, AIM: Work performance of nurses is a critical aspect that concerns the safety of both nurses and patients, but also secondarily influences nurses' turnover due to serious fatigue and burnout. Monitoring nursing performance as well as fatigue is important for appropriate safety management of patients and nurses, and the development of a monitoring tool is essential. The Nursing Performance Instrument (NPI) can provide comprehensive and brief monitoring of nursing performance. This study aimed to verify the validity and reliability of a Japanese version of the NPI (NPI-J). METHODS: A cross-sectional study was conducted on nurses who work in inpatient care across five Japanese hospitals. We considered appropriate factor numbers with parallel analysis and the Velicer's minimum average partial test. The construct validity was evaluated using exploratory factor analysis, confirmatory factor analysis (CFA), and Pearson's correlation analysis. Cronbach's alpha was calculated for the internal consistency. RESULTS: A three-factor model eventually showed acceptable fit indices in the CFA. Moreover, the correlation results among the three factors indicated discriminant validity comparable to the original NPI. The correlation results with acute fatigue, chronic fatigue, and burnout verified convergent validity. Internal consistency was insufficient. CONCLUSION: The NPI-J demonstrated acceptable construct validity. This instrument can assess nurses' perceptions of their performance by measuring each item. Future work on the reliability of the sub-concepts will require additional items or reliability testing based on generalizability theory and item response theory. - Cognitive changes with psychomotor skill acquisition through blended learning among nursing students: A qualitative study

English, Scientific journal, AIM: This study described nursing students' cognitive changes as they acquire psychomotor skills through blended learning. BACKGROUND: Deliberate practice, including feedback from teachers, is vital for acquiring psychomotor skills. Blended learning, a program that allows students to deepen their learning and improve their skills even when students and faculty are physically separated has attracted significant attention in recent years. Although blended learning has been used for learning in the cognitive domain, no study has examined its effectiveness in the acquisition of psychomotor skills. Understanding how students' cognition changes as they acquire skills in a blended learning environment could be a valuable resource for effective teaching. DESIGN: An inductive, qualitative description approach was adopted. METHODS: The program involved a basic nursing skill: making an occupied bed. Eleven second-year nursing students participated. The participants attended face-to-face lectures and e-learning courses comprising self-study content that was designed for easy and frequent reference. Students practiced for a skill test, which was conducted one month after the first lecture. Two interviews were conducted approximately one month apart. Before each interview, the participants' current practices were videotaped. During the interviews, they explained their thought processes and conscious awareness of their actions as they watched the videos. This study was conducted between April and May 2019. RESULTS: Six categories related to changes in participants' cognitive processes while acquiring the skill of making an occupied bed were identified: "feeling that it is easy to acquire," "practicing without much thought," "realizing the difficulty in translating thoughts into practice," "experiencing a sense of purpose in each technique," "gaining a perspective to evaluate one's skills," and "developing one's unique approach." CONCLUSIONS: In a blended learning environment, where a practice environment and audiovisual materials were provided, students could practice and improve their skills at their own pace even without the instructor's frequent advice. The findings show that metacognitive skills are essential to the development of psychomotor skills in a blended learning program because this program requires practicing while monitoring one's skills. Metacognitive skills affect the development of psychomotor skills and the ability to provide care. Therefore, initiatives that address the development of metacognitive skills, such as the current program, during the early stages of basic education programs can contribute to the development of nursing students' practical skills. - Explicit and implicit attitudes toward smoking: Dissociation of attitudes and different characteristics for an implicit attitude in smokers and nonsmokers

Scientific journal, Smoking is a global health risk for premature death and disease. Recently, addictive behaviors, like smoking, were considered to be guided by explicit and implicit processes. The existence of a dissociation between the two attitudes in nonsmokers and the causes of the differences in implicit attitudes toward smoking have not been fully investigated. We investigated the explicit and implicit attitudes toward smoking via a self-reported scale and the single category implicit association test (SC-IAT), respectively, among undergraduate and graduate health sciences students. In addition, we applied the drift-diffusion model (DDM) on the SC-IAT and examined the behavioral characteristics that caused differences in implicit attitude toward smoking between smokers and nonsmokers. The results showed the existence of a dissociation between explicit and implicit attitudes toward smoking among nonsmokers. In addition, nonsmokers had a higher decision threshold than smokers and a higher drift rate in the condition where negative words were associated with smoking. Nonsmokers engaged in SC-IAT with more cautious attitudes and responded more easily in a negative condition since it was consistent with their true attitudes. Conversely, smokers did not show a significant difference in the drift rate between the conditions. These results suggested that the differences in an implicit attitude between smokers and nonsmokers were caused by differences in evidence accumulation speed between the positive and negative conditions. The existence of dissociation between implicit and explicit attitudes toward smoking may indicate the difficulty of measuring true attitude in nonsmokers in a questionnaire survey. Additionally, the DDM results explained the difference of implicit attitude between smokers and nonsmokers; it may provide information on the mechanisms of addictive behaviors and a basis for therapy. However, whether these results are affected by cultural differences requires further investigation. - Effects of 90 Min Napping on Fatigue and Associated Environmental Factors among Nurses Working Long Night Shifts: A Longitudinal Observational Study.

English, Scientific journal, For nurses working long night shifts, it is imperative that they have the ability to take naps to reduce fatigue, and that an appropriate environment is prepared where such naps can be taken. We verified the effects of 90 min napping on fatigue and the associated factors among nurses working 16-h night shifts. We investigated 196-night shifts among 49 nurses for one month. Wearable devices, data logging devices, and questionnaires were used to assess nap parameters, fatigue, and environmental factors such as the napping environment, ways of spending breaks, and working environment. Nurses who nap at least 90 min on most night shifts had more nursing experience. Multivariable logistic regression analysis showed that the environmental factors significantly associated with total nap duration (TND) ≥ 90 min were noise, time spent on electronic devices such as cellphones and tablets during breaks, and nap break duration. The night shifts with TND ≥ 90 min showed lower drowsiness after nap breaks and less fatigue at the end of night shift compared to those with TND < 90 min. Nurses and nursing managers should recognize the importance of napping and make adjustments to nap for at least 90 min during long night shifts. - Salivary Biomarker Profiles and Chronic Fatigue among Nurses Working Rotation Shifts: An Exploratory Pilot Study.

We explored the profiles of salivary biomarkers associated with nurses' chronic fatigue across several shifts. This longitudinal study involved 45 shiftwork nurses and collected their saliva samples before two night and two day shifts for a month. Chronic fatigue was measured using the Cumulative Fatigue Symptom Index before the first night shift. Biomarker profiles were analyzed using hierarchical cluster analysis, and chronic fatigue levels were compared between the profiles. Cortisol profiles were classified into high- and low-level groups across two day shifts; the low-level group presented significantly higher irritability and unwillingness to work. Secretory immunoglobulin A (s-IgA) profiles across the four shifts were classified into high- and low-level groups; the high-level group had significantly higher depressive feelings, decreased vitality, irritability, and unwillingness to work. Cortisol (two day shifts) and s-IgA (four shifts) profiles were combined, and (i) cortisol low-level and s-IgA high-level and (ii) cortisol high-level and s-IgA low-level groups were identified. The former group had significantly higher chronic fatigue sign and irritability than the latter group. The profiles of salivary cortisol and s-IgA across several shifts were associated with nurses' chronic fatigue. - Post-Work Recovery from Fatigue and Sleep Episodes among Nurses Who Are Engaged in 16-Hour Night Shifts: A Prospective Observational Study.

English, Scientific journal, Poor recovery from fatigue among shift-working nurses can cause a transition from acute to chronic fatigue. We aimed to clarify the relationship between nurses' recovery from fatigue and sleep episodes after 16 h night shifts while considering age. This prospective study included 62 nurses who worked 16 h night shifts. Fatigue was assessed by a questionnaire before, during, and after the night shift, and the morning following the night shift. Sleep episodes were continuously measured using a wearable device. We performed a hierarchical cluster analysis of multivariate sleep parameters in first and main sleep episodes after night shifts. A linear mixed model was used to estimate the difference between clusters in recovery from fatigue after the night shift, considering age. The participants were classified into a high sleep quality group (HSQG) and low sleep quality group (LSQG) in sleep episodes after the night shift. There was a significant main effect of clusters, and HSQG was significantly more effective than LSQG in recovering from fatigue. However, no main effects of age or interaction were observed. The quality of first and main sleep episodes at home was associated with recovery from the night shift to the next day, regardless of age. - Accuracy of the Data of Biochemical Parameters in Blood Collected Above the Infusion Insertion Site

English, Scientific journal, BACKGROUND: Dry skin is the most common skin problem, especially in the elderly. However, there is no effective instrument to assess dry skin in Japan. This study aimed to evaluate the reliability and validity of the Japanese version of the overall dry skin score (ODS-J), the gold standard for dry skin assessment. MATERIALS AND METHODS: A cross-sectional study was conducted on 47 patients aged > 65 years. Images of skin on their limbs were captured using a digital camera; both upper and lower limbs were assessed (n = 4/patient). One dermatologist; two wound, ostomy, and continence nurses; and three nursing researchers independently evaluated the images using the ODS-J to assess the intraclass correlation coefficient (ICC) for inter-rater reliability. Stratum corneum hydration (SCH) and transepidermal water loss (TEWL) were the external criteria used to verify concurrent and known-groups validity. RESULTS: In total, 182 sites at which the SCH and TEWL could be measured were evaluated for the ODS-J. The ICC for inter-rater reliability of the six raters was 0.939 (p < 0.001). A higher ODS-J was associated with lower SCH (ρ = -0.374; p < 0.001) and lower TEWL (ρ = -0.287; p < 0.001) values. The ODS-J for the lower legs was significantly higher than that of the forearms (p < 0.001). CONCLUSIONS: The ODS-J showed good inter-rater reliability, concurrent validity, and known-groups validity. It can be used by clinical nurses in Japan to observe patients' skin and is an effective indicator for the evaluation of skin care. - Sleep deprivation and fatigue in early postpartum and their association with postpartum depression in primiparas intending to establish breastfeeding.

English, Scientific journal, Objective: To examine the association of objective sleep and fatigue in the early postpartum period with postpartum depression in Japanese primiparas intending to establish breastfeeding. Materials and Methods: The participants were 34 primiparas who were in the postnatal ward after vaginal delivery and responsively breastfeeding their rooming-in baby. Actigraphy data for objective sleep were collected for three consecutive days starting from the first day postpartum. Fatigue and postpartum depression were assessed using the Postpartum Fatigue Scale and Edinburgh Postnatal Depression Scale, respectively, on numerous days between the first day postpartum and the one-month checkup. Breastfeeding and rooming-in data were also collected. Results: The mean total sleep time was 252.0 ± 73.1 min/day. Mean breastfeeding frequency was 12.4 ± 3.2 times/day and mean total breastfeeding time was 247.4 ± 101.8 min/day. Among the participants, 67.6% were exclusively breastfeeding on the discharge day. Fatigue scores were significantly higher during the hospital stay, compared with one month postpartum. Multiple regression analysis showed that sleep frequency on the third day postpartum and mean total breastfeeding time and fatigue on the fifth day postpartum were factors affecting the Edinburgh Postnatal Depression Scale score. Conclusion: The association between postpartum depression among breastfeeding primiparas in the early postpartum period and objective sleep, fatigue, and total breastfeeding time per day was suggested. An environment wherein breastfeeding mothers can rest and sleep without hesitation will be beneficial. Moreover, the importance of sleep during pregnancy and the early postpartum period must be highlighted. Midwifery and/or nursing care starting while the mother is in a postnatal hospital stay can play a key role in preventing postpartum depression. - Psychometric properties of the Japanese version of the Occupational Fatigue Exhaustion Recovery Scale among shift-work nurses.

English, Scientific journal, OBJECTIVES: Shift-work nurses are at a higher risk of inadequate recovery from fatigue and developing maladaptive fatigue with significant health consequences. Therefore, it is necessary to monitor fatigue and recovery levels with a reliable scale. We investigated psychometric properties of the Japanese version of the Occupational Fatigue Exhaustion Recovery scale (OFER-J) for shift-work nurses. METHODS: Japanese shift-work nurses responded to self-administered questionnaires at baseline (n = 942) and one month later (n = 334). The confirmatory factor analysis (CFA) and exploratory factor analysis (EFA) were conducted to verify the structural validity and the correlation analysis and one-way analysis of variance were conducted to test the construct and discriminative validity. Cronbach's alpha coefficient, intra-class correlation coefficient (ICC), and smallest detectable change (SDC) were calculated to assess reliability. RESULTS: The CFA showed high correlations between the factors and whilst the goodness-of-fit of the three-factor model was suboptimal, it was in an acceptable range. Most modifications included the error covariance of the Acute Fatigue (AF) and Intershift Recovery (IR) items. The EFA showed that Chronic Fatigue (CF) and AF were not clearly separated, indicating that the two AF items dropped out. Construct and discriminative validity were also well indicated. Cronbach's alpha coefficients were 0.75-0.85. Only CF showed sufficient reproductivity (ICC = 0.74). The SDC for CF, AF, and IR was 14.0, 17.1, and 18.7, respectively. CONCLUSIONS: The validity and reliability of the OFER-J were verified as acceptable for shift-work nurses. This cross-sectional study evaluated factors that influenced symptoms of injection site infection among 238 Japanese patients who self-injected insulin for diabetes between October 2015 and January 2016. A structured questionnaire was used to collect data regarding skin disinfection practices, infection symptoms at the injection site, frequency of injections, environment at the time of injection, and hygiene habits. The majority of patients (83.2%) performed skin disinfection before the self-injection. Logistic regression analysis revealed that infection at the injection site was positively associated with skin disinfection before injection, age, and performing injections outside home. It was speculated that omitting skin disinfection before administering subcutaneous insulin injection was not the factor that affected the symptoms of injection site infection. The greatest contributor to infection symptoms was injections performed outside the home. Future studies focusing on the environment, in which patients administer insulin injections, to assess its influence on symptoms of injection site infections are warranted. - Effectiveness of bed bath methods for skin integrity, skin cleanliness and comfort enhancement in adults: A systematic review.

English, Scientific journal, AIM: To evaluate the effectiveness of bed bath methods for skin integrity, skin cleanliness and comfort enhancement in adults. DESIGN: A systematic review based on the PRISMA guidelines. METHODS: We searched for quantitative studies published between 2004-2020, using the PubMed, MEDLINE and CINAHL. The remaining 25 studies were appraised by the JBI tool. RESULTS: Only four of the included studies were of high quality. Studies of above moderate quality demonstrated that disposable towels were as effective as cotton towels for skin lesions and bacterial removal. Applying a hot towel maintained the skin barrier function and provided warmth; cotton towels were effective for cleaning even with weak pressure, and post-bed bath moisturizer treatment contributed to skin integrity. CONCLUSION: Although various methods have been examined, the available evidence is inadequate for establishing best practices. It is necessary to verify empirical research with rigorous methodology involving elderly inpatients and to develop instruments that measure patients' comfort. - Impact of Peer-Led Cancer Education Program on Knowledge, Health Beliefs, Practice, and Self-Esteem Among Pairs of Nepalese High-School Students and Their Knowledge-Sharing Partners.

English, Scientific journal, Raising cancer awareness among adolescents can increase their confidence in identifying cancer symptoms and develop healthy habits. This study tested the effectiveness of cancer education based on a new model among high schoolers. A non-randomized control group pre-post-test design study was conducted among 313 pairs of adolescent students and their knowledge-sharing partners in Lalitpur, Nepal. A baseline test was conducted before the education program, and it was followed up at two weeks and three months. Results were measured using a chi-square test, binary logistic regression, and a two-way repeated-measures ANOVA. There was a significant interaction effect of intervention and time on students' knowledge, beliefs, self-esteem, and practice, along with a change in some scores of knowledge-sharing partners. Joint assignment supported the idea of diffusion of information within the family and in the neighborhood. The peer group discussion could encourage active learning and help students to participate visibly in problem-solving and reflecting more sustainably. Time constraints, lack of human resources, and support groups, might limit this program's usage; however, preparing guidelines, and connecting communities, organizations, hospitals, volunteer health workers, and survivors can help make it more sustainable and approachable. - 「排便パターン分類フローチャート」の臨床での有用性

English, Scientific journal, AIM: To clarify the characteristics of expert nurses' assessments when selecting an insertion site for a peripheral venous catheter (PVC). METHODS: Participants were 11 competent (control group) and 13 expert nurses. Using a simulated patient, we recorded the procedures participants followed when selecting a site for a PVC insertion. The researchers interviewed the nurses after the procedure by asking targeted questions about the site selection to clarify the factors influencing that selection. During the interview, a video of that nurse's procedure was observed, and each step performed during the procedure was investigated. RESULTS: We identified three assessment characteristics specific to expert nurses that influenced their PVC site selection: (a) focusing on a patient's unique characteristics and choosing the appropriate procedure for that individual; (b) avoiding complications and paying attention to the patient's daily self-care needs; and (c) carefully considering the patient's fear and fatigue during site selection and catheter insertion. Other assessments, based on the general knowledge and skill acquired by nurses in selecting a PVC site, were common to both groups: arm selection based on the patient's preference and site selection to avoid nerve injuries or complications. The control group's approach was assessed on the basis of their confidence in selecting a site for a PVC insertion. CONCLUSIONS: Expert nurses assessed the patient's individual characteristics and daily self-care needs and helped mitigate the patient's anxiety. Our findings provide a basis for educational programs that share how expert nurses assess sites for a PVC insertion. - Is skin disinfection before subcutaneous injection necessary? English, Scientific journal, Nurses continue to disinfect the skin before administering subcutaneous injections as a standard process in clinical settings; despite evidence that disinfection is not necessary. To implement evidence-based practice, it is critical to explore why this gap between "evidence" and "practice" exists. This study aimed to describe the reasons offered by Certified Nurses in Infection Control (CNIC) in Japan for performing skin disinfection before subcutaneous injection. Adopting an inductive qualitative design, interviews were conducted with 10 CNIC in 2013. According to the participants, skin disinfection before subcutaneous injection: (a) was common practice; (b) may have been beneficial if it was omitted; (c) adhered to hospital norms; (d) prevented persistent suspicion of infection; (e) had no detrimental effect; (f) was an ingrained custom; and (g) involved a tacit approval for not disinfecting in home care settings. The themes (c) and (g) were cited as the main reasons affecting decision-making. The CNIC administered injections following skin disinfection in hospitals in accordance with hospital norms. On the contrary, outside the hospital, they administered subcutaneous injections without skin disinfection. All themes except (b) and (g) reflect the barriers and resistance to omitting skin disinfection, while (g) shows that it is already partly implemented in home care settings. It is necessary to create a guideline for skin disinfection before subcutaneous injection that considers the quality of life of patients at home, their physical conditions, and the surrounding environment at the time of injection, in addition to the guidelines applicable in hospitals. - 「気持ちよさ」をもたらす看護ケアによる看護師の反応(全国調査より)

Notably, applying a hot towel to the skin for 10 s (AHT10s) during bed baths helped to reduce the risk of skin tears and provided comfort and warmth in previous studies. However, it is still unclear whether autonomic nervous system is affected by bed baths. Thus, this study investigated the effect on the autonomic nervous activity of applying hot towels for 10 s to the back during bed baths. METHODS: This crossover study had 50 participants (25 men and women each; average age 22.2 ± 1.6 years; average body mass index 21.4 ± 2.2 kg/m2) who took bed baths with and without (control condition: CON) AHT10s on their back. Skin temperature, heart rate variability (HRV), and blood pressure (BP) were measured. Subjective evaluations and the State-Trait Anxiety Inventory in Japanese were also performed. RESULTS: A significant interaction of time and bed bath type on skin surface temperature was observed (p < .001). Regarding the means of skin surface temperature at each measurement time point, those for AHT10s were significantly higher than those for CON. Although the total state-anxiety score significantly decreased in both the bed bath types after intervention, the mean values of comfort and warmth were higher for bed baths with AHT10s than for CON (p < .05) during bed baths; AHT10s was significantly higher in warmth than CON after 15 min (p = .032). The interaction and main effects of time on HRV and BP and that of bed bath type were not significant. CONCLUSION: Bed baths that involved AHT10s caused participants to maintain a higher skin temperature and warmer feeling than under the wiping-only condition; they also provided comfort during the interventions. However, the bed baths with AHT10s did not allow participants to reach a relaxed state; moreover, there was no change in autonomic nerve activity. This may be due to participants' increased anxiety from skin exposure and the intervention being limited to one part of the body. - Combination of minimum wiping pressure and number of wipings that can remove pseudo-skin dirt: A digital image color analysis.

English, Scientific journal, BACKGROUND: Excessive wiping friction in skin care may lead to skin damage. Bed baths are required to remove skin dirt without affecting the skin barrier function; the wiping pressure and number of wipings that satisfy these two requirements have not been clarified. This study aimed to determine the minimum wiping pressure and number of wipings that can remove skin dirt. MATERIALS AND METHODS: In this quasi-experimental study, 50 healthy adults received an adhesion of pseudo-oily and aqueous dirt, randomly assigned to the left and right forearms. Each participant was wiped three times with wiping pressure classified into six randomly assigned categories. The dirt removal rate was calculated by color-analyzing images captured before and after each wiping, and its dependence on the combination of wiping pressure and number of wipings was assessed using a linear mixed model. RESULTS: The combinations achieving oily dirt removal rates of 80% or more were wiping once and pressure ≥50 mmHg, wiping twice and pressure ≥40 mmHg, and wiping thrice and pressure ≥10 mmHg. Aqueous dirt was removed almost completely by wiping once, even with pressure ≥5 mmHg. CONCLUSION: Wiping with at least 10 mmHg or more three times can sufficiently remove both oily and aqueous dirt. Dirt removal rates with weak pressure can be made about as effective as those achieved with strong pressure by increasing the number of wipings. This result can be applied to daily nursing, home care, and long-term care health facilities. - Effects of differences in wiping pressure applied by nurses during daily bed baths on skin barrier function, cleanliness, and subjective evaluations.

English, Scientific journal, Objective: We clarified the relationship between the degree of subjective fatigue, sleep, and physical activity among nurses working 16-hour night shifts in a rotating two-shift system. Materials and Methods: We investigated 15 nurses who were surveyed regarding their individual attributes, physical activity level (consumed calories), hours of sleep, sleep efficiency, sleep onset latency, sleep diary, and subjective symptoms. Nurses wore a Fitbit One (Fitbit Inc., San Francisco, CA, USA) for 7 consecutive days to measure sleep and physical activity. Results: Results were analyzed for nine participants, excluding those who withdrew or had missing data. The years of nursing experience, nurses' age, and the length of nocturnal awakening time of the high fatigue group were significantly longer than of the low fatigue group (p < .05). Years of nursing experience in the affiliated department of the high fatigue group was significantly shorter than of low fatigue group (p < .05). The number of nightshifts during the survey period was significantly higher in the high fatigue group than in the low fatigue group. Fatigue after work and body mass index (r = 0.46, p < .001), consumed calories (r = 0.30, p < .05), bedtime (r = -0.36, p < .05), and hours of sleep (r = -0.37, p < .01) were significantly correlated; however, the sleep indices were not correlated. Conclusion: We clarified that the degree of fatigue in nurses working 16-hour night shifts in a rotating two-shift system was related to individual factors, such as age, years of nursing experience, years of nursing experience in the affiliated department, number of nightshifts, and length of nocturnal awakening time. Nurses with greater fatigue had significant differences in their bedtime on days off and work days, which suggests that sleep rhythm may also affect fatigue. - Investigation of the Burnout Stages Experienced by Nurses Working in Hospitals and Related Factors

English, Scientific journal, BACKGROUND: Our previous study into assessing hospital cleanliness in Japan by two common methods, ATP bioluminescence and the stamp agar method, revealed considerable variability in the data of both methods (BMC Research Notes, 7: 121, 2014). To investigate the reason(s) for the variability, we reanalyzed the data (n = 752) from the point of view of the material surface properties of sampling sites. METHODS: Data obtained from surfaces with unknown properties and different purposes such as floor were omitted, and the remaining data (n = 488) were used for this study. The material surface properties on sampling sites were divided into six categories: melamine coated (n = 216), vinyl chloride (n = 16), stainless steel (n = 144), wood (n = 63), and acrylonitrile-butadiene styrene resin coated (n = 48). The data between individual material properties were compared. RESULTS: The ATP values of high-touch places were significantly different depending on the type of surface, but no significant difference in stamp values between material properties was seen, indicating that in contrast to stamp values, ATP-accumulation more depends on the physical properties of the material surface such as electronic charges or roughness. To confirm this, we assessed a degree of roughness on vinyl chloride material surface (disutilized floor samples actually used for each of the hospitals) by observation with scanning electron microscope (SEM). As a result, SEM observation similarly revealed considerable roughness on the materials, which may allow microbes to contaminate the materials without noticing it. CONCLUSION: Material properties must be considered when evaluating hospital cleanliness with ATP values, and provide a strong warning into evaluating hospital cleanliness. - Visualization of hospital cleanliness in three Japanese hospitals with a tendency toward long-term care.

English, Scientific journal, BACKGROUND: Hospital cleanliness in hospitals with a tendency toward long-term care in Japan remains unevaluated. We therefore visualized hospital cleanliness in Japan over a 2-month period by two distinct popular methods: ATP bioluminescence (ATP method) and the standard stamp agar method (stamp method). METHODS: The surfaces of 752 sites within nurse and patient areas in three hospitals located in a central area of Sapporo, Japan were evaluated by the ATP and stamp methods, and each surface was sampled 8 times in 2 months. These areas were located in different ward units (Internal Medicine, Surgery, and Obstetrics and Gynecology). Detection limits for the ATP and stamp methods were determined by spike experiments with a diluted bacterial solution and a wipe test on student tables not in use during winter vacation, respectively. Values were expressed as the fold change over the detection limit, and a sample with a value higher than the detection limit by either method was defined as positive. RESULTS: The detection limits were determined to be 127 relative light units (RLU) per 100 cm2 for the ATP method and 5.3 colony-forming units (CFU) per 10 cm2 for the stamp method. The positive frequency of the ATP and stamp methods was 59.8% (450/752) and 47.7% (359/752), respectively, although no significant difference in the positive frequency among the hospitals was seen. Both methods revealed the presence of a wide range of organic contamination spread via hand touching, including microbial contamination, with a preponderance on the entrance floor and in patient rooms. Interestingly, the data of both methods indicated considerable variability regardless of daily visual assessment with usual wiping, and positive surfaces were irregularly seen. Nurse areas were relatively cleaner than patient areas. Finally, there was no significant correlation between the number of patients or medical personnel in the hospital and organic or microbiological contamination. The purpose of this study was to investigate the relationship among the records from the high school, records of the entrance examination and records during the registered dietitians about the studdents who entered the A college. 1. The purpose of this study was to investigate the relationship among the records from the high school, records of the entrance examination and records during the baccalaureate nursing program about the students who entered the A college. The subjects were 175 students who entered the school on 2000 and 2001. The records during nursing program for two years were used for data. A leaving a school person was removed. The result was as follows : Although correlation of the records between the high school and the entrance examination wasn't seen, the records between nursing program and the entrance examination was seen. The English records during nursing program of the recommendation entered student were lower in some thought than the student who entered a school in the way of selecting others. Correlation of the records during nursing program wasn't seen among the individual personal interview and the group personal interview. Poor correlation was seen in the short essay examination and some of the records during nursing program. As for the subject test records of the entrance examination, correlation with the records during nursing program was seen in some subjects. The purpose of this investigation was to clarify student's studies from the reports which students described after the practices using simulated patients (SPs) in the physical assessment classes. The subjects were 89 students. The reports which students described were analyzed by qualitative approach, and categories were extracted. As that result, it could be classified in 5 categories of [Confusion for the SP] [Structures were connected] [Integration to the communication skills for the SP] [The meaning to give concerns to the SP] [The self noticed through the SP] and 18 sub-categories. The purpose of this study was to identify the types of touch and to describe the meaning of touch in the context. The subjects were 3 cancer patients and 10 nurses in the palliative care unit. The data was collected from the participant observation of patients and nurse in the care unit, semi-structured interview to the nurse about the scene, which a touch arose in. These data were analyzed by the qualitative and inductive approach. The findings were as follows. 1. The touch was classified by eight types; 'the touch which aimed at the working', 'the touch for the confirmation', 'the touch to give a comfort', 'The touch to keep safety', 'the touch to support patient's independence', 'the touch of sign', 'the touch reaches to feeling', 'the touch, instead of the word'. 2. The purpose of this study was to evaluate this physical assessment class. We analyzed the scale for process of the class, the student's reports presented about their thoughts, and self-assessment sheet which had used for physical assessment practice. This paper will report results of a questionnaire given to nursing students regarding the effectiveness of the present evaluation system for second year nursing students. As part of the training process in the Department of Health and Nursing at Tenshi Junior College, students are evaluated to find out how well they can integrate and utilize what they learned in the first year to prepare them for clinical practice afterwards. The evaluation consists of three steps, a written test to reinforce students' knowledge, a written report on nursing process using one case study, and a pre-clinical practice based on the nursing process in their written report. The student responses to the questionnaire showed the following: 1. Most students regard the training as necessary, although they feel evaluation is taxing. 2. The students' sense of being taxed is mainly caused by the written report on nursing process. 3. The educational and the psychological effect on students during the course of the training shows individual variation. 消毒剤や抗菌剤に頼らない温度による新たな病原体制御理論を創成し、空間や高頻度接触面の温度を制御することで、感染予防へと応用を目指すために、2年目は以下の研究を実施しいくつかの成果を得た。
1. 乾燥表面の温度調節による大腸菌の生存性の制御に湿度変化が与える影響: 私達は乾燥面を37℃付近に温めることでその乾燥麺に塗抹した細菌の生存性が顕著に低下することを見いだした。その一方で環境温度は、熱を奪う空気中の水蒸気量すなわち湿度の影響も受ける。そこで恒温恒湿機を使用し,温度(25-37℃)と共に湿度(45-90%)が乾燥表面の大腸菌の生存性に与える影響を調査した。その結果湿度と大腸菌の生存率には逆相関関係(r=-0.241)があり,湿度が上がるほど生存率は有意に低下した(p=0.04)。このように、乾燥面での温度制御において湿度によるネガティブな効果は最小限であることを見いだした。
2. 温度制御手摺デバイス上での細菌の生存性の可視化法の開発: バイク用ハンドヒーターを改良し作成したデバイスの効果を振れ幅の大きい培養に頼らず正確かつ簡便に確認する方法を、無蛍光の透明テープとLIVE/DEAD染色による測定系とキーエンス画像解析ソフトを組み合わせることで実現した。具体的には、ヒーターより距離が離れるほど手摺上の生存菌数は有意に低下し、その効果は温度ヒートマップと一致した。
3. 土壌細菌の空間移動に環境要因の変化が及ぼす影響について: 新型コロナ感染症の影響で公共の閉鎖環境での採材ができなかったので、その代替えとして3Dプリンターを用いて空気中に浮遊する細菌を効率よく生け捕りにできるエアサンプラーを用いて北大農場にて実施した。その結果、環境因子(気圧、蒸気圧、湿度、風向き)が連動し変化することにより、空気中に巻き上げられ浮遊し移動することが明らかになった。

本研究は、患者の周辺環境への接触とその接触面での細菌の生存を具体的な患者の日常生活動作レベルを精査し、適切な看護実践(環境整備)へ応用することを目指す。
医療施設において、病原体は手指やその他の媒介物による接触を介して、感受性のある人に移動する可能性があるため病原体で汚染された環境表面の接触伝播経路に着目した。特に患者や医療従事者の手指が接触する機会が多い、ベッド柵やオーバーベッドテーブル等の患者周辺環境に着目した環境の調査研究を計画した。
2020年度は、新型コロナウイルス感染症の影響を受け、患者が療養している環境での調査が困難となった。そこで、2021年度は新型コロナウイルス感染症が発生する前に調査した結果から交絡因子を検討し、患者周辺の環境微生物汚染度と日常生活動作レベルの差異との関連について、準実験研究を用いて検討する計画へ変更した。
準実験研究の目的は、模擬病室内の患者周辺環境において、日常生活動作レベルの差異と模擬患者の手が触れる接触面積やアデノシン3リン酸(ATP)値との関連を検討した。模擬患者(12名)に先行研究で看護師の動作が分析されている車いすへの移乗を依頼した。その際のベッド周辺環境への接触面積やATP値を測定した。日常生活動作レベルの差異と模擬患者の手が触れる接触面積には相関関係がなかった。しかし、ATPで評価した環境微生物汚染度は、日常生活動作レベルが高くなるほど高値を示した。本結果は、模擬患者の日常生活動作レベルの差異とベッド周辺環境の汚染度に関連がある可能性を示唆したと考える。

今年度は、1)在宅移行支援に関連する文献検討、2)外来患者を支援する看護実践(意思決定支援)に関する実態調査を実施した。1)においては、外来患者に対する在宅療養支援の特徴や実践的取り組み、先駆的な取り組みを行なっている医療施設での報告書を研究者間で検討した。高齢者への支援ニーズは疾患ニーズとともに生活ニーズを考慮した包括的な支援が必要であることを再確認した。同時に、患者経験や価値に基づく、支援の内容や方向性を示している文献は見当たらなかった。患者経験評価を中心とした質評価尺度は海外においては散見されており、それらの質評価指標の精査を進めるとともに、我々の調査で使用可能な指標を精査する予定である。
2)がん患者を支援する外来看護師の看護実践、特に患者経験の理解と患者の意向継続に関する看護実践に焦点を当てて横断調査を実施した。がん治療に関する部分では、外来の場面を外来治療室と規定し、国内のがん診療連携拠点病院の外来治療室に勤務する看護師を対象として配票調査(WEB調査)をした。その結果として、外来看護師は患者のこれまでの療養生活の経験、患者の価値観を理解することを実践していること、同時に患者の今後の意向を尊重するっことを重視していることが示された。一方で、患者のエンドオブライフに関わる患者の意向の把握には困難を感じており、患者経験理解と移行の継続のための方策の検討を行う必要性が示唆された。

令和3年度については、足浴プログラム作成に向けた基礎的な研究、および足浴プログラムの介入研究の準備に取り組んだ。
まず、フットケア看護技術としての足浴の効果に関する基礎的資料として、足浴方法の違いが生体および対象者の主観的評価にどのような影響を与えるかについて検証した。これらは、健康な成人を対象とした介入研究である。具体的には、通常の温湯による足浴と泡を用いた足浴を比較しているクロスオーバー試験を実施した。皮膚表面温度、皮膚水分量、主観的な温かさの感覚を評価した。これらの介入の結果、経過時間による違いはあったものの、両者の介入で同程度の効果が得られた。両郡間に皮膚水分量の変化量の有意差はなかったが、主観的な評価に違いがあることが示唆された。
足浴プログラムの介入研究の準備としては、これまでの期間において取り組んだ文献検討、および上記の結果も参考資料としながら、プロトコール作成に取り組んだ。足浴の時間、温度、深さ、そして測定の場所、タイミングなどを具体的に検討した。糖尿病を持つ高齢者への介入前の基礎的な研究として、健康な高齢者を対象とした介入研究を計画し、倫理審査委員会の承認を得ている段階である。しかし、パイロットスタディを終えた矢先、新型コロナウイルス感染症の感染拡大により調査の実施を中断することとなった。令和4年度には、作成済みのパンフレットを用いて、対象者のリクルートを開始する予定である。

清拭は、対象者の清潔を保持し、温熱刺激等による気持ちよさをもたらす看護ケアである。清拭時間が同じであるならば、誰もが質の高いケアを求めることは当然のことである。しかし、皮膚が脆弱で、スキンテア発生リスクが高い高齢者において、タオル素材に関わらず、どのように清拭すると(清拭圧、回数、温タオル貼用など)、安全で、気持ちよさを提供できるか、清拭方法のエビデンスが十分とは言えず、清拭ケアプログラムは確立されていない。私たちは、気持ちよく、かつ皮膚バリア機能を保持する清拭ケアを目指して、これまで清拭方法に着目して基礎的研究を進めてきた。本研究ではこれまでの成果を基盤とし、追研究を行うとともに、高齢者における安全で、気持ちよさをもたらす清拭ケアプログラムを開発し、その効果を検証する。
2021年度は、高齢入院患者約50名を対象に 弱圧と通常圧による3回の清拭を行う単盲検ランダム化クロスオーバー 試験を実施した。皮膚バリア機能(経表皮水分蒸散量TEWL・角質水分量SCH)の 経時的変化と主観的評価を比較した。その結果、弱圧でも3回の拭き取りで十分に汚れを除去できるため皮膚を強く拭く必要性はないことを可視化した。
また、清拭前の 皮膚乾燥度を簡易的に測定するために、Overall dry skin score ODS 日本語版の信頼性および妥当性を検証し、ODS日本語版を開発した。さらに、これまで清拭における主観的効果は十分に可視化されていないことに着眼し、清潔ケアを提供する看護師の認識から清拭を含む清潔ケアの意味を記述し、構造化した。清拭の効果を生理学的視点から可視化することに加えて、心理・社会的効果を検証することにつながる研究を進めた。

Regarding methodologies of new cancer education to protect our health from cancer, we, as a research object, developed and evaluated them by utilizing peer support and by focusing on the viewpoints of pupils. The policy of new education we have now examined showed that the fact that pupils find issues by themselves of their lives such as sleep and meals as well as those concerning environmental factors will arouse interest in cancer prevention and that it effectively led to the improvement, etc. of their knowledge about cancer, value belief about health and cancer, and health behavior. At the same time, it also produced effects that the consciousness and behaviors concerning health of those around them including their family were changed. This was considered to be new knowledge leading to cancer-prevention education on community base.

We develop the model of Comfort care.The purpose of nursing care to bring comfort is to make use of the bond between the patient and the family for the care, to fulfill the patient's wishes based on the relationship of trust with the patient, and to pride the professional as a nurse.The patient's response to care was the development of motivation and self-confident, feeling of gratitude, the opening of communication channels and the formation of rich interpersonal relationships.The nurse's response was with the patient's satisfaction with the care, confidence and willingness to care, sharing care with the team, and fostering a sense of nursing.
The effects of care were found to be obtained with combined care and multiple care rather than single care.

We integrated the previously developed hand bath results model with a system for analyzing hand bath care to create a post-stroke hand bath care program. The participants showed positive physical and mental changes after receiving hand bath care. However, the question of whether foot bath, a similar warm bath, would have the same effect, or whether the effect is unique to hand baths, as well as whether there are any long-term effects, such as on ADLs or length of hospitalization, has remained unclear, and so the intervention has not been widespread nor established in clinical practice. Therefore, the objective of this study was to demonstrate the long-term effectiveness of introducing the post-stroke hand bath care program and to develop guidelines for its implementation, in order to encourage its widespread use and integration into medical facilities.

Our previous study into assessing hospital cleanliness in Japan by two common methods, ATP bioluminescence and the stamp agar method. Although a weak correlation was found between ATP-based assessments and stamp-based assessments, fluctuations by day were observed in those numbers, and the error bars were large. Therefore, to search for these factors, we used next generation sequencing to survey the microbial community membership of each sample from each hospital. The three hospitals were of different sizes, and we tested for inter-hospital differences in microbial community structure.We surveyed the microbial diversity in each of 50 environmental samples from three hospitals of different sizes. Although taxonomic diversity differed between hospitals A and B, intestinal bacteria were particularly overrepresented in hospital C. These results suggest that bacterial flora may be related to factors influencing the level of cleanliness within a hospital.

This study was conducted as part of a project to formulate a hand bathing care model to help patients improve their well-being and accommodate the post-stroke life. Its purpose was, firstly, to validate the clinical effects of a model showing the hand movement and verbal communication improvement process in hand bathing care of stroke patients in the convalescent phase which had been developed by the author and, secondly, to develop a software-based tool which could be used to analyze the quantity and quality of verbal communication of such patients.
It was verified that, after receiving hand bathing, the patients became more aware of their hand movement and talked more about oneself and more positively. The “hand bathing care analysis system” was developed and data obtained with it were analyzed to examine its usability and identify any problems.

The purpose of this study was to develop a hand-bathing care outcome model for stroke patients in the convalescent phase. Stroke patients in convalescent-phase rehabilitation wards who met the inclusion criteria, were randomly allocated to hand-bathing or control groups. Repeated measures analysis of variance showed significantly greater improvement in subjective hand movements in the hand-bath group. An interaction effect between intervention and point of measurement was observed from repeated measures ANOVA for the proportion of positive adjectives, with significantly higher results in the hand-bathing group. The path model indicated that hand-bathing had primary effects for subjective improvement in hand movements and an increase in the proportion of positive narratives and secondary effects in terms of increased social relationships and increased satisfaction and sense of health.

本研究は、血圧測定(マンシェット装着)、採血、手浴の看護技術に焦点をあて、(1)看護学生の技術習得過程における習得困難な動作を明確にすること、(2)質の高い看護技術を提供していると予測される熟練看護師の経験知を言語化すること、(3)その結果をもとに看護技術の教材を開発し、その有効性の検討を試みることを目的とし、研究活動を行った。その結果、熟練看護師の経験知を抽出することの看護技術教育における有効性および可能性が示唆された。

主に研究の目的3:臨床の看護師が、どのように薬理学の知識を看護判断、実践などに役立てているか、新人看護師にとって困難なことは何かなど現状の問題点、基礎教育との関連を実態調査する。に焦点をあて、臨床看護師を対象として面接調査を実施した。面接は、半構成的質問紙を用いて実施した。質問紙の構成は、1.看護師の個人的特性、2.与薬の際必要とされた知識とアセスメント内容、看護への適応、3.与薬ミスの体験とその要因、4.新人看護師の困難状況、5.新人看護師の状況から考え得る基礎教育に必要とされる能力とした。対象とした臨床看護師(経験年数6、12、13年)3名には、文書で研究の主旨、プライバシーの保護、結果及び公表には匿名性を確保することを説明した上で、同意が得られた場合、研究承諾書にサインを得た。<結果・考察>与薬の際、看護師は、薬剤を使用する目的・副作用・用量・用法・禁忌・相互作用・薬物動態などが不明であれば、必ず文献または添付文書をもとに確認していた。また、これらをもとに、対象の生体の変化を予測し、対象・家族への説明を考慮し、対象の観察をし、効果の確認及び副作用の早期発見に努めている。また、対象のアセスメントをもとに、薬剤の作用と生活の関連性を判断し、看護実践に活用していることが予測された。また、新人看護師の困難状況として、薬剤の使用と患者の状況とを総合してアセスメントし、看護につなげていくには時間がかかる現状が述べられた。この要因として、薬剤の目的・薬物動態などを調べることは出来ても、対象を中心として理解することが困難、薬剤の身体への影響についての認識が不十分であることなどがあげられた。このような状況から、今後の基礎教育に期待することとして、与薬の手技を原則に基づいて確実に行えること、与薬することが対象及び対象の生活にどのように影響するのかを理解できるような教授方略の必要性が示唆された。平成15年度は、臨床看護師を対象とした面接調査を継続し事例を積み重ねていくこと、また新人看護師への面接を通して何に困難を感じているか、その要因は何かを明らかにし、看護実践に必要とされる薬理学について検討を深めていきたい。
