English, Scientific journal, Importance: Enhancing purpose in life is vital in cognitive interventions for older adults, yet its application in occupational therapy remains limited.
Objective: To evaluate the effectiveness of the occupation-focused Color Narrative program compared with common Japanese community-based preventive care.
Design: Randomized controlled trial.
Setting: Community centers.
Participants: Community-dwelling older adults (N = 44).
Intervention: The Color Narrative program included group-based storytelling using 100 color cards as prompts (90 min), independent engagement in meaningful occupations (3 mo), and group reflection (90 min). Control participants attended health education and group recreational activities (90 min each).
Outcomes and Measures: Primary outcomes included cognitive function (Mini-Mental State Examination– Japanese version [MMSE–J]) and purpose in life (Life Engagement Test [LET]). Secondary outcomes included domain-specific cognitive functions (Digit Span, Tapping Span, Symbol Search, Wisconsin Card Sorting Test), meaning in life (Meaning in Life Questionnaire–Short Form), functional status (FRAIL Scale, Frenchay Activities Index, 12-item Short-Form Health Survey).
Results: A two-way analysis of variance showed significant interactions for MMSE–J, F(1.76, 73.89) = 4.531, p = .017, η2p = .097, and LET, F(1.56, 58.78) = 8.743, p = .001, η2p = .172. Post hoc tests revealed declined and stable MMSE–J scores in the control and intervention groups, respectively (corrected p = .031, Cohen’s d = 0.80). LET scores increased in the intervention group (corrected p = .041, Cohen’s d = 0.78).
Conclusions and Relevance: The Color Narrative program enhanced purpose and prevented cognitive decline, highlighting the significance of occupational therapy in preventive care.
Plain-Language Summary: This study investigated the impact of the Color Narrative program on improving cognitive health and a sense of purpose in life among older adults. The program included group-based storytelling using 100 color cards as visual prompts, personalized goal setting, and independent engagement in meaningful occupations over 3 mo, followed by a group reflection session. Compared with individuals who participated in a standard community-based health education and recreation program, the participants in this program maintained cognitive function and reported an increased sense of purpose. The findings revealed the significance of maintaining cognitive function and purpose in life in improving older adults’ overall well-being. English, Scientific journal, PURPOSE: Loneliness is a serious concern among older rural men. This study examined how social and solitary activities are associated with each other and collectively relate to loneliness through path analysis. Two models were hypothesized: (1) solitary activities performance is associated with social activities, thereby relating to loneliness, and (2) social activity participation is associated with solitary activities, which in turn relates to loneliness. METHODS: Data were collected from 510 men aged 65 years and older residing in a rural Japanese town using self-administered questionnaires. The study measured loneliness (Japanese version of the UCLA Loneliness Scale), performance in social and solitary activities (Self-completed Occupational Performance Index), number of social activities (Vivid Social Activities Checklist), and depressive symptoms (Geriatric Depression Scale). Structural equation modeling was conducted; model fit was assessed using absolute, incremental, and parsimony indices. FINDINGS: Model 2 demonstrated acceptable fit (χ2 = 0.923, comparative fit index = 1.000, normed fit index = 1.000, root mean square error of approximation<0.001, Akaike information criterion = 38.009, Expected Cross-Validation Index = 0.074, and Hoelter = 208.044). Social activities were associated with solitary activities, which were in turn related to loneliness. Solitary activities were indirectly associated with loneliness through depression. CONCLUSIONS: This study demonstrates that higher performance in social activities may support healthier, more autonomous solitary engagement as a component of the social-solitary balance that shapes loneliness among older rural men. Locally grounded opportunities that enable meaningful social connection in addition to enhancing rural work rhythms and self-directed solitary routines may help reduce loneliness and support well-being in later life., 40655543 - Nurses’ Attitudes and Clinical Judgment on Skin Disinfection Before Subcutaneous Injection: Impact of Setting, Experience, and Normative Beliefs

English, Scientific journal, Background/Objectives: Skin disinfection before insulin administration is widely regarded as essential for preventing injection-site infection. However, the World Health Organization advises that while hand hygiene and washing with soap and water are crucial, alcohol-based disinfection before subcutaneous injection is not required. Thus, the necessity for pre-injection (subcutaneous) skin preparation remains controversial. Therefore, this study aimed to clarify the determinants of nurses’ attitudes toward the necessity of skin disinfection before subcutaneous injection. We simultaneously examined the effects of workplace setting, years of professional experience, and social norms to identify the most significant factors influencing clinical judgment. Methods: Nurses employed in wards, outpatient settings, and home care settings were surveyed between October 2021 and January 2022 in this cross-sectional study. A structured questionnaire assessed frequency of skin disinfection and attitudes regarding its necessity before subcutaneous injection. Ordinal logistic regression was performed to identify factors associated with the attitude of nurses. Results: Overall, 992 valid responses were analyzed. Ordinal logistic regression indicated that the attitude of nurses were significantly influenced by years of professional experience (Odds Ratio [OR] = 0.98, 95% Confidence Interval [CI] [0.96, 0.99]), normative expectations (OR = 2.88, 95% CI [2.32, 3.56]), and sanctions (OR = 1.36, 95% CI [1.15, 1.62]) (all p < 0.001). Conclusions: Nurses’ beliefs regarding skin disinfection before subcutaneous injections are primarily influenced by normative expectations and professional experience, rather than workplace environment and experiential expectations. English, Scientific journal, Although social activity reduces loneliness levels, this relationship is not always consistent. Some older adults experience high loneliness despite active social participation. We aimed to clarify the characteristics of these individuals by focusing on their preference for solitude and satisfaction with and performance in valuable social and solitary activities. This cross-sectional survey included 3205 individuals aged ≥65 years and assessed loneliness, participation in social activities, preference for solitude, performance of valuable social and solitary activities, and depression. Participants were categorized into 3 groups based on the relationship between loneliness and social activity participation: higher social activity with a tendency toward loneliness, lower social activity without a tendency toward loneliness, and a correlated group. A one-way analysis of variance was conducted to clarify each group’s characteristics. Structural equation modeling was used to explore the structure underlying the discrepancy between loneliness and social activity. Older adults experiencing high loneliness despite active social engagement preferred solitude, had low performance of valuable social and solitary activities, and reported high depression. In the high loneliness despite social activity group, a higher preference for solitude was associated with a decreased performance of social activities valuable to the individual, leading to reduced performance of solitary activities valuable to the individual. These effects contributed to increased depression, further exacerbating the discrepancy between loneliness and social activity. Some older adults may experience little to no benefit from increasing social activities. Aim
To explore how perceptions of hygiene care practices among nurses in acute‐care wards change over their clinical experience.
Methods
Based on symbolic interactionism, we employed a grounded theory methodology to explore interrelationships of meaning arising from actors' perceptions and interactions. Thirty‐three nurses working in acute‐care wards for >3 years were selected by purposive snowball and theoretical sampling; semi‐structured individual interviews were conducted from October 2023 to February 2024. Data were analyzed for 32 participants who provided consent.
Results
The core category was “adjusting care channels”; seven other categories also emerged. Care channels refer to options for hygiene care practices. When nurses are first assigned to a ward, they regard hygiene care as the “bare task under organization rules.” However, through repeated processes of “emerging caring with patient resonance” and “internalizing the meaning of care,” they can “form care channels.” Subsequently, nurses become practitioners who can transform practices of hygiene care into “care to be restricted” or “wealth of ‘life behind the curtain’” by “adjusting care channels” according to the patient and ward organization situation. In contrast, if nurses fail to internalize the meaning of care, they end up performing “pro forma activities.”
Conclusions
Our findings highlight the importance of nurses to improve their ability to “adjust care channels,” which would allow them to provide hygiene care in a manner appropriate to the patient and organizational context. These results contribute to the development of educational strategies that foster nurses' values of hygiene care and support strategies by nursing managers. - Loneliness and social participation among older Japanese adults: The influence of gender on social participation

Aim
To explore the recognition of pregnant nurses on how they managed their health conditions to examine safe working strategies.
Design
A qualitative study with a grounded theory approach.
Methods
Twenty‐one nurses engaged in work during their pregnancy were recruited and interviewed using a semi‐structured questionnaire from January to June 2021. The data were analysed using a constant comparative method.
Results
The core category ‘duelling roles’ and the four other categories emerged. Pregnant nurses understand the ‘weight of one’ of being a professional in the workplace. Therefore, despite their health concerns, they struggle to complete their work as one team member to avoid inconveniencing others. However, through experiencing various nursing situations, they ‘perceive one's limits’ of working as they had done before pregnancy and protect their health and patients. Nevertheless, interactions with patients and their colleagues bring ‘delight in nursing’, which encourages them to continue working. Pregnant nurses thus develop a ‘prioritizing the foetus’ working style to continue being nurses while protecting their health.
Implications for the Profession and/or Patient Care
These results provide meaningful guidance in considering safe job retention strategies for pregnant nurses. Sharing and developing the ‘prioritizing the foetus’ mindset and management skills gained by the participants may be beneficial for the appropriate health management of pregnant nurses. The study may also facilitate nursing managers' understanding of the experiences of pregnant nurses and encourage them to consider reviewing nursing practices.

English, Scientific journal, This study aimed to verify the effectiveness of an application (app) in establishing social connectedness among unacquainted older men, as well as improving their physical health. The nine participants were men aged 65 and older in the subarctic zone of Hokkaido, Japan. A mix of quantitative and qualitative methods were adopted as the study design. A questionnaire survey was completed before and after the intervention, and a semi-structured interview was conducted after the intervention. An app-installed smartphone was loaned to the participants, which allowed them to share pictures, voice recordings, and their step count. Quantitative data were analyzed using descriptive statistics and the Wilcoxon signed-rank test, and qualitative data were analyzed using qualitative descriptive analysis to generate categories. The average age of the participants was 77.7 years. The relationship between participants who were interacting for the first time through the app advanced as their understanding of each other’s personalities deepened. The average step count during the third and fourth months was significantly higher than in the first two months. By using the app, older men were able to build relationships with one another. Japanese, 目的:大学と自治体との共同研究のプロセスを報告し,共同研究を推進させた要因を検討する.
方法:対象は都市部と地方の2つの自治体とした.まず,自治体ごとに共同研究のプロセスを記述したシートを作成し,共同研究の推進要素を研究者の観点から整理した.次に,保健師へインタビューを実施し,インタビュー内容から共同研究の推進要素を保健師の観点から整理した.上記2つの結果をもとに,公衆衛生看護の共同研究を推進させた要因を検討した.
結果・考察:共同研究を推進させた要因として,保健師と研究者の対等な関係性のなかでの目的共有があった.また,住民を第一義的にとらえる保健師の認識とそれに基づくコーディネートや,日々の活動を通じた保健師と住民との関係構築があった.さらに,共同研究と地域の健康課題解決の方向性を一致させる目的の再共有や,保健師の保健師活動に対する熱意や認識が示された. - Differences of expected intervention effects between participant-led and facilitator-led preventive care services in Japan

English, Scientific journal,Objective To examine the effect of different management methods on the effectiveness of care preventive programmes for community-dwelling older adults.Methods This study comprised two facilitator-led (FL) and one participant-led (PL) preventive care classes in Japan. All participants received the intervention for approximately 12 weeks. Functional assessments, occupational dysfunctions, and subjective health were measured before and after the interventions. A two-way mixed design analysis of covariance (ANCOVA) was adopted to examine the effect of the interventions, adjusted for previous experiences with preventive care services. The level of significance was set at P < 0.05.Results Fourteen participants in the PL group (76.64 ± 6.48 years, 92.9% women) and 29 participants in the FL group (76.55 ± 5.75 years, 75.9% women) were included in the statistical analysis. ANCOVA showed significant group × time interaction effects in the Five Times Sit-to-Stand Test (FTSST), the Timed Up & Go (TUG), occupational deprivation of the Classification and Assessment of Occupational Dysfunction Scale, and self-rated health. Simple main effect tests showed that the TUG decreased significantly in the PL group, while occupational deprivation and self-rated health scores improved significantly. In contrast, FTSST scores significantly improved in the FL group.Conclusion PL-type management may be more appropriate for preventing social isolation and withdrawal, while FL-type management may be more appropriate for preventing physical frailty. Selecting not only adequate programmes but also an appropriate management type that matches the service purpose can help provide more effective care preventive services. - Is skin disinfection before subcutaneous injection necessary? English, Scientific journal, Nurses continue to disinfect the skin before administering subcutaneous injections as a standard process in clinical settings; despite evidence that disinfection is not necessary. To implement evidence-based practice, it is critical to explore why this gap between “evidence” and “practice” exists. This study aimed to describe the reasons offered by Certified Nurses in Infection Control (CNIC) in Japan for performing skin disinfection before subcutaneous injection. Adopting an inductive qualitative design, interviews were conducted with 10 CNIC in 2013. According to the participants, skin disinfection before subcutaneous injection: (a) was common practice; (b) may have been beneficial if it was omitted; (c) adhered to hospital norms; (d) prevented persistent suspicion of infection; (e) had no detrimental effect; (f) was an ingrained custom; and (g) involved a tacit approval for not disinfecting in home care settings. The themes (c) and (g) were cited as the main reasons affecting decision-making. The CNIC administered injections following skin disinfection in hospitals in accordance with hospital norms. On the contrary, outside the hospital, they administered subcutaneous injections without skin disinfection. All themes except (b) and (g) reflect the barriers and resistance to omitting skin disinfection, while (g) shows that it is already partly implemented in home care settings. It is necessary to create a guideline for skin disinfection before subcutaneous injection that considers the quality of life of patients at home, their physical conditions, and the surrounding environment at the time of injection, in addition to the guidelines applicable in hospitals. - Exploring Students’ Intercultural Sensitivity in the EFL Classroom

English, Scientific journal,Objective/background The rapidly increasing incidence of patients with dementia in Japan is creating an urgent demand for evidence-based occupational therapy (EBOT), which has been reported to improve clinical efficacy. This study aimed to examine the current practice of EBOT for patients with dementia in Japan and clarify factors influencing its application.Methods We conducted an anonymous, self-administered questionnaire survey by mail. The participants were 432 occupational therapists treating patients with dementia at 432 designated medical institutions nationwide. Descriptive statistics were calculated, and multiple regression analysis was performed to clarify the factors influencing the present application of EBOT.Results The response rate was 31.3%. Among the participants, 46.3% responded ‘somewhat frequently’ or more (5, 6, and 7 on a 7-grade scale) to a question on the frequency of practice of EBOT. Using multiple regression analysis with the stepwise method, we selected the model with the highest degree of fit. This model extracted three factors, namely, ability to understand scientific papers, sufficiency of means of getting information, and availability of advice, which had standardized partial regression coefficients (β) of 0.419, 0.214, and 0.158, respectively.Conclusions The three factors extracted using the multiple regression analysis indicate that in order to encourage EBOT for patients with dementia, occupational therapists need to acquire reading comprehension skills, so that they can assess the quality of scientific papers. English, Scientific journal, Previous studies have indicated that older men often experience disconnection from the community after retirement. Social activities have been shown to be effective in preventing social isolation among older urban men. Nevertheless, it has been reported that they often do not participate in community social activities and tend to be reluctant to do so. We explored the values and meanings of social activities for retired older men living in an urban area of Japan to understand support using social activities that are more suitable for them. Semi-structured interviews were conducted with 15 older men (aged 68-80 years; M = 74.6 ± 3.79 years) about their interactions with family and non-family members, and their participation in various community social activities. The grounded theory approach was used for the analysis. As a result, the following five categories were derived as the values that participants place on the social activities that they engage in: "health as a resource and reward for social activities," "feeling I am still useful," "feeling that something is my responsibility," "feeling of time well spent," and "finding interest through interactions." In addition, the following three categories were extracted as meanings of social activities: "fulfilling social life," "maintaining stable family relationships," and "maintaining safety and peace in the community." When considering the social activities that older urban retired men are interested in and likely to participate in, these five values can be considered indicators. In contrast, to maintain stable family relationships and safety and peace in the community, participants sometimes used strategies to stop or abandon social activities. English, Scientific journal, The coronavirus disease 2019 (COVID-19) pandemic has exposed older adults to health and social risks. This study examined the perceptions of community-dwelling older adults regarding how COVID-19 restricted their daily lives. Six focus-group interviews were conducted with 24 participants (mean age, 78.2 ± 5.5 years) living in urban and rural areas in Japan. Then, a qualitative inductive content analysis was performed. Six themes were generated: “fear of infection and public, watchful eyes,” “consistency in daily personal life,” “pain from reducing my social life,” “readiness to endure a restricted life,” “awareness of positive changes in myself,” and “concern for a languishing society.” There was no change that would make their lives untenable, and they continued their daily personal lives at a minimum level. However, their social lives were reduced, which over the long term can lead to a lost sense of purpose in life. This was reported as an adverse factor in the development of other diseases and functional decline in previous studies. 社会的孤立・孤独は、さまざまな疾患のリスク因子となっており、孤立・孤独を生まない体制や社会の構築は喫緊の課題であるが、十分な体制が構築されているとはいえず、社会の理解も進んでいるとは言い難い現状がある。このため、当事者は自身の気持ちを表出することに抵抗感を感じてしまう。また、孤立・孤独予防を目的とした活動を含め、高齢者が集う地域の場で男性が少ないことが、全国共通の課題になっている。その背景として、既存の地域の場が「男らしさやニーズに十分な配慮をしないまま運営されてきた」ことが挙げられる。
本プロジェクトでは、「シチズンサポートプロジェクト」として、a)高齢男性向けの新たな居場所(コミュニティー・シェッド)の立ち上げ、b)孤立リスクを簡便に可視化できるツール(社会的つながり可視化ツール)の開発を通し、コミュニティー・シェッドを運営するとともに、その効果を検証する。具体的には、プロジェクト期間内に熊本県水上村および北海道札幌市においてコミュニティー・シェッドを立ち上げ、社会的孤立・孤独の一次予防システムとして有効に機能するか、フィールド調査および心理・脳・健康の調査から検証する。また、「社会的つながり可視化ツール」をコミュニティー・シェッドの効果検証に活用するとともに、地域の支援者や医療専門職が住民の孤立・孤独リスクのスクリーニングや地域診断に活用できるツールとして社会実装することを目指す。これらを通じて、日本独自のコミュニティー・シェッドの在り方を提案し、実践と研究の両輪で高齢者の社会的孤立・孤独に立ち向かい、望まない孤立・孤独を未然に防ぐ取り組み、すなわち一次予防システムの構築につなげる。
