Aim

To explore how perceptions of hygiene care practices among nurses in acute‐care wards change over their clinical experience.

Methods

Based on symbolic interactionism, we employed a grounded theory methodology to explore interrelationships of meaning arising from actors' perceptions and interactions. Thirty‐three nurses working in acute‐care wards for >3 years were selected by purposive snowball and theoretical sampling; semi‐structured individual interviews were conducted from October 2023 to February 2024. Data were analyzed for 32 participants who provided consent.

Results

The core category was “adjusting care channels”; seven other categories also emerged. Care channels refer to options for hygiene care practices. When nurses are first assigned to a ward, they regard hygiene care as the “bare task under organization rules.” However, through repeated processes of “emerging caring with patient resonance” and “internalizing the meaning of care,” they can “form care channels.” Subsequently, nurses become practitioners who can transform practices of hygiene care into “care to be restricted” or “wealth of ‘life behind the curtain’” by “adjusting care channels” according to the patient and ward organization situation. In contrast, if nurses fail to internalize the meaning of care, they end up performing “pro forma activities.”

Conclusions

Our findings highlight the importance of nurses to improve their ability to “adjust care channels,” which would allow them to provide hygiene care in a manner appropriate to the patient and organizational context. These results contribute to the development of educational strategies that foster nurses' values of hygiene care and support strategies by nursing managers.

Health Management of Working Pregnant Nurses: A grounded theory study

Aim

To explore the recognition of pregnant nurses on how they managed their health conditions to examine safe working strategies.

Design

A qualitative study with a grounded theory approach.

Methods

Twenty‐one nurses engaged in work during their pregnancy were recruited and interviewed using a semi‐structured questionnaire from January to June 2021. The data were analysed using a constant comparative method.

Results

The core category ‘duelling roles’ and the four other categories emerged. Pregnant nurses understand the ‘weight of one’ of being a professional in the workplace. Therefore, despite their health concerns, they struggle to complete their work as one team member to avoid inconveniencing others. However, through experiencing various nursing situations, they ‘perceive one's limits’ of working as they had done before pregnancy and protect their health and patients. Nevertheless, interactions with patients and their colleagues bring ‘delight in nursing’, which encourages them to continue working. Pregnant nurses thus develop a ‘prioritizing the foetus’ working style to continue being nurses while protecting their health.

Implications for the Profession and/or Patient Care

These results provide meaningful guidance in considering safe job retention strategies for pregnant nurses. Sharing and developing the ‘prioritizing the foetus’ mindset and management skills gained by the participants may be beneficial for the appropriate health management of pregnant nurses. The study may also facilitate nursing managers' understanding of the experiences of pregnant nurses and encourage them to consider reviewing nursing practices.

The Consolidated Criteria for Reporting Qualitative Studies checklist was used to ensure the quality of research reporting.

Members of the nursing team were involved in the design, conduct and interpretation of the data in this study.

英語, 研究論文（学術雑誌）, This study aimed to verify the effectiveness of an application (app) in establishing social connectedness among unacquainted older men, as well as improving their physical health. The nine participants were men aged 65 and older in the subarctic zone of Hokkaido, Japan. A mix of quantitative and qualitative methods were adopted as the study design. A questionnaire survey was completed before and after the intervention, and a semi-structured interview was conducted after the intervention. An app-installed smartphone was loaned to the participants, which allowed them to share pictures, voice recordings, and their step count. Quantitative data were analyzed using descriptive statistics and the Wilcoxon signed-rank test, and qualitative data were analyzed using qualitative descriptive analysis to generate categories. The average age of the participants was 77.7 years. The relationship between participants who were interacting for the first time through the app advanced as their understanding of each other’s personalities deepened. The average step count during the third and fourth months was significantly higher than in the first two months. By using the app, older men were able to build relationships with one another. 日本語, 目的：大学と自治体との共同研究のプロセスを報告し，共同研究を推進させた要因を検討する．

方法：対象は都市部と地方の2つの自治体とした．まず，自治体ごとに共同研究のプロセスを記述したシートを作成し，共同研究の推進要素を研究者の観点から整理した．次に，保健師へインタビューを実施し，インタビュー内容から共同研究の推進要素を保健師の観点から整理した．上記2つの結果をもとに，公衆衛生看護の共同研究を推進させた要因を検討した．

結果・考察：共同研究を推進させた要因として，保健師と研究者の対等な関係性のなかでの目的共有があった．また，住民を第一義的にとらえる保健師の認識とそれに基づくコーディネートや，日々の活動を通じた保健師と住民との関係構築があった．さらに，共同研究と地域の健康課題解決の方向性を一致させる目的の再共有や，保健師の保健師活動に対する熱意や認識が示された．

Differences of expected intervention effects between participant-led and facilitator-led preventive care services in Japan

This study comprised two facilitator-led (FL) and one participant-led (PL) preventive care classes in Japan. All participants received the intervention for approximately 12 weeks. Functional assessments, occupational dysfunctions, and subjective health were measured before and after the interventions. A two-way mixed design analysis of covariance (ANCOVA) was adopted to examine the effect of the interventions, adjusted for previous experiences with preventive care services. The level of significance was set at P < 0.05.

Fourteen participants in the PL group (76.64 ± 6.48 years, 92.9% women) and 29 participants in the FL group (76.55 ± 5.75 years, 75.9% women) were included in the statistical analysis. ANCOVA showed significant group × time interaction effects in the Five Times Sit-to-Stand Test (FTSST), the Timed Up & Go (TUG), occupational deprivation of the Classification and Assessment of Occupational Dysfunction Scale, and self-rated health. Simple main effect tests showed that the TUG decreased significantly in the PL group, while occupational deprivation and self-rated health scores improved significantly. In contrast, FTSST scores significantly improved in the FL group.

PL-type management may be more appropriate for preventing social isolation and withdrawal, while FL-type management may be more appropriate for preventing physical frailty. Selecting not only adequate programmes but also an appropriate management type that matches the service purpose can help provide more effective care preventive services.

Is skin disinfection before subcutaneous injection necessary? The reasoning of Certified Nurses in Infection Control in Japan

英語, 研究論文（学術雑誌）, Nurses continue to disinfect the skin before administering subcutaneous injections as a standard process in clinical settings; despite evidence that disinfection is not necessary. To implement evidence-based practice, it is critical to explore why this gap between “evidence” and “practice” exists. This study aimed to describe the reasons offered by Certified Nurses in Infection Control (CNIC) in Japan for performing skin disinfection before subcutaneous injection. Adopting an inductive qualitative design, interviews were conducted with 10 CNIC in 2013. According to the participants, skin disinfection before subcutaneous injection: (a) was common practice; (b) may have been beneficial if it was omitted; (c) adhered to hospital norms; (d) prevented persistent suspicion of infection; (e) had no detrimental effect; (f) was an ingrained custom; and (g) involved a tacit approval for not disinfecting in home care settings. The themes (c) and (g) were cited as the main reasons affecting decision-making. The CNIC administered injections following skin disinfection in hospitals in accordance with hospital norms. On the contrary, outside the hospital, they administered subcutaneous injections without skin disinfection. All themes except (b) and (g) reflect the barriers and resistance to omitting skin disinfection, while (g) shows that it is already partly implemented in home care settings. It is necessary to create a guideline for skin disinfection before subcutaneous injection that considers the quality of life of patients at home, their physical conditions, and the surrounding environment at the time of injection, in addition to the guidelines applicable in hospitals.

英語, 研究論文（学術雑誌）, The rapidly increasing incidence of patients with dementia in Japan is creating an urgent demand for evidence-based occupational therapy (EBOT), which has been reported to improve clinical efficacy. This study aimed to examine the current practice of EBOT for patients with dementia in Japan and clarify factors influencing its application.

We conducted an anonymous, self-administered questionnaire survey by mail. The participants were 432 occupational therapists treating patients with dementia at 432 designated medical institutions nationwide. Descriptive statistics were calculated, and multiple regression analysis was performed to clarify the factors influencing the present application of EBOT.

The response rate was 31.3%. Among the participants, 46.3% responded ‘somewhat frequently’ or more (5, 6, and 7 on a 7-grade scale) to a question on the frequency of practice of EBOT. Using multiple regression analysis with the stepwise method, we selected the model with the highest degree of fit. This model extracted three factors, namely, ability to understand scientific papers, sufficiency of means of getting information, and availability of advice, which had standardized partial regression coefficients (β) of 0.419, 0.214, and 0.158, respectively.

The three factors extracted using the multiple regression analysis indicate that in order to encourage EBOT for patients with dementia, occupational therapists need to acquire reading comprehension skills, so that they can assess the quality of scientific papers. Furthermore, it is important to create environments where they have access to publications and can discuss research with superiors and colleagues.

The values and meanings of social activities for older urban men after retirement

Social activities have been shown to be effective in preventing social isolation among older urban men. Nevertheless, it has been reported that they often do not participate in community social activities and tend to be reluctant to do so. We explored the values and meanings of social activities for retired older men living in an urban area of Japan to understand support using social activities that are more suitable for them. Semi-structured interviews were conducted with 15 older men (aged 68-80 years; M = 74.6 ± 3.79 years) about their interactions with family and non-family members, and their participation in various community social activities. The grounded theory approach was used for the analysis. As a result, the following five categories were derived as the values that participants place on the social activities that they engage in: "health as a resource and reward for social activities," "feeling I am still useful," "feeling that something is my responsibility," "feeling of time well spent," and "finding interest through interactions." In addition, the following three categories were extracted as meanings of social activities: "fulfilling social life," "maintaining stable family relationships," and "maintaining safety and peace in the community." When considering the social activities that older urban retired men are interested in and likely to participate in, these five values can be considered indicators. In contrast, to maintain stable family relationships and safety and peace in the community, participants sometimes used strategies to stop or abandon social activities. Therefore, in situations where a peaceful life within a family or neighborhood is threatened, it may be useful to help set aside sufficient time and allow for psychological leeway in advance to incorporate social activities into their lives., 13712651

Perception of COVID-19 Restrictions on Daily Life among Japanese Older Adults: A Qualitative Focus Group Study

This study examined the perceptions of community-dwelling older adults regarding how COVID-19 restricted their daily lives. Six focus-group interviews were conducted with 24 participants (mean age, 78.2 ± 5.5 years) living in urban and rural areas in Japan. Then, a qualitative inductive content analysis was performed. Six themes were generated: “fear of infection and public, watchful eyes,” “consistency in daily personal life,” “pain from reducing my social life,” “readiness to endure a restricted life,” “awareness of positive changes in myself,” and “concern for a languishing society.” There was no change that would make their lives untenable, and they continued their daily personal lives at a minimum level. However, their social lives were reduced, which over the long term can lead to a lost sense of purpose in life. This was reported as an adverse factor in the development of other diseases and functional decline in previous studies. While there is no doubt that infection prevention is important, supporting older adults in engaging in activities that provide a sense of purpose in life could contribute to their present and future overall health including mental health., 13712651

日本語, :地域で暮らす高齢者の"自助"や"互助"の醸成に向けて，困難に対する高齢者自らの対処戦略を知ることが有用となる．本研究の目的は，地域で生活する高齢者の老年期における作業的困難とその対処過程を明らかにすることとした．地域で暮らす高齢者４名に個別インタビューを行い質的記述的に分析した．その結果，コアカテゴリー『求め続ける可能性』と，10個のカテゴリーが抽出された．コアカテゴリー『求め続ける可能性』は，高齢者は自らの作業への可能性を簡単には捨てないということを意味する．高齢者の"可能性を求め続ける"ことをサポートすることが，高齢者にとって自分らしい満足した生活の実現につながると期待する．
