  Japanese, 動的矯正歯科治療を終了した両側性唇顎口蓋裂患者23例(全例男性、平均20歳10ヵ月)の顎顔面形態の特徴と咬合について検討した。対照は正常咬合者男子30名で、永久歯列期の側面頭部X線規格写真分析値を資料として用いた。正常咬合者群との比較で、両側性唇顎口蓋裂群でSNAが77.9±3.7°、SNBが75.3±5.3°と上下顎の後方位がみられ、ANBが2.6±4.1と上下顎関係にはAngle Class III傾向がみられた。また、前上顔面高が大きく、後上顔面高は小さく、口蓋平面の傾斜が急傾斜を示していたが、咬合平面の傾斜に有意な差はみられなかった。症例裂群と正常咬合者群の間で有意な差がみられた前歯部咬合関係について他の計測項目とのスピアマン順位相関係数は、両側性唇顎口蓋裂群では水平的および垂直的前歯部咬合関係のいずれも上顎前歯部の歯・歯槽高のみで、それぞれ有意な正の相関が認められた。

  Japanese, Since 1990, several kinds of specialists in Hokkaido University Hospital have gathered under a team approach for cleft lip and palate patients. Prosthetic treatment is the final procedure by this team approach, and has a good prognosis that can be predicted well. The final restoration for cleft patients should maintain good function for a long time, and has an important role in preventing post-orthodontic relapse. We investigated the validity of prosthodontic treatment over the past 16 years.

  Japanese, Impacted maxillary canines (impacted canines) are observed at a relatively high frequency. It is important to understand the impaction condition as various treatment options are available. Therefore, the objectives of this study were to classify the impaction conditions and determine the frequency of impacted canines. 1,379 patients were selected from among 1,905 patients examined at Hokkaido University Hospital Orthodontic clinic between November 1, 2001 and October 30, 2008. Those who changed hospital or had systemic diseases were excluded. Impaction was detected from panoramic radiograph when (1) a distance of 5mm or more from the cusp to the occlusal plane and (2) a tipping angle of the tooth axis of 6.0° or more from the midline. Patients who complied with one or both of these conditions were included after excluding those with submerged teeth in the intraoral photos. The impacted canines were classified according to the vertical position, horizontal position and tipping angle. 46 patients with 79 impacted canines were examined. In the vertical position, the distance from the cusp to the occlusal plane was mostly less than 25.0mm. The cusps most often corresponded to the horizontal position of the deciduous canines. The most common tipping angles ranged from 10.0°to 20.0°. The classification and frequency of the impaction condition of impacted canines were determined in this study.

  Japanese, To improve typical maxillary dental arch collapses and underdevelopment which were observed in unilateral cleft lip and palate patients, orthodontists frequently use maxillary lateral expansion appliances. Clinically, there seem to be differences of movement between segments of cleft sides and those of non-cleft sides. The current study investigated the three-dimensional morphological changes on the upper dental arch treated with the rapid maxillary lateral expansion in unilateral cleft lip and palate patients who had maxillary dental arch collapses and bilateral posterior crossbite. Two Japanese males (expanded at 16 years, 4 months of age and 16 years, 2 months of age) and 2 Japanese females (expanded at 15 years, 5 months of age and 15 years, 1 months of age) with unilateral cleft lip and palate were examined. In all cases, the upper dental arches had been expanded with fixed rapid expansion screw appliances in the permanent dentition stages, orthodontically. The whole surface of the upper and lower dental casts of all cases were measured and recorded by the non-contact three-dimensional shape measuring system before and after lateral expansion, respectively. In all cases, following the rapid maxillary lateral expansion, segments of cleft sides were moved more effectively than those of non-cleft sides. This finding may be related to the characteristic anatomical structures in unilateral cleft lip and palate patients.
• Clinical Practice as an Undergraduate Course of the Dental Hygienist student at the Orthodontic Clinic of the Hokkaido University Hospital

  Japanese, The aim of this study was to evaluate the educational situation around clinical training of dental hygienist students at Hokkaido University Hospital. A questionnaire survey was conducted with a total of 41 dental hygienist students from three training institutions for dental hygienists who had completed of orthodontic clinical training. The questionnaire was aimed to analyze the status of the educational program and to identify problems in the educational situation at the clinic of the authors as an educational facility. The following points were established. (1) Over 85% of the students were aware of the General Instructional Objectives (GIO) before clinical training, and also checked these during the training. (2) Over 80% of the students considered that they achieved the GIO and that Hokkaido University Hospital had provided adequate educational programs. (3) Over 85% of the students thought that instruction was "Intelligible" or "Somewhat intelligible" and that teaching materials were adequate. (4) Comments by satisfied students included "studying from many cases", "detailed instructions from the staff", "instruction from their own teachers", and similar. Among the dissatisfied responses were "There are few chances to learn the service of hygienist as it actually takes place", "short training period", and similar. (5) About 30% of the students met with some risky situation during the training. (6) Over 80% students had zeal for their occupation after training, while the remaining had a negative image.

  北海道大学歯学部の専門教育期に行われる臨床基礎実習のうち，歯科矯正学教室が担当する歯科矯正学臨床基礎実習は舌側弧線装置作製実習と症例分析・診断学実習で構成されている.本実習の目標は「歯科矯正学講義で習得した基礎的，臨床的知識を基に症例分析法・矯正治療，技工技術を身に付けること」である.このようなものは一部パフォーマンス課題と呼ばれるものと考えられるが，従来型の評価法では適正な評価が行われない部分である.昨今，教育カリキェラムがプロセス基盤型からアウトカム基盤型へ移行するに伴い，ポートフォリオなどが徐々に取り入れられており，パフォーマンス系の課題にも対応可能な事が知られている.今回われわれは将来的なポートフォリオ評価の導入も視野に入れることも踏まえ，特にアウトカムに着目したルーブリックを実習にて試行し，有用性を評価することを目的とした.対象は平成21年度の3年次学生(平成22年度の4年次学生)46名とした.これらの学生に対して行われた舌側弧線装置作製実習と症例分析・診断学実習に際して，ルーブリックを用いてアウトカムに対する自己評価をさせた.得られたルーブリックから各項目の平均点を算出し比較した.結果より，両実習ともにプロフェッショナリズムは漸増する傾向がみられ，初回よりも最終回で有意に高かった.さらに，全体に舌側弧線装置作製実習の平均値よりも症例分析・診断学実習のそれが高い傾向を示した.この結果より，将来的なポートフォリオ評価を念頭におくこともふまえ，ルーブリックは歯科矯正学臨床基礎実習の実習に対する応用に有効なツールであることが示唆された., 北海道歯学会, Japanese

  近年，アウトカム基盤型教育の浸透とともに，ポートフォリオの使用が学習ツールとしても評価としても重要視されている.現在，歯科矯正学臨床基礎実習の4年次前期に行われる症例分析・診断学実習に対しては，将来的なポートフォリオの導入も視野に入れ，「ルーブリック」と「振り返り」を導入している.今回われわれは，この「ルーブリック」と「振り返り」が学習者である学生にとってどの程度の効果があったかを調べるためにアンケートを行った.対象は2010年の4年次学生55名である.これら学生に対して，症例分析・診断学実習における「ルーブリック」および「振り返り」について各々に無記名式のアンケートを行った.アンケート内容はルーブリックや振り返りの効果や実習への反映を問うものである.結果より，「ルーブリック」「振り返り」ともに，学習ツールと しての有効性を認める学生が多かった事，アウトカムの重要性や振り返りを認識できた学生が少なからずいた事，がわかった.一方，「ルーブリック」は日常の学習への反映と言う点ではうまくできていない学生もみられた.これらより，「ルーブリック」「振り返り」の利用は歯科矯正学臨床基礎実習にとって有効である事が示唆された., 北海道歯学会, Japanese

  In this study, the effects of treatment of anterior maxillary advancement using distraction osteogenesis were investigated. The subjects were three severe skeletal class III patients who needed surgical orthodontic treatment for mandibular retrusion (one female aged 16 years and 5 months of age, one female aged 28 years and 9 months, and one male and 26 years and 3 months at the initial examination). At the initial examination, extra-oral clinical examination revealed concave profiles and intra-oral examination revealed crowding in the upper arches in all patients. In presurgical orthodontic treatment of the maxilla, anterior maxillary advancement was performed using an intraoral appliance a palatal expansion screw from a few days after the maxillary osteotomy. Lateral expansion was performed simultaneously in one case. Anterior maxillary advancement was performed at a rate of 1 mm of advancement per day for seven to nine days. Data were obtained from maxillary dental casts, profile facial photographs and lateral cephalograms before and just after the completion of expansion. Just after the completion of expansion, the upper incisor edges had moved 4.8-9.1 mm anteriorly. In the patient whose palate was also expanded laterally, the inter-premolar width increased to 6.8 mm. It was found that SNA increased 3.51-5.12 degrees, SNB decreased 1.17-0.31 degrees, NSP-A increased 4.4-5.6 mm and U1-SN increased 7.10-11.17 degrees. Anterior maxillary advancement using distraction osteogenesis resulted in creation of sufficient space in the upper arches to cancel the arch length discrepancies in the upper arches.   The purpose of this study is to predict the mandibular position following correction of functional anterior crossbite. Multiple regression analyses were carried out using data obtained from lateral cephalograms in the intercuspal and rest positions at the first examination and at an examination after correction of anterior crossbite in 19 patients. The following results were obtained. 1. For Δ SNB", a multiple regression equation (a multiple correlation coefficient : 0.76) was obtained when S・Pog-OP (Upper) , NF-MP, ANB and overbite were used as the explanatory variables. 2. For Δ B", a multiple regression equation (a multiple correlation coefficient : 0.87) was obtained when S・Pog-OP (Upper), ANB, NF-OP (Upper), Δ B (X)' and SNB were used as the explanatory variables. 3. For Δ B (X)", a multiple regression equation (a multiple correlation coefficient : 0.82) was obtained when S*Pog-OP (Upper) , ANB, NF-MP and Δ SNB' were used as the explanatory variables. 4. For Δ B (Y)", a multiple regression equation (a multiple correlation coefficient : 0.39) was obtained when overjet was used as the explanatory variable. • Quantitative analysis of dental compensation for variations in saggital jaw relationships

  The aim of this study was to evaluate dental compensation with variations in sagittal jaw relationships quantitatively. For this purpose, we examined seventy-three adult females with normal incisor relationships and a Class I molar relationship, and performed cephalometric analysis to evaluate sagittal jaw relationships, maxillary and mandibular incisor inclinations, and the cant of the occlusal planes. Our previous study had examined the sagittal jaw relationships of the subjects with skeletal class III and skeletal Class I only, but this time we extended from skeletal Class II to skeletal Class III categories. Correlation analysis was also performed between the skeletal and dental parameters and found that the most appropriate cephalometric parameters describing dental compensation quantitatively were SN-AB as a skeletal measurement and SN-U1, SN-L1, and SN-OP as dental measurements. With these parameters, a 1.0° shift in the sagittal jaw relationship from skeletal Class II to skeletal Class III, corresponds to the upper incisor inclining more labially by 0.6°, the lower incisor inclining more lingually by 1.1° and the occlusal plane flattening by 0.6°.   As far as the dental compensation with skeletal Class III jaw relationship is concerned, researches were carried out on the basis of the lateral cephalograms. However, it is often observed that cases with antero-posterior disharmony also have disharmony of the width of the dental arch. So we think that the dental compensation should not be investigated only by upper and lower incisor but also the lateral teeth. Especially, because canine is the place where curve of the dental arch is most strong, compensatory changes are considered to vary. In the present study dental compensation in treated cases with skeletal Class III jaw relationship was investigated both by model and cephalometric analysis. 1. When compared to Class I molar relationship cases, Class in molar relationship cases showed lingual inclination of lower incisor and distal inclination of lower canine in relation to the occlusal plane. Furthermore, in Class III molar relationship cases either labial inclination of upper canine or lingual inclination of lower canine was observed. 2. As the jaw relationship became worse, occlusal plane became flatter in Class I molar relationship group. But flattening of occlusal plane wasn't enough in Class III molar relationship group. So there was antero-posterior disharmony of upper and lower dental arch. 3. • 3D Craniofacial Growth Simulation System in Cleft Lip and Palate Patients

  The prediction of the quantity of frontal growth of the maxilla at the pretreatment stage was carried out in cleft lip and palate patients who used maxillary protraction appliance at an early period. 22 unilateral cleft lip and palate patients (13 males, 9 females) were selected from the Orthodontic Clinic of Hokkaido University Dental Hospital who already received lip and palate repair. The mean age of these 22 patients at the pretreatment stage was 6 years 7 months (Ranging between 5 years 7 months to 8 years 2 months). All these patients had undergone maxillary protraction appliance therapy with the treatment duration more than one year. The palate was repaired by push back mechanism in these cases at the age of about 1 year 6 months, in the Department of Oral Surgery of the same hospital. Frontal and lateral cephalograms taken at pretreatment stage and at an interval of one year were used for the analysis. Using the frontal cephalogram and applying the Andoh etc method, curveture of the nasal septum was measured. From the lateral cephalogram 2 angular and 4 linear cephalometric measurements were measured. The correlation analysis between the amount of frontal growth caused by the appliance and the pretreatment craniofacial pattern was carried out. Further, multiple regression analysis was also done. The following results were obtained :
  1) The correlation coefficients between the amount of frontal growth of maxilla following the use of maxillary protraction appliance and post upper facial height and also the curveture of nasal septum presented significant results. However the results were not sufficient for prediction of the amount frontal growth of the maxilla.
  2) The formula for the prediction of the amount of frontal growth of the maxilla was established by multiple regression analysis using the data of the craniofacial pattern at pretreatment stage.
  3) It can be suggested that from the established formula, the amount of frontal growth of the upper jaw can be predicted.
