• A Bayesian Network–Based Browsing Model for Patients Seeking Radiology-Related Information on Hospital Websites: Development and Usability Study

  English, Scientific journal, BACKGROUND: An increasing number of people are visiting hospital websites to seek better services and treatments compared to the past. It is therefore important for hospitals to develop websites to meet the needs of their patients. However, few studies have investigated whether and how the current hospital websites meet the patient's needs. Above all, in radiation departments, it may be difficult for patients to obtain the desired information regarding modality and diagnosis because such information is subdivided when described on a website. OBJECTIVE: The purpose of this study is to suggest a hospital website search behavior model by analyzing the browsing behavior model using a Bayesian network from the perspective of one-to-one marketing. METHODS: First, we followed the website access log of Hokkaido University Hospital, which was collected from September 1, 2016, to August 31, 2017, and analyzed the access log using Google Analytics. Second, we specified the access records related to radiology from visitor browsing pages and keywords. Third, using these resources, we structured 3 Bayesian network models based on specific patient needs: radiotherapy, nuclear medicine examination, and radiological diagnosis. Analyzing each model, this study considered why some visitors could not reach their desired page and improvements to meet the needs of visitors seeking radiology-related information. RESULTS: The radiotherapy model showed that 74% (67/90) of the target visitors could reach their requested page, but only 2% (2/90) could reach the Center page where inspection information, one of their requested pages, is posted. By analyzing the behavior of the visitors, we clarified that connecting with the radiotherapy and radiological diagnosis pages is useful for increasing the proportion of patients reaching their requested page. CONCLUSIONS: We proposed solutions for patient web-browsing accessibility based on a Bayesian network. Further analysis is necessary to verify the accuracy of the proposed model in comparison to other models. It is expected that information provided on hospital websites will be improved using this method.

  Japanese, 地理情報システム(GIS)を用いて受療動向、通勤通学動向、買物動向の分析を行い、北海道における現在の21二次医療圏の圏域を検証し、新たな二次医療圏を提案した。二次医療圏別およびに入院・外来別に算出したPIより、現在の二次医療圏で医療が完結していない市町村の大部分は圏域境界に位置した。7二次医療圏の7町2村で二次医療圏の見直しが必要とされ、新しい二次医療圏を提案した。

  Japanese, 地域住民の視点から、小児の救急医療体制において重要となる評価基準を明らかにすることを目的として、階層分析法により分析した。調査は2006年1,2月に、岩見沢市の四つの幼稚園に通う3歳から5歳迄の子供のいる保護者を対象とし、調査票を配布して行った。上位2階層(総合目的と評価基準)に着目して分析を行い、総合目的は「子供が急病の時の医療機関の選択」とした。評価基準は予備調査などから七つを選択した。有効回答数は73であった。各評価基準の重要度を比較したところ、「小児科の医師がいること」が全体の4割を占め、比較的大きかった他の評価基準「病院の評判が良い」「以前に受診したことがある」を4倍以上引き離していた。保護者は小児科専門医の診療を望んでおり、小児科医の存在が保護者の安心につながっていると考えられた。

  Japanese, タイムスタディで得られたデータを人員と時間毎に配置し、業務グループ毎に色分けして視覚的に分かりやすく表示する業務分布図を開発した。更に、最大エントロピー法を用いた周波数解析を行い、業務に内在する周期的特徴を明らかにしたが、業務の基点を表している位相の情報を抽出できなかった。そこで、位相の情報を明らかにするため、変動する周期的特徴の推移を抽出できるセグメント時系列解析を加えた分析を行い、業務改善の前後の相違を評価できるか検討した。その結果、タイムスタディの時系列解析によって周期的特徴の時間的推移を抽出でき、三次元スペクトルアレイで視覚化することで容易に認識できた。この手法を用いて業務改善の前後で周期的特徴の変化を比較し、新たなタイムスタディの評価が可能と思われた。

  Japanese, 病院医療情報システムの導入状況を日本と中国の間で比較するため、両国の医療機関に対してアンケート調査を実施した。対象は両国の500床以上の総合病院、各200施設で、2006年4〜6月に各施設の医療情報責任者にあてて自記式調査表を2回に分けて郵送した。調査内容は、病院情報システム(システム)の代表的機能、システム導入による影響、及びコンピュータ化する代表的な機能の3点、各10項目で、得られた回答を分析した。その結果、日本のシステムのうち、初期の医事会計機能の導入は中国より約20年早いことがわかった。その他の機能は日本では長い時間をかけて普及してきたが、中国では最近の10年間で急速に普及した。また、中国の病院情報責任者はシステム導入の効果を日本より高く評価していた。

  当院では、2013年9月の医療情報システム更新時に、患者の皮膚病変などの情報を医療チーム内で共有できるよう、医療用デジタルカメラ統合システムを導入しており、患者情報と撮影者情報が添付された画像を看護記録の患者経過記録および看護過程記録に掲載し、医療チーム内で共有するシステムを構築している。今回、同システムの活用状況を、システムに登録された画像件数の推移と、部署への聞き取り調査により検討した。その結果、システムの運用は各部署での操作説明会が終了した2014年12月以降、本格化しており、画像登録件数は、200〜250枚/月となっている。聞き取り調査では、システムの運用による、看護師の皮膚病変などに対する意識の前向きな変化が確認できた。, 日本医療情報学会看護部会, Japanese

  In our previous study, we applied frequency analysis on the sequential time study data of nurses in a busy in-patient ward, in order to extract periodic feature of the nursing service. In this paper, we prepared and analyzed a pair of simulated data of time study, in which either continuous or fragmented work schedule are assigned. Comparison of these simulated data with a pair of actual data, taken before and after the change of work schedule, was performed to examine whether the effect of the schedule change could be recognized and evaluated by the analysis. The simulated data was generated based on the total amount of classified items observed in the actual time study. The segment time series analysis was performed at the range of segment time series of 24hours and the shift of segment time of one hour for two days. The maximum entropy method (MEM) was used to calculate the power spectrum. The result of segment time series analysis of the simulated data featured a short cycle area and a medium to long cycle area. In the short cycle area, detailed variation of the cycle was clearly recognized, while the long term trend appeared in the medium to long cycle area. It is expected that the analysis of simulated data provides the visual assessment of the change of cycles of nursing service when applied to the data of actual time studies before and after the change of work schedule, as reported in our previous work.   Background: For the automatic retrieval of medical terms from clinical documents, the problems still remain in processing the noun-noun compounds. In addition, the semantic relations extracted from the compounds are useful for building the ontologies in the domain. Purpose: The purpose of the study is to investigate the distribution of the semantic relations between the two atomic medical terms of the noun-noun compounds extracted from the clinical documents. Methods: 100 clinical reports for CT examination generated in July 2005 were collected in a random way at Hokkaido University Hospital. The medical noun-noun compounds were extracted from the reports. We indexed the UMLS semantic type, group for each atomic medical term in the compounds, extracted the UMLS semantic relation according to the index, and analyzed the distribution of the semantic relations. Results: Only 29.9% of the compounds had the UMLS semantic relation defined (mainly including "location_of" and "adjacent_to"), and 70.1% of them undefined.    We developed a HIV/AIDS information network in Hokkaido University Hospital in order to make HIV/AIDS information public rapidly and to make it use effectively. The homepage of HIV/AIDS information, which consisted of two parts: one for medical staff and the other for public, was constructed. In 1997, the electronic HIV/AIDS-Consultation-Manual was published for medical staff in Hokkaido University Hospital through hospital LAN, as well as for those all over the country through UMIN (University Medical Information Network). This time, HIV/AIDS-Treatment-Manual as a new part was added to the homepage of HIV information for public. Contents of the homepage and structures of the system were introduced in this paper. In addition, the operational status and problem were reported according to the practical use of the system.
