  • A Case of Prosthodontic Treatment for a Patient with Extensive Tooth and Lip Loss due to Trauma

    Japanese, The patient was a male in his 30s with the chief complaints of esthetic and masticatory disorder due to missing upper and lower front teeth and flap reconstruction of the lip soft tissue. Seven months had passed since upper lip revision surgery was performed by a plastic surgeon, but the lower lip still had volume from the flap reconstruction. Prosthodontic treatment was requested prior to further surgery. The prosthesis fabrication was planned according to the usual method, and the patient himself chose dentures. However, depending on the condition of the soft tissues after surgery, each step until the completion of the denture continued through trial and error, but after the completion of the denture, the patient himself got used to it faster than the prosthodontist had expected, and a certain degree of satisfaction was obtained with the insertion of the prosthetic device.

    In the future, we would like to study a treatment workflow with more predictability and efficiency by utilizing digital technologies such as intraoral scanners and simulation software for facial analysis and denture duplication.

  • History of and Current Situation Regarding Dental CAD / CAM Systems and Future Perspectives.

    English, Scientific journal, The Dental CAD/CAM system has been in use for more than 30 years, and has been an indispensable tool for dental workflow. Before this practical system was in operation, it was necessary to develop computer technology in the handling of 3-dimensional data freely and to advance new dental materials. This trend will continue to expand in the future. Recently, optical impressions are taken instead of using impression materials against the background of great progress in sensor technology. Likewise, dental workflows that do not need plaster working models have become mainstream. In this paper, we outline the history and current trends of this technology, and describe prospects for the future.
  • Construction method of a tooth crown model using data generated by micro-focus CT - Voxel models of anterior and premolar teeth -

    English, Scientific journal, Currently, a number of dental CAD/CAM systems are in operation in Japan. Many of them adopt a non-contact optical system using a CCD camera and light patterns to measure the model teeth. One disadvantage of this method is that it is difficult to measure the parts in the shade. However, this problem can be solved if we can carry out measurements using 3D CT. In this study we have examined how to construct a 3D model of a tooth crown shape using micro-CT data. An R_mCT2 micro-CT manufactured by Rigaku Corporation was used under maging conditions of FOV10(φ10 mm×H10 mm), 90 kV tube voltage and 160 μA tube current. Basic study models manufactured by KaVo Dental GmbH were used in the imaging, which was carried out in two phases, as the size of the artificial teeth was greater than that of the FOV. The image data was output in 512 slices in the form of DICOM files, and transferred to a personal computer. We examined the CT values of all the voxels of all images using software we developed ourselves. Based on the results, we determined the threshold by comparing the distribution of the CT values of the artificial teeth area with those of the remaining area in each image, and extracted the shape of the artificial tooth by binarizing the image. The separate image data was aligned by detecting the location of the closest number of pixels to the binarized image. Artifacts included in the images were removed manually and the completed voxel data output in DICOM format. Shape was confirmed using free DICOM viewer software(OsiriX). As a result, the margin of error for superimposing the contour shape of the split image data is approximately one voxel(± 20 μm). Artifacts were seen in 20 to 50 slices of the binarized images but because they were minor, we were able to process the images quickly and easily. In this study we were able to construct a high-resolution model of artificial teeth with relative ease. In future, if we can success in realizing low-cost, higher performance dental cone-beam CT, we will be able to use it to measure abutment teeth.

    Japanese, Scientific journal, 北海道大学病院歯科診療センターでは,2001年からRapid Prototyping Techniqueによる粉末凝結式積層造型装置を導入し,この装置により作製した顎顔面模型(以下RP顎顔面模型)を用いて手術シミュレーションを行なっている.そこでわれわれは,当院におけるRP顎顔面模型の臨床応用の実績と概要を報告する.対象は2001年1月から2009年1月までに北海道大学病院歯科診療センターでRP顎顔面模型を用いて手術シミュレーションなどを行った症例とした.症例の顎顔面をCT撮影し,骨データを抽出して金属アーチファクトを除いたデータを取得した.これらのデータを三次元画像表示解析ソフトウェアで三次元構築処理し,積層造形装置Z402で症例のRP顎顔面模型を作製した.8年間でRP顎顔面模型を使用した症例は139例であった.その内訳は,顎矯正手術93例,顎堤形成術・骨移植術22例,悪性腫瘍切除術・顎骨再建術15例,口蓋形成術4例,インプラント埋入手術3例,エピテーゼ2例であった. 症例1:小下顎症と顔面非対称の顎矯正手術において,RP顎顔面模型を骨切りし,上下顎骨形成術をシミュレーションして手術を行なった. 症例2:骨格性右側方開咬の顎矯正手術において,RP顎顔面模型を骨切りし,移動骨片の異動量・方向,骨延長装置を決定し,骨延長法をシミュレーションして手術を行なった. 症例3:右下顎歯内癌の腫瘍切除術と顎再建術においてRP顎顔面模型を用いて下顎骨区域切除と再建をシミュレーションして手術を行なった. 症例4:耳介エピテーゼの製作に際し,RP顎顔面模型によりエピテーゼの原型を作製した.これらのRP顎顔面模型の臨床応用は,治療計画の困難な症例において,精度と安全性を高めるうえで有用であった.

    English, 著者等はセンサブルテクノロジー社製の感触モデリングシステムに着目した.このシステムは,フォースフィードバックを有する感触デバイスPHANToMと三次元CADソフトFreeFormから構成されており,触覚に関して対話性の高いインターフェイスを有している.三次元CAD上で自由にモデルを触ったり動かすことができるようになったため,二次元のスライス画像上で処理を行うのではなく,三次元的に金属アーチファクトを判別し除去することができるようになった.実際に感触モデリングシステムを用いてみた結果,金属アーチファクトを除去して頭蓋模型データを構築するのには極めて有効であり,処理時間の短縮にもなることが確認された

    English, Scientific journal, Purpose : This paper proposes the application of haptic modeling to a virtual waxing-up system to be used for dental education and dental CAD systems.
    Materials and Methods : A mandibular right first molar model was selected from the tooth crown model data library we had previously created from the three-dimensional coordinate data of plaster models enlarged four times. After partially damaging the occlusal surface shape of the tooth model, we reconstructed its original shape using a haptic modeling system consisting of a haptic force feedback device called the PHANToM® and 3D modeling software called FreeForm®.
    Results : The FreeForm modeling system enables easier control of the functional shape of the occlusal surface than previous modification methods that handled the control points of various free-form surfaces with the 2D mouse. Additionally, the haptic interfaces were extremely useful for resolving visual ambiguities ; the sense of touch led to greater accuracy.
    Conclusion : We affirmed that the use of this haptic modeling system can provide a significant benefit to dental education and dental CAD systems.

    Japanese, Scientific journal, Currently, it is getting difficult to obtain extracted teeth used for cutting training of dental students, due to the improvement of oral hygiene and dental treatments. To cope with such situation, a virtual reality-based tooth-cutting simulator has been developed. This simulator is not necessarily suitable for cutting training of many dental students, due to its high cost. Then, we developed a personal computer-based simulator, Decs/2, which enables us to practice the preparation of dental cavities. This paper describes its subsystem capable of evaluating a prepared dental cavity in comparison with its reference. Each voxel of the prepared tooth model was evaluated into 5 categories such as "normal", "excessive" and "insufficient". Such evaluation was visualized in a 3-D presentation and a 2-D cross-section of the prepared tooth model by using pseudo-color, enabling us to visually check improperly prepared portions of the tooth.

    Japanese, Scientific journal, A 33-year-old female had a class II malocclusion with a severe unilateral scissors bite. Initial records showed the overerupted upper left premolars and molars, and lingo-inclined lower left premolars and molars. The lower first molars and the second molar of the right side were missing. The upper left molars impinged on the buccal gingiva of the lower arch in habitual occlusion. She also complained of difficulty of closing her lips. Orthodontic treatment followed by surgical procedures was performed. Extraction of the upper first premolars was performed at first for the elimination of crowding in the upper arch, and then one of these teeth was transplanted to the lower right posterior segment as the orthodontic anchor. After the alignment of the upper arch, the upper left posterior segmental osteotomy and the lower left posterior segmental corticotomy were performed for the correction of the scissors bite. The lower left posterior segment was expanded buccally after surgery, and then the upper and lower anterior segments were retracted orthodontically. Finally fairly good functional occlusion was obtained, and her lips could be competent at rest.

    English, surface dataを生成する方法を検討し,軟組織の形態を評価するための基礎として,咬筋を抽出することを試みた.咬筋とその周囲の各ボクセルのCT値と座標を詳細に検討し,そのCT値を15〜95とした.抽出方法は以下の通りである.1)注目点のボクセルのCT値が筋組織の値の範囲内にある.2)注目点のボクセルに対し三次元的な周囲26近傍のボクセルの中で注目点を通り対称の位置にあるボクセル対(13通り)を考え,そのうち一方のCT値が筋組織を示す値の範囲内に含まれかつ他方がそれ以外の値をとるようなボクセル対が一つでも存在する.結果は以下の通りである.1)本方法では,咬筋の境界は幅を持った線として抽出され,原画像と比較して,その外周はほぼ適正と思われた.2)近似したCT値を持ち近接する耳下腺との境界は,比較的明瞭に示された.3)咬筋の境界は部分的に骨周囲を誤って抽出した.4)前頭面画像において,本方法では境界は,つながった線として抽出された.5)本方法は,従来法よりも約20%処理に多くかかったが,100秒を越えるものではなかった.今回用いた方法では,パーソナルコンピュータ上で半自動的に境界が検出され,得られたデータは,手作業によるデータの修正なしに上下スライス間の連続性を保っており,従来法と比べてトータルのデータ処理時間を大幅に短縮できた.又,得られたデータから,咬筋及び咬筋付き頭蓋骨模型を作製できた

    Japanese, Scientific journal, The extracted teeth for cutting training of the students in department of dentistry are difficult to obtained, due to the improvement of oral hygiene. Therefore, a tooth-cutting simulator was developed, in order to compensate for actual cutting training. This virtual reality-based system, however, is still very expensive, it is not suitable for the practice in dental school which needs a lot of systems. To improve this problem, we have developed a personal computer-based simulator, Decs/1, which enables us to practice the making the dental cavities. The previous only allows cutting the first molar tooth from occlusal surface direction only. In order to extend the function, Decs/2 has been developed. The all tooth were modeled in solid model on the new system. And also, Decs/2 is capable of cutting any tooth from its side.

    Japanese, Scientific journal, There have been many studies on orthognathic surgery of class III patients with mandibular excess. In such patients, it is necessary to achieve the proper transverse relationship between the upper and lower jaws, as well as incisors, in the upright position, and alignment of both arches before surgery. However, it is difficult to diagnose discrepancies in transverse widths related to the inclinations of the upper and lower molars. The purpose of this study was to determine the proper transverse relationship by evaluating basal arch width discrepancies.<BR>Ten patients were diagnosed as skeletal class III characterized by mandibular excess. They underwent presurgical orthodontic treatment, with extraction of bilateral upper first premolars, to reduce the labial inclination of the upper incisors, and to obtain an adequate basal arch width relationship between the mandible and maxilla. Then they were treated by bilateral sagittal split ramus osteotomy and postsurgical orthodontic correction. Lateral cephalometric radiographs before and after orthognathic surgery (T1 and T2, respectively) were used for Wits appraisal. Dental models were also taken before and after orthognathic surgery. Measurements in dental models included 1) bucco-lingual inclinations of the upper and lower first molars to the occlusal plane, and 2) transverse widths of the upper and lower basal bones (basal arch width).<BR>Improved transverse relationships between the mandible and maxilla were achieved in all cases after treatment, although three of the ten cases showed crossbite on the first molars after orthognathic surgery. Inclinations of the upper or lower molars are correlated with discrepancy in the basal arch widths between upper and lower jaws. Discrepancy in the basal arch widths less than -0.5mm (minus means the lower jaw is wider than the upper jaw) has limited the position of the first molars in the proper bucco-lingual relationship.
  • Effects of mechanical stress on the expression of type XII collagen mRNA in human periodontal ligament cell

    English, Scientific journal, The aim of this study is to analyze the effect of mechanical stress on the formation and maintenance of the periodontal ligament(PDL). Type XII collagen was chosen as a characteristic marker of extracellular matrix, together with three other noncollagenous proteins, osteopontin, osteonectin and osteocalcin. Human PDL cells were cultured and subjected to intermittent tensile stress using a Flexercell Strain Unit^(1/20Hrz, 18% elongation for 1, 3, 5 or 7days). By the RT-PCR method with a LightCycler^ mRNAs of these proteins were quaentitatively analyzed. It was found that the mRNA expression of collagen XII, osteocalcin and osteonectin was increased by the mechanical stress in a time-dependent manner, wheres no significant change of mRNA of osteopontin did not observed. The results suggested that intermittent stress positively contributes to the formation and maturation of PDL cells to form the PDL.

    English, 特殊な指数関数の一種を,微妙な形状をコントロールする専用の関数として提案した.歯冠形状の表現法は,基本となる有理ベジエ曲面モデルに手を加えず,局所の微妙な形態の表現には,この関数により生成される形状を付加していく方法とした.結果として,この関数の指数部の値が,2.0〜2.6の間においては,有効な形状のコントロールが得られることが確認できた

    Japanese, Scientific journal, For the past 10 years, we have been engaged in research on development of CAD/CAM system for prosthetic restoration such as crown and bridge in molar teeth. In order to apply this system to anterior teeth, it is necessary to create the crown basic model data of the anterior teeth. The purpose of this study was to complete a library of basic crown model data of all teeth for the CAD system.<BR>The results were as follows:<BR>1. By dividing the incisal edge or the cuspid into a grid pattern, solid models of anterior teeth represented by bicubic Bezier surfaces were made from three-dimensional coordinate data of four-fold enlarged plaster models measured from five directions.<BR>2. In order to represent the coronal anatomical features of the anterior teeth, it was necessary to divide the four-fold enlarged models into more than 104 Bezier surfaces, and the real size models into more than 43 Bezier surfaces.<BR>3. The results suggested that consideration of the functional crown forms of the natural teeth may be necessary to decide the most suitable surface for basic crown models.

As a result, there is no model to represent a detailed tooth crown form like an occlusal surface. The purpose of this study was to create a tooth crown model showing minute coronal features. The results were as follows.<BR>1. By dividing the occlusal surface into a grid pattern, solid models of molar teeth represented by bicubic B&eacute;zier surfaces were made from three-dimensional coordinate data of four times enlarged plaster models measured from five directions.<BR>2. In order to represent the minute occlusal surface shapes of the molar teeth, it was necessary to divide the occlusal surfaces of the four times enlarged models into more than 1, 024 B&auml;zier surfaces, and the occlusal surfaces of the actual size models into more than 256 B&auml;zier surfaces.<BR>3. In the premolar teeth, it was necessary to divide the occlusal surfaces of the four times enlarged models into more than 256 B&eacute;zier surfaces, and the occlusal surfaces of the actual size models into more than 64 B&eacute;zier surfaces.<BR>4. Topological structure models may represent minute shapes efficiently with little data.
  • A Study of Computer Aided Design System for Tooth Crown Form

    Japanese, Scientific journal, The introduction of CAD/CAM technology was tried for the improvement of the quality of prosthetic restoration and of efficiency of the dental technician's work. In this study, CAD was examined as a method for prosthetic restoration, and as a result, the conclusions were as follows : 1. In order to design a tootla crown form by computer, it was necessary to freely treat a tooth crown form that has a complex three-dimensional freedom curved surface. 2. For this purpose,voxel model,spline model,and rational bezier model were examined. The data of the voxel model was easy to treat but difficult to transform effectively, and the problem there of was the quantity of data increased xvhen the surface was smoother. In case of the rational bezier model, it was possible to transform the model according to expectation. 3. As one method of transformation of the tooth crown form using rational bezier surface, it was advantageous to transform without chau?ging the control points by changing the weight-parameters. 4. The CAD system for prosthetic restoration was based on MCCM-solid-model using rational bezier surface, and it was found to be possible to make the prosthetic restoration in this CAD system like those made in dental technical work.

    初年度は,CADソフトウェアとしてAutodesk社のFusion360を導入し,脱脂・焼成の確認用サンプルとしての円盤状のものを,また後の試験を想定したJIS規格試験片の形状の試作サンプルなどの設計を行った.適合の精度を検証するクラウン形状のものは,円盤形状のもののテストが進んでから行うことにした.設計終了後は,当初の計画では,自力での3Dプリントは困難であると考え,順次,外注により産業用機器でプリント(成形),脱脂,焼結を行う想定だったが,手許に導入していた小型の光造形式3Dプリンタを利用して,材料の調整とプリント時の成形条件を様々にテストした結果,ジルコニア粉末と紫外線硬化レジンの配合比率が1:1程度で,プリント(成形)してグリーン体の製作をすることが可能になった.初年度は62個のプリントを完了したが,設計サイズと成形体のサイズにズレがあるため,加工条件と対比しながら状態の確認作業を続けている.また,成形物は予備実験として一部脱脂工程をテストしており,400℃,1時間程度で肉眼的には脱脂できているように見られることから,次年度にはその周囲の条件から確認を進めていく予定である.当初計画では,形状再現性と精度,歯冠形状のものの支台歯への適合性なども確認する予定だったが,これらについてはまだ実施できていない.成形体の製作は,初年度のべ100個の想定に対し現状は62個であったが,製作工程がある程度確立できたので,次年度は数百個程度の試作を行える見通しがついた.

  • Challenge to optical impression method of abutment-tooth under the gingiva - Application of near-infrared light diffusion simulation -

    We investigated the use of near infrared as a method to capture the part that can not be measured with the intraoral scanner. Instead of soft tissue, we wrapped raw ham around abutment teeth made by melamine resin. Light scattered inside the resin tooth was imaged with an infrared camera installed at a position orthogonal to the incident light axis. The margin shape of the abutment tooth was generated by analyzing the pixel value of the fluoroscopic images. But, the shape that can be generated had a error with respect to the depth direction of the image, which was considerably shifted from the original shape. In order to generate a more accurate shape, it is considered necessary to increase the amount of information, review the intensity of light, direction of irradiation and imaging direction, narrow down the search area in consideration of the shape of the preparation tools.

    The purpose of this study was to investigate the real-time adsorption behavior of biological molecules and osteogenic cells on a light-induced superamphiphilic titanium surface using a quartz crystal microbalance(QCM). We showed the potential usefulness of QCM in elucidating the sequential interaction of biological molecules and cells with the tissue-material interface at the molecular level. The results suggest that photofunctionalization of Ti boosts the initial adsorption rate and increases the adsorptive capacity.

  • Construction of 3D shape database of auricle ear for making prothesis

    The purpose of this study is the construction of three dimensional auricular shape database.
    (1) Extraction, collection, and analysis of auricular shape data
    It was possible to collect about 130 pieces of auricular shape data extracting from volume data of 3-D CT.
    (2) Construction of 3-D shape database
    Three dimensional shape database that was required to make auricular prosthesis was constructed. Auricle model in the database contained the internal structural data that was based on the voxel model.
    (3) Examination of materials and making of prosthesis for trial purposes
    Auricular prosthesis was made for patient using shape data base. We used the rapid prototyping technology to make the auricular model instead of wax up. In addition, with the proposal of the plastic surgeon, the template for the auricle reconstruction operation was made. As the materials, we examined the silicone and the polyurethane.
    (4) Examination of problem for practical use
    It was thought that the development of CAD software to practical use was necessary. It will need to be able to operate the shape of the data base easily. It will need to be able to synthesize the shape of auricle data to the patient's photograph of face. And it will need to be able to confirm the result of simulation easily.

  • A study on a development of a virtual simulator for dental education using the 3-D haptic modeling system

    This study tried to apply the haptic modeling system to a virtual simulator for dental education.
    In the dental clinical field, the sense of touch is considered most important for the mastery of various treatment techniques ; all dental school students are practiced in dentistry skills, such as tooth preparation, cavity filling, taking impressions, occlusal adjustments. Especially in prosthetic dentistry education, the fabrication of prostheses is regarded as a fundamental technique, and requires considerable class time for practical training.
    In virtual reality technologies, accommodating the sense of touch has been behind the practical use, and compares with the other senses such as vision and hearing. We focused on the haptic modeling system, the fundamental technology of which has been developed at the Massachusetts Institute of Technology Artificial Intelligence Lab. The haptic interface has a good quality of human-computer interaction by accommodating our sense of touch, so its use has spread not only in the field of the industrial design but also in the medical.
    First, we tried to simulate a process of waxing up utilizing a virtual reality system. The FreeForm modeling system enables easier control of the functional shape of the occlusal surface than former modification methods of handling control points of various free-form surfaces with the 2D mouse. Additionally, the haptic interfaces were extremely useful for resolving visual ambiguities ; the sense of touch leading to greater accuracy. Next, we tried to apply this system to the modification of an individual craniofacial bone model for surgical simulation. As a result, a haptic modeling system provided a significant benefit for removing metal artifacts, and reduced the amount of time required for modeling and modification.
    We affirmed that the use of this haptic modeling system provides significant benefit for dental education.

  • An Application Study of Rapid Protetyping Technology for Dental Science

    The purpose of this study is a making application of rapid prototyping technology far dental fields. We considered two applications, one is a process technology of dental CAD/CAM system and the other is a real model of patient based on 3D-CT data for surgical simulation.
    In the former, we tried to made a crown using ceramics powder. A powder mixed with resin by the various ratio was burned. But, it was difficult to control of shape with the volume shrinkage at burning. After all we could not have technical canviction and break through. On the other hand, molding of metal by titanium began to progress by the industrial world and a university. As for titanium, there is the possibility that can be applied in the living body directly. Then we continue to collect more information.
    In the latter, we suggested Voxel Formed STL format as the technique that data of 3D-CT were converted into STL data easily for the rapid prototyping. We interpolated by 3 dimensional Bi-Cubic method for all the volume of 3D-CT data, and it is possible for controlling deficit of shape in the details furthermore. As a result, when size of 1 Voxel did it in 0.25(W) ^*0.25(D) ^*0.50(H)mm, we could make a model for praetical surgical simulation. About soft tissue, process became possible only masseter muscle. We think it need to recognize and to extract organizations more easily in future. So, we try domain expansion method, and we will continue to examine this successively after this subject came to an end.

    1. Objective
    Orthognathic surgery has been widely performed to improve function and esthetics of patients suffering from jaw deformity. We have applied rapid prototyping (RP) technology to fabricate 3-dimensional cranio-facial bone models from 3D-CT data and the models have been used for presurgical simulation. However, post-operative relapse usually takes place due to tension from distorted soft tissues and muscles. Therefore, the purpose of this study was to fabricate simulation models including soft tissue in order to evaluate soft tissue change before and after surgery.
    2. Materials and Methods
    The masseter was modelled because masticatory muscles, especially the masseter, are supposed to have strong influence on postoperative relapse. Among patients underwent sagittal split or vertical ramus osteotomy, those who met the following criteria were included as subjects ; 1)no maxillary surgery or limited blow the zygomatic and nasal bones, 2)Preoperative CT taken after growth period was over and postoperative CT taken within 6 months after surgery are available. The masseter data was extracted from each CT slice. The long axis of the masseter was defined as a regression line of the center of gravity of the masseter on each slice. The change of the masseter muscle angluation was evaluated by the angle between the reference axes before and after the surgery.
    3.Results
    The long axis of the masseter muscles tended to tilt slightly to the posterior direction after surgery. However, if the technical errors regarding the scanning angle and the reference axes determination are taken into account, it can be concluded that the distortion of the masseter muscles, was minimal. The method used in this study would be inaccurate when the difference between the CT scanning angles taken pre and postoperatively was large. Therefore, in order to evaluate soft tissue changes more accurately, it may be necessary to strictly standardize pre and postoperative CT scanning conditions.

    (1)Development Hybrid Type of Periodontal Ligament
    The purpose of this study is to confirm the regeneration in the replanted tooth of Beagle dog by rhBMP-2 combined with ribbon (3 mm width) of fibrous collagen membrane (FCM). Twenty-four premolars of three beagle dogs were carefully extracted after thier root canals were treated. Before replantation their periodontal ligament and cementum were partially removed in a belt-type area (3 mm width) around the middle part of the root, and the denuded areas of root dentin were covered with FCM/rhBMP-2. It was concluded that combination of rhBMP-2 and FCM incduces periodontal ligament and cementum in the replanted tooth.
    (2)Diagnostic Imaging by Radiographs for Periodontal Membrane of Replanted and Transplanted tooth in Human
    We concluded that the normal space of periodontal membrane comfirmed 2-weeks fter the replantation and 4-weeks after the transplantation.
    (3)Periodontal regeneration in replanted tooth in rabit.
    We comfirmed the regeneration of priodontalligament 4-weeks after replantation was equivalent to the control tooth.

  • A MOLECULAR BIOLOGICAL INVESTIGATION ABOUT REGENERATION OF PERIODONTAL TISSUE

    Mechanical stress is expected to be an effective role for regeneration of periodontal tissue. The effect of mechanical stress for the activity of the periodontal ligament cell was investigated by biochemical approach. Mechanical stress was loaded by Flexercell. The cell number (the amount of DNA), the activity of alkaliphosphatase, the concentration of type I collagen and type XII collagen were selected as biochemical markers. RT-PCR was adopted in the case of alkaliphosphatase, type I collagen and type XII collagen.
    The periodontal ligament cell which was obtained by exfoliation from human extracted premolar tooth due to orthodontic treatment was cultivated in α-MEM cultivator. After third subculture, about one hundred thousand cells were disseminated per one well of Flexercell and stress was loaded to it. 20 seconds was decided as one loading cycle in which 10 seconds for on-stress, the other 10 seconds for off-stress. The expansion rate of cell was 18 %. The term of stress loading was 1, 3 and 5 days. After stress loading, stress loading groups and control groups were compared. The effect of stress loading term was also investigated. The results indicated that the periodontal ligament cell was activated and proliferated by relatively long term stress.

  • A STUDY OF CLINICAL APPLICATION OF DENTAL CAD/CAM SYSTEM

    The clinical application was carried out using CAD/CAM systems for crown, and the evaluation of the system was examined. In the measurement of antagonistic tooth, the antagonistic tooth was described by the exclusive black occlusion material. And there was the danger which had designed the crown that it slightly stands out from the model and is higher than the practice, when this was measured. It is considered that on this, it must be also examined that the direct antagonistic tooth model is measured. The data for the processing in which that CAD software becomes more and more practical, and that moreover, the design in the detail easily does it in the short time of 10-20 minutes would be possible was automaticaliy made in 15〜20 minutes. The processing time finished within 60 minutes almost. And, the problem might be also clinically very effective without recognizing especialiy, abutment tooth mounting condition. Using measured data of the abutment tooth, thickness of the crown was continuously measured in addition to each facing tilt angle of meslal-distal axial surface and buccal-lingual side axial surface as an application to the dentistry education in 1998. In the menu of the CAD software, after the formation there is a function which automatically checks the thickness of designed crown, and first the reference value of the thickness of the crown is input, and this function is applied this time abutment tooth data crown external form data reference value screen, and the data of melamine tooth In the before formed as a crown data is registered. Deletion quantity of thickness of the crown was measured by the comparison with the abutment tooth data. In the macroscope, by present evaluation method, It was judged as the result, that the decision became also automatically possible on the deletion shortage of small range with the difficult decision and that the over half of formed abutment tooth is insufficient in the deletion.

  • 顔面形態の非対称性に対する視覚的な認識に関する研究

    本研究の目的は、顎変形症疾患の顎を含めた顔面形態の変形を視覚的に認識するために、顔面形態の非対称性の有無について、判断基準を明らかにすることを目的とし、顔面規格撮影装置を用いて撮影した顔面正面写真上で顔面形態を構成する各部位の位置関係を検討した。
    被験者は、顔面形態が非対称と思われる15名の成人を正常者群とし、外科的矯正治療対象者の中で下顎対称と診断された13名を非対称群とした。
    計測箇所としは、顔面正中線は左右内眼角点の上唇点を結ぶ線とし、計測点は左右外眼角点・左右内眼角点・上唇点・鼻下点・口唇交点・下顎下縁部交点(外眼角点と鼻下点を結ぶ直線が下顎下縁部と交わる点)・頬部交点(鼻下点を通り顔面正中線と直交する直線が頬部外縁と交わる点)とした。
    結果:正常者群では、(1)左右外眼角点を結ぶ直線と左右頬部交点を結ぶ直線および外眼角点と下顎下縁部交点を結ぶ直線で囲まれる四角形はほぼ正方形なった。(2)片側の外眼角点と下顎下縁部交点および頬部交点で囲まれる三角形はほぼ二等辺三角となり左右同形となった。(3)左右下顎下縁部交点と上唇点で囲まれる三角形はほぼ二等辺三角形となった。(4)片側の外眼角点と口唇交点との距離は左右ほぼ同じでしかも被験者間でほぼ一定の値を示した。非対称群では、正常者群で示した一定の形は示さず下顎の変位した側へ四角形や三角形がなびく形態となった。
    しかし、非対称群の術後の検討数が少なく、また軽度の非対称の数が少ないため、非対称性の有無の判断基準はまだ得られていない。

  • Development of system measuring and analysing teeth crown form using light cutting method for dental CAD/CAM technology

    The purpose of this study is mesurment and analisys tooth crown form and dental arch using digital CCD camera and light cutting method. Moreover we considered measuring the shape of tooth crown form in the mouth directly. At first, we made a special stage with 6 degrees of freedom. Then we examined the methods of measurments using slit light, cross-pattern, and maluti-value pattern, et al. From the result, we decided to use multi slit pattern. This pattern was constituted alternately white line (2 pixels width) and black line (32 pixels width). The image provided using CCD camera was handled by the dynamic threshold value, and it was converted into black and white image, and 3 dimensional value was calculated. Then, we considerd the problems of crook or dim of image coming from focusing in optics system, and concluded that it is easy to measure rough shape, but not enough to do a minute measurement for abutment tooth. If the shape of tooth is mesured directly in the mouth using this method, it is important to make up this system into small size that is possible to insert in a mouth. We think it need the new technology break through the problems.

  • A STUDY OF FABRICATION OF CROWNS BY STEREO LITHOGRAPHY

    The aim of this study is to try fabrication of crowns by laser lithography or machine of computer aided manufacturing. First we studied about processing precision and established the most suitable processing (output power of laser : 13mVV,scan speed : 240mm/s, diameter of laser spot : The 0.2 mm, product layr pace of Z direction : 0.07 mm, scan method : Normal scan). As for data of crowns, we designed full crowns using Dental CAD/CAM system which NIKON co., GC co., HITACHI co.developed. And we input it into a work station for the control of light molding device after having ontput this as Bezie curved surface patch in the cause. And then, we did setting of processing condition, a simulation and made resin model. As the results the manufacturing process that made resin model of crown for abutment tooth in the mouth of real patient was established. A form of crown made reproduced a design of CAD faithfully, too. But what was careless is regarded as future examination subject till the resinous shrinkage rate used adapts it in abutment tooth greatly.

    The development of the living body substance model high speed framing device by the light molding methed that used CT film. It is that the purpose of this studies develops tha device which can make the living body model simply from CT film.
    At first, we digitized CT film and converted it into numerical data, and we got possible to make the living body model when seemed not to be got CT data directly from CT device. Then we compared a model made from CT data with this model. As a result, there are some problems by setting of precision and threshold value of image, but this model is very useful for simulation of simple operation.
    Next, we tried a method to copy 1 slice at a time furthermore.
    So, (1) We developed original software for image processing, and extracted shape of internal organs in the living body from CT film automatically, and established technology to generate data for transcription.
    (2) We changed image from CT film to monochrome, and copyed on film that ultraviolet rays transmit. So, we could make a model by exposured 1 slice at a time.
    In particular, about film for exposure mask, we ware able to confirm that we could utilize extremely cheap materials. But, about a quantity of light and the control of resinous depth to stiffen to transmit film, still many problems.

  • The relationship between electromyograph and energy metabolism in masseter mucles of TMD patients

    The purpose of this study was to verify the relationship between electromyograph (EMG) and energy metabolism in masseter mucles of TMD patients. The EMG data were compared with energy metabolism data analyzed by 31P-MRS (Magnetic Resonance Spectroscopy).
    The results were as follows ;
    1. Before and after fatigue-loading, the power spectral analysis of EMG showed the similar tendency to 31P-MRS analysis. It was suggested that the power spectral analysis of EMG might reveal the state of muscle fatigue and energy metabolism in masseter muscle.
    2. In the chewing rhythm analysis by EMG,the mean of CV (coefficiency of variation) in TMD patients suffering from pain and contracture of masseter muscles was higher than that in normal subjects. But there was little correlation between the result of CV analysis and that of 31P-MRS analysis.
    3. The mean of interval/cycle time ratio in TMD patients suffering from pain and contracture of masseter muscles was higher than that in normal subjects. The result of interval/cycle time analysis was correlative with the result of energy metabolism analysis by 31P-MRS.
    4. In one TMD patient, 31P-MRS analysis showed improvement of muscle fatigue after treatments, but interval/cycle time ratio was not changed evidently. In this patient, the ratio before treatment was almost in nomal range, because the border of burst was not obvious and the duration of burst was enlarged abnomally. After treatment the border of burst became clear and the burst shape became normal. This results suggest that the analysis of interval/cycle time ratio must be done together with the burst shape analysis.
