    The purpose of the present study was to determine an adequate stage for initiating tooth-brushing and optimal tooth-brushing force/duration following periodontal tissue regenerative therapy.
    Six beagle dogs were involved in the experiment First, the maxillary second premolar and mandibular third premolar were extracted. 4x4x4 mm of size, was formed between the maxillary first and third premolars and between the mandibular second and fourth premolars. Then, notch was marked on the root surface for recording the bottom of bone defect. In order to induce periodontitis, metal matrix was inserted in the area of bone defect The dogs were then kept for 8 weeks with being fed by soft food. After these treatments, the metal matrix were removed and periodontal tissue regenerative therapy was performed using Emdogain. Experimental teeth were divided into four groups according to the split-mouth design for each dog. The control tooth was given daily plaque removal using a hand instrument. The other three was given daily professional tooth-brushing following 1 day (1D), 3 days (3D), or 7 days (7D) after the periodontal regenerative therapy. The force and duration of tooth-brushing was set for 200g and 20 seconds, respectively. The stress gauze was used for controlling the tooth-brushing force.
    Histological observation found similar results with respect to gingival inflammation and remodeling of alveolar bone. No marked inflammation and down growth of sulcular epithelium was observed in all the groups. Newly-formed alveolar bone was found around the one-third of initial bone defect area. A little amount of newly formed cementum was observed around the notch. However, epithelial downgrowth was partially observed for 1D, 3D dogs.
    The results of this study indicate that tooth-brushing can be initiated next day after periodontal surgical treatment.

  • Effect of gingival massage by tooth-brushing in diabetic rats

    The purpose of the present study was to determine an optimal tooth-brushing force for gingival massage in streptozotocin (STZ)-induced diabetic rats with experimental periodontitis
    Forty-one male blister rats with STZ-induced diabetes mellitus (STZ-rat) were involved in the experiment. Under general anesthesia, lipopolysaccaride solution (25 μ g/ml) was applied once a day the palatal gingival sulcus of the maxillary first and second molars of STZ-rat to induce peridontitis for 8 weeks. From 5th to 8th week, the control group was given daily plaque removal using a hand instrument and the three experimental groups were given daily professional tooth-brushing with force of 100g (100G), 200g (200G), and 250g (250G) respectively The duration of tooth brushing was set for 20 seconds. The stress gauze was used for controlling the tooth brushing force. After 8 weeks, the STZ-rats sacrificed. The teeth and gingivae were fixed, decalcified and embedded in paraffin. Buccolingual sections (5μm) were used for histologically observation and histometrically evaluation.
    The blood glucose level of STZ-rats assessed by glucose debydrogenase method was about 450 mg/dl through experimental duration. Histological observation found similar results with respect to gingival inflammation and epithelial downgrowth. Strongly gingival inflammation and resorption of alveolar bone was partially observed for control, 200G, 250G rats. On the other hand, no marked epithelial downgrowth and severe resorption of alveolar bone was observed for 100g rats. In addition, histometrically evaluation indicated that a degree of epithelial downgrowth and resorption of alveolar bone was low in the 100G group compare with in the another groups.
    The results of this study indicate that tooth-brushing with force of 100g was available for gingival massage in diabetic status.

  • 唾液の新しい臨床検査法-環境工学を応用した口臭検査-

    口臭治療に携わる歯科医師の間では、唾液中の酸素が減少することで口腔内嫌気性細菌の発育・増殖を促し、悪臭を伴った口臭を発生させると考えられている。しかし、基礎的な根拠(evidence)が未だ明確でなく、解明が必要なテーマであった。
    そこで今回の研究では、唾液中の様々な因子から、溶存酸素(DO)と化学的酸素要求度(COD)の高低が口腔内の嫌気的環境度(汚染度)を示す指標ではないかと考え、これらの指標と口臭(揮発性硫化物の濃度を測定)との関係を明らかにすることを目的とした。
    平成16年〜18年度末までに、北海道大学病院歯科診療センターにおいて、本研究への参加に承諾の得られた被験者130名の口臭を簡易ガスクロマトグラフィーにより分析、硫化水素、メチルメルカプタン、ジメチルサルファイドの口臭構成物質の濃度を測定した。同時に被験者からガム法におる刺激時唾液を採取して、DOとCOD測定を行い、口臭構成物質の濃度との関連を調べた。
    (結果)1)各測定項目の平均値±標準偏差は、硫化水素276.6±419.3ppb、メチルメルカプタン51.1±105.8ppb、ジメチルサルファイド27.5±109.8ppb、溶存酸素8.12±1.20mg/L、化学的酸素要求度2.24±0.92g/Lであった。2)それぞれの口臭構成物質の濃度と溶存酸素、化学的酸素要求度の値には、統計学的関連は認められなかった。また、唾液中の溶存酸素と化学的酸素要求度の間にも統計学的には何の関連も認められなかった。
    (考察)当初の作業仮説とは大幅に異なり、口臭と刺激唾液中の酸素量や化学的酸素要求度の間には何ら関係がないことが明らかとなった。しかし、唾液中の溶存酸素および化学的酸素要求度の値は個人間のばらつきが少なく、一定範囲内であった。このことは唾液の特質を考える上で興味深い事実といえる。

  • Level of human lactoferrin in whole saliva, research for risk factors for periodontal disease

    The purpose of this study was to measure the levels of human lactoferrin in whole saliva among various age groups and examine the relationship between levels of lactoferrin and periodontal status. Whole saliva sample from donors (n=351) were collected by gum stimulation in ten minutes. The levels of lactoferrin were determined by the enzyme-linked immunosorbent assay (ELISA) using a monoclonal antibody. The results were as follows:
    1)The levels of lactoferrin in 60's and 70's year old group increased significantly than those under 30's year old groups. 2) Relationships between the levels of lactoferrin and periodontal status were unclear.

  • Establishment of New Evaluation for Periodontal Disease, Research for Risk Factors in Human Saliva

    The purpose of this study was to examine the quantities of histatin 5 and Cu/Zn super oxide dismutase (Cu/Zn SOD) in whole saliva sample from subjects divided into age groups ranging from 10 years to 70's (n=287) and to evaluate the individual risk for development of periodontal disease. The quantities of histatin 5 and Cu/Zn super oxide dismutase were determined hy using an enzyme-linked immunosorbent assay (ELISA) kit with a monoclonal antibody.
    The results were as follows : 1)the quantities (μg/mg of total salivary protein) of histatin 5 and Cu/Zn SOD in whole saliva showed large individual differences, 2)the mean quantities of histatin 5 in subjects aged 10-19 years was 5.3, and it showed a tendency to decrease with advance of the value being 2.5 in the 70's age group, whereas the mean quantity of Cu/Zn SOD showed no significant difference (p<0.05,ANOVA) between age groups, and 3) the mean values of the quantity of both histatin 5 and Cu/Zn SOD showed a significant difference (p<0.05,Mann-Whitney test) in healthy people aged over 40 years in subjects aged over 40 years who had periodontal disease.
    The results of this study suggest that it is possible to a new screening test kit that can be used at the chairside in a dental clinic for diagnosis ofperiodontal disease.
