  • The impact of social and solitary activities on loneliness in older rural male adults: A path analysis study.

    English, Scientific journal, PURPOSE: Loneliness is a serious concern among older rural men. This study examined how social and solitary activities are associated with each other and collectively relate to loneliness through path analysis. Two models were hypothesized: (1) solitary activities performance is associated with social activities, thereby relating to loneliness, and (2) social activity participation is associated with solitary activities, which in turn relates to loneliness. METHODS: Data were collected from 510 men aged 65 years and older residing in a rural Japanese town using self-administered questionnaires. The study measured loneliness (Japanese version of the UCLA Loneliness Scale), performance in social and solitary activities (Self-completed Occupational Performance Index), number of social activities (Vivid Social Activities Checklist), and depressive symptoms (Geriatric Depression Scale). Structural equation modeling was conducted; model fit was assessed using absolute, incremental, and parsimony indices. FINDINGS: Model 2 demonstrated acceptable fit (χ2 = 0.923, comparative fit index = 1.000, normed fit index = 1.000, root mean square error of approximation<0.001, Akaike information criterion = 38.009, Expected Cross-Validation Index = 0.074, and Hoelter = 208.044). Social activities were associated with solitary activities, which were in turn related to loneliness. Solitary activities were indirectly associated with loneliness through depression. CONCLUSIONS: This study demonstrates that higher performance in social activities may support healthier, more autonomous solitary engagement as a component of the social-solitary balance that shapes loneliness among older rural men. Locally grounded opportunities that enable meaningful social connection in addition to enhancing rural work rhythms and self-directed solitary routines may help reduce loneliness and support well-being in later life.
  • Orchestration of Skin Pathology in Cutaneous Lupus Erythematosus by HLA Class I Down-Regulated Senescent Epidermal Basal Cells.

    English, Scientific journal, OBJECTIVE: To investigate the role of senescent epidermal basal cells in cutaneous lupus erythematosus (CLE) pathogenesis using skin samples from patients with CLE and a mouse model of systemic lupus erythematosus (SLE). METHODS: Cellular senescence profiling utilized datasets from the NCBI Gene Expression Omnibus database and Accelerating Medicines Partnership® (AMP®) Phase 1-Metro. Gene array data from GSE184989 (CLE: n = 68, control: n = 4), single-cell RNA sequencing data from GSE186476 (CLE: n = 7, control: n = 14), and AMP® Phase 1-Metro (SLE: n = 17) were utilized. In vitro experiments further examined the expression of p21WAF1/CIP1, type I interferon (IFN), human leukocyte antigen class I (HLA-I), and epidermal growth factor receptor (EGFR) signalling. Pharmacological clearance of senescent cells was performed using the senolytic drug fisetin in the SLE mouse model. RESULTS: p21WAF1/CIP1-high senescent epidermal basal cells in patients with CLE exhibit high type I IFN expression. These cells enhance IFN signalling in surrounding normal epidermal basal cells, leading to increased HLA-I expression. In contrast, senescent epidermal cells upregulate EGFR signalling, which downregulates HLA-I expression, allowing them to evade immune surveillance. This heterogeneity of HLA-I expression promotes CD8-positive T-mediated toxicity against normal epidermal basal cells, resulting in their apoptosis. Pharmacological clearance of senescent epidermal basal cells improved SLE-like skin lesions. CONCLUSION: Senescent cells create a microenvironment that directs cytotoxic T-cell-mediated responses against normal epidermis in CLE, contributing to disease pathology. Targeting senescent cells and their signalling pathways may offer novel therapeutic strategies for CLE and SLE skin lesions.
  • Loneliness and social participation among older Japanese adults: The influence of gender on social participation.

    English, Scientific journal, Previous studies have demonstrated that social participation can alleviate loneliness; however, the specific types and characteristics of social participation that contribute to loneliness prevention remain unclear, particularly considering gender differences. In this study, we examined the gender-related influence of the types and characteristics of social participation on loneliness among older adults aged ≥65 years. A total of 381 participants (175 men and 206 women) from a regional city in Hokkaido, Japan, completed questionnaires assessing loneliness, social participation, life-space mobility, self-evaluation of abilities, and depression. Hierarchical multiple regression analysis revealed that loneliness among men was significantly associated with participation in neighborhood association activities, group exercise, and depression, suggesting that maintaining social roles and responsibilities through these activities is essential for reducing loneliness. In contrast, for women, no specific type of social participation had a significant effect on loneliness. However, correlation analysis indicated that women's loneliness was related to a broader range of social participation compared to that of men. These findings suggest that, for women, the extent of participation across various types of social participation is more important than engaging in specific types. Providing accessible opportunities for diverse forms of participation may be particularly effective in reducing loneliness among women. This study highlights the importance of developing gender-specific interventions to prevent loneliness. Tailored support strategies that consider the differing social roles, patterns of participation, and needs of men and women are necessary to effectively address loneliness among older adults.
  • Characteristics of Older Adults Experiencing High Loneliness Despite Active Social Participation: A Cross-Sectional Study.

Some older adults experience high loneliness despite active social participation. We aimed to clarify the characteristics of these individuals by focusing on their preference for solitude and satisfaction with and performance in valuable social and solitary activities. This cross-sectional survey included 3205 individuals aged ≥65 years and assessed loneliness, participation in social activities, preference for solitude, performance of valuable social and solitary activities, and depression. Participants were categorized into 3 groups based on the relationship between loneliness and social activity participation: higher social activity with a tendency toward loneliness, lower social activity without a tendency toward loneliness, and a correlated group. A one-way analysis of variance was conducted to clarify each group's characteristics. Structural equation modeling was used to explore the structure underlying the discrepancy between loneliness and social activity. Older adults experiencing high loneliness despite active social engagement preferred solitude, had low performance of valuable social and solitary activities, and reported high depression. In the high loneliness despite social activity group, a higher preference for solitude was associated with a decreased performance of social activities valuable to the individual, leading to reduced performance of solitary activities valuable to the individual. These effects contributed to increased depression, further exacerbating the discrepancy between loneliness and social activity. Some older adults may experience little to no benefit from increasing social activities. Social activities that consider older adults' solitude values could be more effective in addressing loneliness.
  • Senescence-associated secretory phenotypes in mesenchymal cells contribute to cytotoxic immune response in oral lichen planus.

    English, Scientific journal, Oral lichen planus is a chronic inflammatory condition that adversely affects the oral mucosa; however, its etiology remains elusive. Consequently, therapeutic interventions for oral lichen planus are limited to symptomatic management. This study provides evidence of the accumulation of senescent mesenchymal cells, CD8 + T cells, and natural killer cells in patients with oral lichen planus. We profiled the patients' tissues using the National Center for Biotechnology Information Gene Expression Omnibus database and found that senescence-related genes were upregulated in these tissues by gene set enrichment analysis. Immunohistochemical analysis showed increased senescent mesenchymal cells in the subepithelial layer of patients with oral lichen planus. Single-cell RNA-seq data retrieved from the Gene Expression Omnibus database of patients with oral lichen planus revealed that mesenchymal cells were marked by the upregulation of senescence-related genes. Cell-cell communication analysis using CellChat showed that senescent mesenchymal cells significantly influenced CD8 + T cells and natural killer cells via CXCL12-CXCR4 signaling, which is known to activate and recruit CD8 + T cells and NK cells. Finally, in vitro assays demonstrated that the secretion of senescence-associated factors from mesenchymal cells stimulated the activation of T cells and natural killer cells and promoted epithelial cell senescence and cytotoxicity. These findings suggest that the accumulation of mesenchymal cells with senescence-associated secretory phenotype may be a key driver of oral lichen planus pathogenesis.

    Up to 60% of patients with systemic lupus erythematosus (SLE) experience autonomic symptom. Sympathetic nervous system damage can cause dysfunction of the bone marrow that activates inflammatory cells, potentially causing multiple organ damage. We hypothesized that sympathetic nervous system damage would induce bone marrow dysfunction with multiple organ damage in SLE, and that multiple organ damage could be improved by therapy targeting the nervous system. Here, we showed that damage to autonomic nerves and Schwann cells occurred in the bone marrow and central nervous system of SLE model mice. A neurotoxic drug increased mortality and induced severe neuropathy and multiple organ damage, while a neuroprotective drug prevented multiple organ damage. The administration of bone marrow-derived mesenchymal stromal cells (BMSCs) cultured on a 3-dimensional fiber scaffold improved bone marrow neuropathy, skin lesions, kidney function, and mortality. Our results reveal that bone marrow neuropathy influence multiple organ damage associated with SLE, and improvement of bone marrow neuropathy by intrathecal injection of BMSC may be a target for SLE multiple-organ damage.
  • Altered regulation of mesenchymal cell senescence in adipose tissue promotes pathological changes associated with diabetic wound healing.

    Pathologic diabetic wound healing is caused by sequential and progressive deterioration of hemostasis, inflammation, proliferation, and resolution/remodeling. Cellular senescence promotes wound healing; however, diabetic wounds exhibit low levels of senescent factors and accumulate senescent cells, which impair the healing process. Here we show that the number of p15INK4B + PDGFRα + senescent mesenchymal cells in adipose tissue increases transiently during early phases of wound healing in both non-diabetic mice and humans. Transplantation of adipose tissue from diabetic mice into non-diabetic mice results in impaired wound healing and an altered cellular senescence–associated secretory phenotype (SASP), suggesting that insufficient induction of adipose tissue senescence after injury is a pathological mechanism of diabetic wound healing. These results provide insight into how regulation of senescence in adipose tissue contributes to wound healing and could constitute a basis for developing therapeutic treatment for wound healing impairment in diabetes.

    English, Scientific journal, Systemic lupus erythematosus (SLE) is a chronic autoimmune inflammatory disease characterized by the involvement of multiple organs. Lupus nephritis (LN) is a major risk factor for overall morbidity and mortality in SLE patients. Hence, designing effective drugs is pivotal for treating individuals with LN. Fisetin plays a senolytic role by specifically eliminating senescent cells, inhibiting cell proliferation, and exerting anti-inflammatory, anti-oxidant, and anti-tumorigenic effects. However, limited research has been conducted on the utility and therapeutic mechanisms of fisetin in chronic inflammation. Similarly, whether the effects of fisetin depend on cell type remains unclear. In this study, we found that LN-prone MRL/lpr mice demonstrated accumulation of Ki-67-positive myofibroblasts and p15INK4B-positive senescent tubular epithelial cells (TECs) that highly expressed transforming growth factor β (TGF-β). TGF-β stimulation induced senescence of NRK-52E renal TECs and proliferation of NRK-49F renal fibroblasts, suggesting that TGF-β promotes senescence and proliferation in a cell type-dependent manner, which is inhibited by fisetin treatment in vitro. Furthermore, fisetin treatment in vivo reduced the number of senescent TECs and myofibroblasts, which attenuated kidney fibrosis, reduced senescence-associated secretory phenotype (SASP) expression, and increased TEC proliferation. These data suggest that the effects of fisetin vary depending on the cell type and may have therapeutic effects in complex and diverse LN pathologies.
  • Accumulation of Senescent Neural Cells in Murine Lupus With Depression-Like Behavior.

    English, Scientific journal, Neuropsychiatric manifestations targeting the central, peripheral, and autonomic nervous system are common in systemic lupus erythematosus (SLE); collectively, these symptoms are termed neuropsychiatric SLE (NPSLE). Among a wide variety of neuropsychiatric symptoms, depression is observed in about 24-39% of SLE patients. Several cytokines and chemokines have been identified as biomarkers or therapeutic targets of NPSLE; in particular, the levels of type 1 interferons, TNFs, and IL-6 are elevated in SLE patient's cerebrospinal fluid (CSF), and these factors contribute to the pathology of depression. Here, we show that senescent neural cells accumulate in the hippocampal cornu ammonis 3 (CA3) region in MRL/lpr SLE model mice with depressive behavior. Furthermore, oral administration of fisetin, a senolytic drug, reduced the number of senescent neural cells and reduced depressive behavior in the MRL/lpr mice. In addition, transcription of several senescence and senescence-associated secretory phenotype (SASP) factors in the hippocampal region also decreased after fisetin treatment in the MRL/lpr mice. These results indicate that the accumulation of senescent neural cells in the hippocampus plays a role in NPSLE pathogenesis, and therapies targeting senescent cells may represent a candidate approach to treat NPSLE.

    Japanese, Research institution, The purpose of this study is to examine whether or not an intervention using Frontal / Executive Program (FEP) is effective to improve cognitive functions of people with MCI. Subject was a woman in her 80s who was living alone at the time of the intervention. With regard to her cognitive functions, she was aware of problems with memory, such as being not good at remembering. Total of 22 sessions of FEP were performed; two sessions a week, one on one, each session lasting about an hour. She was assessed to have MCI using MoCA-J. To obtain an objective evaluation before and after the intervention, the Brief Assessment of Cognition in Schizophrenia- Japanese version (BACS-J), Wisconsin Card Sorting Test (WCST) and Continuous Performance Test (CPT) were performed. As a result, the subject's verbal fluency and digit sequencing performances before the intervention were notably low in BACS-J, but after the intervention, composite score, verbal memory and the Tower of London showed significant improvement. It was shown that FEP improves the results of cognitive function tests, such as reduction of errors in WCST and CPT, suggesting the applicability of FEP to people with MCI. Compared with other mental illnesses, the result of this study was characteristic because the subject's verbal fluency did not show enough improvement. In the future, it will be necessary to conduct further studies with more number of cases and to examine the specificity of MCI's cognitive functions.Article

    Japanese, Scientific journal, The purpose of this study was to investigate factors associated with the psychological impact of the Great East Japan Earthquake on high school students 1 year and 4 months after the disaster, and clarify support needs of the students. In the outreach program for students of three high schools in coastal areas of southern Miyagi Prefecture, Japan, 1,973 students were surveyed after obtaining informed consent for participation. Questionnaires included: the Quick Inventory of Depressive Symptomatology (QIDS-J), Self-rating Anxiety Scale (SAS), Impact of Event Scale-revised (IES-R), and Connor-Davidson Resilience Scale (CD-RISC10). All scores were compared using SPSS 20.0 J between school grades, locations of the schools, and extent of damage due to the Great East Japan Earthquake. Our analysis showed a significant positive correlation between school grades and the level of anxiety. PTSR scores, but not anxiety nor depressive scores, of students whose lives have suffered extensive damage were significantly higher than those of students who have not. Students of high schools which have suffered extensive damage and use temporary buildings showed significantly higher levels of depression and anxiety, and significantly lower resilience, compared to students of high schools which were not damaged. Although previous findings demonstrated that younger children have a higher risk of being influenced by disasters, symptoms related to PTSR and depression were found frequently in the high school students as well. Among the high school students, our analysis showed a positive correlation between the level of anxiety and school grades, probably because the disaster has affected an influential and pivotal point in their lives.

    急性期精神疾患の治療の多くは、通常、患者の精神症状の緩和や機能障害の軽減を目的とした臨床的な回復に重点が置かれている。しかし、臨床的な回復と個人的な回復の両方が、将来の計画や治療動機に影響するため、個人的な回復の重要性が示唆されている。しかし急性期精神疾患患者に対する個人的回復を促す介入の効果については、これまで明らかにされていない。さらに自閉スペクトラム症(ASD)は併存疾患や二次障害の罹患率が高く、治療効果が低く難治性に至る症例も存在する。ASDでは自己効力感の低さが二次障害を併発される原因でもあり、パーソナルリカバリーへの介入は必須である。そこで本研究では、急性期精神疾患入院患者のうち、特に自閉スペクトラム症の診断を受けている、または疑いのある人を対象に、疾病管理・回復プログラムおよび認知機能改善療法の効果を明らかにすることを目的とした。その目的を明らかにするために、前向きな前後介入研究を実施し、対象者は標準的な治療に加えて、疾病管理・回復プログラムと前頭葉・実行機能の認知機能改善療法を受けた。その結果、疾病管理・回復プログラムと認知機能改善療法の複合的支援の効果として、現時点では複数の機能評価尺度において改善が認められている.現在はまだサンプルの収集途中であり,今後は複合的治療効果について臨床的リカバリーおよびパーソナルリカバリーの視点で分析し,その効果を明らかにしていく予定である.
