English, Scientific journal, Thyroid hormones are essential for fetal and early childhood development. Manganese (Mn) and selenium (Se) are involved in thyroid hormone homeostasis; however, the effects of prenatal Mn and Se exposure on thyroid function in offspring remain unclear. We analyzed 1,773 mother-child pairs from the Japan Environment and Children's Study, measuring Mn and Se in maternal whole blood during the second or third trimester and in cord blood at delivery. Neonatal thyroid-stimulating hormone (TSH) was measured on postnatal days 4-6, whereas plasma TSH and free thyroxine (FT4) were measured at ages 2 and 4 years. Log2-transformed exposure and outcome values were analyzed using multivariable linear regression, quartile models, restricted cubic spline analyses, and repeated-measures generalized estimating equation models. Cord-blood Mn was inversely associated with neonatal TSH, with adjusted β values (95% confidence interval) of -0.226 (-0.364 to -0.089) overall, -0.411 (-0.608 to -0.214) in males, and -0.031 (-0.225 to 0.163) in females. Quartile analyses supported this inverse pattern, particularly in males: the adjusted β values for the highest versus lowest quartil of cord-blood Mn were -0.210 (-0.361 to -0.059) overall and -0.436 (-0.658 to -0.214) in males. Repeated-measures, quartile, and nonlinear analyses partially supported an inverse or non-monotonic cord-blood Mn-TSH pattern across follow-up ages, although the findings at ages 2 and 4 years were weaker and less consistent. Maternal-blood Mn showed similar but weaker inverse associations with TSH, whereas maternal- and cord-blood Se showed no consistent associations with TSH or FT4. Higher prenatal Mn exposure, particularly cord-blood Mn, was associated with lower neonatal TSH levels, especially in males. English, Scientific journal, BACKGROUND: The lower-extremity muscles are the main target of rehabilitation post-anterior cruciate ligament reconstruction (ACLR). This study aimed to investigate time-course changes in the cross-sectional area of lower-extremity muscles (MCSA) amongACLR patients. METHODS: Twenty-seven male patients who underwent ACLRwere included and groupedintoPASS and non-PASS. PASS could achieve a ratio of injured-to-uninjured performance surpassing 90 % in all hop-tests and Lysholm score of ≥ 90. Those who failed to achieve even one of those conditions were designated as non-PASS. About MCSA, magnetic resonance images of the lower extremity on the injured side were obtained. Quadriceps, hamstrings, gastrocnemius, and soleus were manually surroundedatthe slice featuredthe maximal MCSAfor each muscle. Range of motion (ROM) and muscle strength of the knee and anklewere measured. All parameters were measured before surgery (PRE) and at 3, 6, 9, and 12 months post-ACLR(POST3/POST6/POST9/POST12, respectively). Knee stability was measured using KT-1000 in patients who underwent second-look surgery. The significance level was set at 5 %. RESULTS: At POST12, PASS (n = 14) exhibited greater ankle dorsiflexion ROM, knee extension strength, and ankle plantarflexion strength than non-PASS (n = 13). The interaction of MCSA was not detected, thus both data were integrated into a unified group for post-hoc tests.Quadriceps MCSA at POST12 was significantly larger than that at PRE. Hamstring, gastrocnemius, and soleusMCSAsat POST12did not have significant differences than those at PRE. CONCLUSIONS: Restoration of ankle function and strength may be important for returning to sports. - Vastus medialis onset activation in patients who underwent medial patellofemoral ligament reconstruction with and without tibial tubercular osteotomy.

English, Scientific journal, BACKGROUND: Medial patellofemoral ligament reconstruction (MPFLR) and tibial tubercular osteotomy (TTO) are common surgical treatments for patellar dislocation. However, the impact of isolated MPFLR and MPFLR combined with TTO (MPFLR+TTO) on vastus medialis oblique (VMO) onset activation remains unclear. This study investigated the VMO onset activation relative to the vastus lateralis (VL) among patients undergoing isolated MPFLR, MPFLR+TTO, and healthy controls during maximum voluntary isometric knee extension. METHODS: This study included 25 patients who underwent MPFLR, divided into two groups: isolated MPFLR (n = 12, age: 22.6 ± 11.9, follow-up time: 24.0 ± 16.8 months) and MPFLR+TTO (n = 15, age: 28.2 ± 12.8, follow-up time: 25.2 ± 15.6 months). A control group of 11 healthy adults (22 knees, age: 28.4 ± 1.3) was also included. Electromyographic (EMG) activities of the VMO and VL muscles were recorded during maximal voluntary isometric knee extensions at 30°, 60°, and 90° of flexion. Differences in VMO onset activation relative to VL (VMO-VL) were analyzed. RESULTS: At 30° knee flexion, the MPFLR group showed a significant delay in VMO activation compared to healthy controls (p < 0.001). In contrast, the MPFLR+TTO group exhibited no significant differences in VMO activation relative to healthy controls. No significant differences were observed at 60° and 90° flexion among all groups. CONCLUSION: Isolated MPFLR delayed VMO activation relative to VL. In contrast, MPFLR+TTO balanced the onset activation between VMO and VL. These findings highlight the need for targeted rehabilitation programs to address delayed VMO activation in patients undergoing isolated MPFLR. - Acute Effects of Dynamic and Ballistic Stretching on Flexibility: A Systematic Review and Meta-analysis.

English, Scientific journal, Dynamic stretching (DS) uses a controlled movement through the range of motion (ROM) of the active joint(s) by contracting the agonist muscles without being held in an end position. In contrast, ballistic stretching (BS) typically uses uncontrolled movements of a higher velocity with bouncing actions. However, BS is often considered to be a form of DS. When considered together, DS and BS reportedly increase flexibility, evidenced by single- and multiple-joint ROM improvements and other measurements. However, a meta-analysis with subgroup analyses revealing the acute effects of DS and BS on flexibility in detail, independently of other stretching methods, has yet to be conducted. The purpose of this meta-analysis was to investigate the acute effects of DS and BS on flexibility in healthy participants. The PubMed, Web of Science, and Scopus databases were searched for eligible papers published before September 9, 2024; 17 papers were included in the meta-analysis. The main meta-analysis was performed with a random-effect model, and subgroup analyses were performed to examine the effects of age (young vs. middle-aged and older), sex (male vs. mixed sex), stretching methods (DS vs. BS), stretched muscles (hamstrings vs. plantar flexors vs. multiple muscles), and flexibility outcomes (single-joint ROM vs. straight-leg raise test vs. sit-and-reach test). A small increase in flexibility was found following DS and BS (considered together) (effect size = 0.372, Z = 3.936, 95% confidence interval = 0.187-0.557, p < 0.001, I2 = 27%). Subgroup analyses revealed no significant differences between age (p = 0.24), sex (p = 0.76), stretching method (p = 0.83), stretched muscle (p = 0.20), or flexibility outcome (p = 0.34) groups. Our results suggest that DS and BS effectively provide acute, small-magnitude improvements in flexibility that are not significantly affected by individual characteristics, stretching methods, stretched muscles, or flexibility outcomes. - Acute effects of static stretching on postural control during a maximum forward-leaning task in young and older adults

English, Scientific journal, OBJECTIVES: This study investigated the acute effects of dynamic stretch (DS) duration on the muscle stiffness of the ankle plantar flexor using shear wave ultrasound elastography. METHODS: Eighteen healthy young participants were enrolled in this study. DS with one set (DS1) or four sets (DS4) of 30 s each was performed randomly. Shear wave velocity in the medial gastrocnemius (MG) was measured before and after DS to assess muscle stiffness of the MG. RESULTS: Two-way repeated-measures analysis of variance (condition × time) showed a significant interaction with the shear wave velocity (p = 0.02). Shear wave velocity significantly decreased after the DS4 than before (before:3.09 ± 0.59 m/s; after: 2.86 ± 0.43 m/s). However, no significant differences were observed in shear wave velocity between before and after DS1 (before: 2.96 ± 0.56 m/s; after: 3.19 ± 0.56 m/s). There were no significant differences in shear wave velocity at baseline condition. After the intervention, significantly lower shear wave velocity was observed in DS4 than in DS1. CONCLUSIONS: The results of this study demonstrate that DS with four sets of 30 s effectively decreased the muscle stiffness of the MG. - Effects of short-duration constant torque and constant angle stretching on the musculotendinous stiffness of ankle plantar flexors

English, Scientific journal, OBJECTIVES: This study aimed to investigate the impact of shoulder dysfunction on concomitant neck disability in patients with shoulder disorders. METHODS: The participants were patients with subacromial impingement syndrome (SIS) and frozen shoulder (FS). Twenty patients with SIS and 21 with FS without cervical radiculopathy were enrolled. The participants were assessed for the 4-week prevalence of neck pain, Neck Disability Index (NDI), shoulder strength and range of motion, a short version of the Disabilities of the Arm, Shoulder, and Hand questionnaire (quick-DASH), and the Pain Catastrophizing Scale (PCS). RESULTS: The 4-week prevalence of neck pain was 12 out of 20 (60%) in patients with SIS and 13 out of 21 (62%) in patients with FS. The median NDIs were 13 and 12 for SIS and FS, respectively, with no statistically significant difference. About 41% (17/41) of the participants displayed an NDI greater than the cutoff value for disability in daily living. Although shoulder abduction strength correlated with the NDI in patients with SIS, the PCS score correlated with the NDI in patients with FS. CONCLUSIONS: Concomitant neck disability is a critical concern for patients with shoulder disorders. The clinical factors related to concomitant neck disability differ between SIS and FS, with specific interventions recommended for each condition. - Acute effects of static and dynamic stretching for ankle plantar flexors on postural control during the single-leg standing task.

English, Scientific journal, Static stretching (SS) and dynamic stretching (DS) are widely used as warm-ups before sports. However, whether stretching affects postural control remains unclear. We compared the effects of SS and DS on the plantar flexors and postural control during single-leg standing. Fifteen healthy young participants performed SS, DS, or no stretching (control). The stretch condition consisted of four sets lasting 30 s each. The control condition was a rest with standing for 210 s. Center of pressure (COP) displacement was measured using a force plate before and after each intervention to assess postural control during the single-leg standing task. The COP area, COP velocity, and anteroposterior (COPAP) and mediolateral (COPML) range were calculated. DS significantly decreased in the COPML range (21.5 ± 4.1 to 19.0 ± 2.5 mm; P = 0.02), COP velocity (33.8 ± 7.6 to 29.8 ± 6.5 mm/s; P < 0.01), and COP area (498.6 ± 148.3 to 393.3 ± 101.1 mm2; P < 0.01), whereas SS did not change in the COP parameters (COP area 457.2 ± 108.3 to 477.8 ± 106.1 mm2, P = .49; COP velocity 31.2 ± 4.2 to 30.7 ± 5.8 mm/s, P = 0.60; COPAP 25.4 ± 3.1 to 25.3 ± 3.2 mm, P = 0.02; COPML 20.7 ± 3.3 to 21.1 ± 2.5 mm, P = 0.94). Therefore, DS of the plantar flexors enhances postural control during single-leg standing and may be effective for both injury prevention and performance enhancement. - Sex-related differences in stretch-induced musculotendinous stiffness changes

English, Scientific journal, BACKGROUND: Baseball pitching induces a large elbow valgus load, stressing the ulnar collateral ligament. Flexor-pronator mass contraction contributes to valgus stability; however, repetitive baseball pitching may weaken the flexor-pronator mass contractile function. The present study investigated the effects of repetitive baseball pitching on the medial valgus stability measured using ultrasonography. We hypothesized that repetitive pitching would decrease elbow valgus stability. METHODS: This was a controlled laboratory study. Fifteen young male baseball players at the collegiate level (age: 23.0 ± 1.4 years) were enrolled. The medial elbow joint space was measured using ultrasonography (B-mode, 12-MHz linear array transducer) in the following three conditions: at rest (unloaded), under 3 kg valgus load (loaded), and under valgus load with maximal grip contraction to activate flexor-pronator mass (loaded-contracted). All measurements were performed before and after the pitching tasks, which comprised five sets of 20 pitches. Two-way repeated-measures analysis of variance was applied to determine changes in the medial elbow joint space. The post-hoc test with Bonferroni adjustment was applied to assess the changes within the time and condition. RESULTS: The medial elbow joint space was significantly greater under the loaded than the unloaded and loaded-contracted conditions both before- and after- pitching (p < 0.001). In the loaded-contracted condition, the medial elbow joint space significantly increased after repetitive baseball pitching (p < 0.001). CONCLUSIONS: The results of the present study indicated that repetitive baseball pitching reduced the elbow valgus stability. This reduction could be attributed to the decreased flexor-pronator mass contractile function. Insufficient contraction may increase the tensile load on the ulnar collateral ligament with pitching. Flexor-pronator mass contraction plays a role in narrowing the medial elbow joint space; however, repetitive baseball pitching reduced the elbow valgus stability. It has been suggested that sufficient rest and recovery of the flexor-pronator mass function are required to reduce the ulnar collateral ligament injury risk. - Characteristics of isometric knee extensor strength in patients with medial patellofemoral ligament reconstruction using artificial ligaments

Japanese, Objective: The purpose of this study was to clarify the difference in knee extensor muscle strength in patients with medial patellofemoral ligament reconstruction (MPFLR) using artificial ligaments and MPFLR with concomitant tibial tubercle osteotomy (MPFLR+TTO).
Methods: Fifteen patients in the MPFLR group (follow-up time: 22.8±15.6 months) and 11 patients in the MPFLR+TTO group (follow-up time: 22.8±9.6 months) participated in the present study. The isometric knee extensor strength at 90° and 30° of knee flexion were measured for all participants. The differences and ratios in knee extensor strength between the operated and non-operated knees were determined.
Results: In the MPFLR group, there were no significant differences in isometric knee extensor strength between operated and non-operated knees. In the MPFLR+TTO group, the operated knee extensor strength was significantly lower than the non-operated knee. The isometric knee extensor strength ratio of the operated to the non-operated knee was significantly lower at 30° of knee flexion in the MPFLR+TTO group than in the MPFLR group.
Conclusion: Isolated MPFLR using artificial ligaments provided good recovery of knee extensor muscle strength. On the other hand, it was suggested that MPFLR with concomitant TTO remained the differences in knee extensor strength between operated and non-operated knees and postoperative recovery of knee extensor muscle strength was smaller than isolated MPFLR. - Torque-angle curve of the knee flexors in athletes with a prior history of hamstring strain.

English, Scientific journal, OBJECTIVES: To investigate the knee flexor torque-angle curve after hamstring strain injury using different muscle action types and angular velocities. DESIGN: Cross-sectional. SETTING: Controlled laboratory. PARTICIPANTS: Thirteen collegiate athletes injured hamstring strain (21.0 ± 0.8 years; 173.9 ± 6.5 cm; 70.1 ± 10.5 kg). MAIN OUTCOME MEASURES: Concentric and eccentric knee flexor torque was measured at 60 & 300°/sec. Peak torque and average torque every 10° were determined from torque-angle curve and injured side was compared with non-injured side. RESULTS: No significant differences were found in the concentric muscle actions. However, the eccentric peak torque was significantly lower on the injured side at 60°/sec (p = 0.048) and at 300°/sec (p = 0.002). The average eccentric torque was significantly lower on the injured side at 60°/sec from 10° to 20° of knee flexion (p = 0.012-0.018) and at 300°/sec from 10° to 60° of knee flexion (p = 0.005-0.049). CONCLUSION: The knee flexor torque-angle curve changes with eccentric muscle action after hamstring injury. Eccentric torque declines were close to full knee extension at 60°/sec and a wide range of knee flexion at 300°/sec. The assessment and rehabilitation of eccentric hamstring strength may be important to consider the effect of the angular velocity after hamstring strain injury. - The deficits of isometric knee flexor strength in lengthened hamstring position after hamstring strain injury.

English, Scientific journal, OBJECTIVE: To investigate the effects of knee flexion angle on peak torque, rate of torque development (RTD) during isometric contraction and hamstring flexibility after hamstring strain injury (HSI). DESIGN: Cross-sectional. SETTING: Controlled laboratory research. PARTICIPANTS: Fourteen male athletes with a history of HSI and 14 athletes without HSI (controls). MAIN OUTCOME MEASURES: Hamstring flexibility was evaluated using active knee extension test. Isometric knee flexion peak torque and RTD were determined at 30°, 60°, and 90° of knee flexion measured by an isokinetic dynamometer. RESULTS: Individuals with a history of HSI had statistically significant, moderate deficits in isometric peak torque at 30° of knee flexion (P = 0.037; effect size = 0.55) in the HSI limb than in the uninjured limb, but not at 60° and 90° of knee flexion. In the control group, no significant differences in isometric peak torque at any angle were found between limbs. No differences in peak RTD and flexibility were found between limbs in both groups. CONCLUSIONS: Isometric peak torque at 30° of knee flexion was lower in the injured limb than in the uninjured limb. Isometric strength deficits after HSI tended to be affected by lengthened hamstring angles. - Effects of Intermittent and Continuous Static Stretching on Range of Motion and Musculotendinous Viscoelastic Properties Based on a Duration-Matched Protocol.

This study aimed to compare the effects of intermittent and continuous stretching for the same duration on the range of motion (ROM), passive resistive torque (PRT), and musculotendinous stiffness (MTS) of ankle plantar flexors. Eighteen healthy young men participated in the study. Intermittent (four sets × 30 s) and continuous stretching (one set × 120 s) were performed in random orders on two separate days. Both stretching protocols were conducted using a dynamometer with a constant torque applied. ROM and PRT were determined using a dynamometer, and MTS was calculated using the torque-angle relationship measured before and after stretching. Two-way repeated measures analysis of variance was performed for all parameters. Both intermittent and continuous stretching significantly increased ROM and decreased PRT and MTS (p < 0.05). Intermittent stretching led to greater changes in ROM and PRT than continuous stretching. However, the reduction in MTS did not differ between the two conditions. These results suggest that intermittent stretching is more effective in increasing ROM and changing the mechanical properties of the musculotendinous complex. - The Effects of Static Stretching On Dynamic Postural Control During Maximum Forward Leaning Task.

English, Scientific journal, The purpose of this study was to determine how the application of static stretching to ankle plantar flexors affects postural control during maximum forward leaning. Twenty-six volunteer males (age 21.4 ± 1.2 years) were randomly assigned to stretching and control conditions. Participants conducted 5-min stretching on a stretch board for the stretching condition and were kept standing for 6-min for the control condition. Before and after intervention, the range of motion (ROM) at ankle dorsiflexion and the center of pressure (COP) excursion during maximal forward leaning were determined. Mean anteroposterior COP position, COP velocity and COP areas were calculated to compare the change in postural control. After stretching, ROM was significantly increased. During maximal forward leaning after stretching, both COP position and velocity showed significant increases compared to before stretching. Moreover, COP position and velocity in the stretching condition were significantly higher than in the control condition after stretching. No significant differences were found in COP area before and after stretching. Five-minute stretching increased not only ROM but also the anterior limit of stability while maintaining posture and led to faster COP shift than before stretching. Fourteen healthy men performed 4 trials of differing stretch intensities: no stretching (control), 50, 75, and 100%. Stretch intensity was defined as maximum passive resistive torque predetermined at a familiarization trial. Each stretch trial consisted of 5 sets of 60-second CTS at the designated stretch intensity. Both maximal ROM and passive resistive torque were assessed during passive dorsiflexion, and MTU stiffness was calculated using the torque-angle curves measured before and after CTS. There were no significant differences in maximal ROM or MTU stiffness at the baseline condition. After the intervention, significantly greater maximal ROM and significantly lower MTU stiffness were observed in the 100% CTS condition than the control condition, whereas there were no significant differences between the submaximal intensity condition (i.e., 50 or 75% intensity) and the control condition. Therefore, our findings suggest that maximal intensity stretching is the most effective approach for improving both flexibility and MTU stiffness with CTS. - Warm-Up Intensity and Time Course Effects on Jump Performance.

English, Scientific journal, Jump performance is affected by warm-up intensity and body temperature, but the time course effects have not been thoroughly investigated. The purpose of this study was to investigate time course effects on jump performance after warm-up at different intensities. Nine male athletes (age: 20.9 ± 1.0 years; height: 1.75 ± 0.03 m; weight: 66.4 ± 6.3 kg; mean ± SD) volunteered for this study. The participants performed three warm-ups at different intensities: 15 min at 80% VO2 max, 15 min at 60% VO2 max, and no warm-up (control). After each warm-up, counter movement jump (CMJ) height, vastus lateralis temperature, heart rate and subjective fatigue level were measured at three intervals: immediately after warm-up, 10 min after, and 20 min after, respectively. Significant main effects and interactions were found for muscle temperature (intensity: p < 0.01, η2 p = 0.909; time: p < 0.01, η2 p = 0.898; interaction: p < 0.01, η2 p = 0.917). There was a significant increase of muscle temperature from the baseline after warm-up, which lasted for 20 min after warm-up with 80% VO2 max and 60% VO2 max (p < 0.01). Muscle temperature was significantly higher with warm-up at 80% VO2 max than other conditions (P < 0.01). Significant main effects and interactions for CMJ height were found (intensity: p < 0.01, η2 p = 0.762; time: p < 0.01, η2 p = 0.810; interaction: p < 0.01, η2 p = 0.696). Compared with the control conditions, CMJ height after 80% VO2 max and 60% VO2 max warm-ups were significantly higher (p < 0.01 and p < 0.05, respectively). CMJ height at 20 min after warm-up was significantly higher for 80% VO2 max warm-up than for 60% VO2 max warm-up (p < 0.01). However, CMJ height at 10 min after 60% VO2 max warm-up was not significantly different from the baseline (p < 0.05). These results showed that both high and moderate intensity warm-up can maintain an increase in muscle temperature for 20 min. スタティックストレッチング(static stretching以下SS)は,関節可動域の増加を目的に広く行なわれている.一方,近年SSが最大筋力を低下させる報告が数多くみられ,その機序に筋の力学的特性変化や神経学的要因の影響が推測されている.SSが筋の力学的特性に与える影響に関しては, stiffness低下がこれまで受動トルク計測や超音波撮影により報告されているが,これらの指標は非収縮時に計測されている.一方,SS実施による影響として,末梢神経への電気刺激によって生じるトルク(twitch torque)の低下から,筋の力生産能力低下が考えられている.そこで本研究は,SSによる伸張時間がtwitch torque,最大筋力に与える影響を調べた.
【方法】
対象は,健常若年男性14名(年齢22.5 ± 1.4歳)とした.SSの時間が異なる4条件1分 × 1(1分),1分 × 3(3分),1分 × 5(5分),Controlを同一被験者に対してランダムに実施した.SSはダイナモメーターを用いて右足関節底屈筋に対して行い,SS前後にtwitch torqueと最大筋力を評価した.Twitch torqueは安静時にM波導出に必要な強度の120%で脛骨神経の刺激により生じたトルク最大値とした.最大筋力は3秒間等尺性最大底屈時の最大値とした.測定はSS前後で各2回行い,その平均値を算出してSS前後の変化率を調べた.SS時間の長さによる影響を一元配置反復測定分散分析で検討し,最大筋力とtwitch torqueの変化率の関連をピアソンの積率相関係数で検討した.有意水準はp < 0.05に設定した.
【結果】
最大筋力は,5分 のSSで他の3条件と比較して有意な低下がみられた(p < 0.05).Twitch torqueは,3分のSS,5分のSSにおいて1分およびControl条件より有意な低下を認めた(p < 0.05).最大筋力とtwitch torqueそれぞれの変化率の関係では,5分のSSにおいてのみ有意な相関がみられた(r = 0.54; p < 0.05).
【結論(考察も含む)】
本研究の結果より,twitch torqueの低下は3分および5分のSS間で差がなかったことから,SSによる筋の力生産能力低下は計3分以上のSSでは定常となることが示された.一方で,最大筋力は5分のSSでのみ有意な低下がみられ,twitch torqueの低下との関連性が示された.したがって,3分のSSではtwitch torque低下による負の影響が神経学的活性により打ち消され,5分のSSで最大筋力に負の影響を与えていた.以上の結果から,筋の器質的な変化は3分のSSで生じ,最大筋力は5分のSSで生じるという用量反応関係の違いを呈していた.

Oct. 2016 - Oct. 2016, 【はじめに,目的】ストレッチングは,柔軟性改善を目的として臨床でよく行われている。近年,ストレッチングの効果検証には粘弾性の評価指標である筋腱複合体スティフネス(Musculotendinous stiffness:以下MTS)が用いられ,ストレッチング後のMTS低下や,MTSと指床間距離(FFD)など柔軟性評価指標との関連性が示されている。一方で,ストレッチング実施前のMTS値がストレッチング効果に異なる影響を与えるのではないかと考えられるが,MTSとストレッチング介入効果の関連性についてはあまりまだ検証されていない。そこで本研究は,足関節底屈筋筋腱複合体スティフネスとストレッチング効果との関連性を検証した。【方法】対象は整形外科的,神経学的疾患のない健常男子学生12名(年齢22.1±0.8歳)とし,対象筋は右足関節底屈筋とした。ROM,MTS計測は等速度性筋力測定機器(Biodex system 3)を用い,測定肢位は背臥位,膝関節完全伸展位とした。ストレッチング介入48時間前に底屈10 degから5 deg/secで最大背屈させ受動トルク,背屈ROMを測定し,ストレッチング介入の条件設定に用いた。介入直前,介入直後にも同様の測定を行い,前後のMTS,ROM増加率を算出した。MTSは,トルク-角度曲線における最終背屈角度から50%の角度での直線の傾きと定義した。ストレッチングは,事前に測定したROMの90%でのトルク値を用いて30秒×4セット合計2分,セット間は20秒間の休息とした。統計学的処理は,介入前後でのROM,MTSについて対応ありのt検定,MTSとROM増加率に対してピアソンの積率相関係数を算出した。有意水準は0.05未満とした。【結果】ストレッチング前後でのROMは,介入前32.9±6.1 deg,介入後37.2±6.2 degと有意な増加を認めた(p<0.05)。MTSは,介入前1.8±0.6 Nm/deg,介入後1.5±0.6 Nm/degで有意に低下した(p<0.05)。また,介入前のMTS値とROM増加率においては有意な負の相関が認められた(r=-0.75,p<0.05)。【結論】本研究結果より,2分間の足関節底屈筋へのストレッチングは,ROM増加とMTSを低下させたことから,柔軟性改善効果を示した。また,介入前のMTSが低値であるほどROM増加率も高かったこと,MTS高値群と低値群では伸張時の筋腱動態が異なる報告もみられることから,介入前のMTS値はストレッチング効果に影響を及ぼし得る因子であると考えられた。今後,柔軟性改善の持続効果や,ストレッチング時間を変更した際の柔軟性改善効果を合わせて検討していく必要があると思われた。 - ハムストリングスの柔軟性がSit and reachテスト時の骨盤傾斜および脊柱可動性に与える影響

Oct. 2016 - Oct. 2016, 【はじめに,目的】腰痛は日本人の約12%で罹患している(Yoshimura, 2014)。腰痛発生への関連因子として,ハムストリングス短縮(Adams,1999),骨盤傾斜の増加(Min,2013),腹筋群筋力低下(Nourbakhsh, 2002)などが挙げられている。一方腰痛患者を対象とした研究では,腰椎可動性の減少を示した報告はあるものの,ハムストリングス柔軟性や骨盤傾斜角度との関連性はまだ調べられていない。本研究は,ハムストリングス柔軟性の違いがSit and reachテスト時における骨盤傾斜および脊柱可動性へ与える影響を検討した。【方法】対象は,腰背部および下肢に整形外科的・神経学的疾患のない健常若年男性30名(年齢21.8±2.0歳,身長172.5±5.3cm)とした。ハムストリングス柔軟性はStraight leg raise(SLR)で調べ,被験者をLow SLR群(LS群)とHigh SLR群(HS群)に分けた。Sit and reachテストは,長座体前屈測定器(TOEILIGHT製)を用いて最大到達距離を測定した。胸椎・腰椎可動性と骨盤傾斜は,反射マーカーを第1胸椎棘突起(Th1),第1腰椎棘突起(L1),第1仙椎棘突起(S1),上前腸骨棘(ASIS),上後腸骨棘(PSIS)に貼付し(Vismarara,2010),デジタルカメラ(OLYMPUSμ-7020)で開始肢位と最大前屈位を撮影後,解析ソフトウェアImage J(NIH製)にて各角度を計測した。胸椎角はTh1-L1とL1-S1,腰椎角はL1-S1とASIS-PSIS,骨盤傾斜をASIS-PSISと水平線のそれぞれ成す角と定義し,開始肢位と最大前屈位での変化量を算出した。統計処理にはLeveneの検定を用い,胸椎・腰椎角および骨盤傾斜変化量の群間差を検討した。有意水準はp<0.05とした。【結果】SLR値は,LS群57.9±3.6°,HS群80.9±7.0°と両群間で差を認めた(p<0.001)。Sit and reachテスト時の腰椎角変化量は,LS群8.5±2.9°,HS群17.5±6.0°,骨盤傾斜変化量については,LS群11.9±4.4°,HS群20.2±4.2°と,HS群で有意に増大していた(ともにp<0.001)。胸椎角変化量については,LS群26.9±16.0°,HS群20.7±5.9°と差は認められなかった(p=0.168)。【結論】本研究結果より,ハムストリングス柔軟性とSit and reachテスト時における腰椎角および骨盤傾斜変化量で関係性が示された。一方で,胸椎角においては関係性を認めなかった。前屈動作時の脊柱可動性や骨盤傾斜評価の際には,ハムストリングス柔軟性に影響を受ける可能性を示唆する結果であった。 - 足関節底屈筋筋腱複合体スティフネスとストレッチング効果の関連性について
