    Scientific journal, Aging is a risk factor for severe COVID-19, characterized by vascular endothelial dysfunction. Although possible susceptibility of vascular endothelial cells (ECs) to SARS-CoV-2 infection has been suggested, the details of entry into cells have not been clarified. Previously, we reported that in an aged mouse model of severe COVID-19, ECs show a massive viral uptake and inflammatory response. Here, we focused on the endocytic capacity of senescent ECs. We found that the senescent ECs showed high endocytic capacity and SARS-CoV-2 virus uptake. This triggers an nuclear factor-kappa B (NF-κB) pathway–mediated inflammatory response. Further, Basigin enhanced endocytosis in the senescent ECs by activating the intracellular vascular endothelial growth factor signaling. Thus, EC senescence is associated with enhanced SARS-CoV-2 endocytosis and subsequent vascular endothelial dysfunction. This could prove a potential target for treating severe COVID-19 in older adults.

  • Hypothyroidism Revealed During General Anesthesia in a Patient With Treatment-Resistant Depression: A Case Report

    Scientific journal, General anesthesia in patients with undiagnosed hypothyroidism can lead to neurological, pulmonary, and cardiovascular complications. We report a case of hypothyroidism in a patient with treatment-resistant depression detected intraoperatively based on multiple clinical findings. A 49-year-old woman was scheduled for orthognathic surgery for mandibular prognathism. She had depression since the age of 36 and was taking multiple psychotropic medications. After induction, she had persistent hypotension, bradycardia, low bispectral index, and hypothermia that were resistant to treatment. After ruling out common causes and reducing the anesthetic agents, blood tests were performed intraoperatively to examine thyroid function which revealed hypothyroidism. No delayed recovery or postoperative abnormalities were observed. Lithium carbonate was identified as the most likely cause of hypothyroidism by an endocrinologist. Given the overlap in signs and symptoms, hypothyroidism may be overlooked in a patient with depression. The possibility of hypothyroidism should be considered especially noted in patients taking lithium carbonate. Furthermore, suspicion of hypothyroidism based on clinical findings during anesthesia may prompt the need for further evaluation.

    English, Scientific journal, Thrombosis is the major cause of death in severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, and the pathology of vascular endothelial cells (ECs) has received much attention. Although there is evidence of the infection of ECs in human autopsy tissues, their detailed pathophysiology remains unclear due to the lack of animal model to study it. We used a mouse-adapted SARS-CoV-2 virus strain in young and mid-aged mice. Only mid-aged mice developed fatal pneumonia with thrombosis. Pulmonary ECs were isolated from these infected mice and RNA-Seq was performed. The pulmonary EC transcriptome revealed that significantly higher levels of viral genes were detected in ECs from mid-aged mice with upregulation of viral response genes such as DDX58 and IRF7. In addition, the thrombogenesis-related genes encoding PLAT, PF4, F3 PAI-1, and P-selectin were upregulated. In addition, the inflammation-related molecules such as CXCL2 and CXCL10 were upregulated in the mid-aged ECs upon viral infection. Our mouse model demonstrated that SARS-CoV-2 virus entry into aged vascular ECs upregulated thrombogenesis and inflammation-related genes and led to fatal pneumonia with thrombosis. Current results of EC transcriptome showed that EC uptake virus and become thrombogenic by activating neutrophils and platelets in the aged mice, suggesting age-associated EC response as a novel finding in human severe COVID-19.
  • Suspected Accidental Infiltration of Rocuronium During General Anesthesia Induction: A Case Report.

    English, Scientific journal, There are few reports on rocuronium infiltration under general anesthesia. We report a case of suspected accidental rocuronium infiltration during anesthesia induction. A 25-year-old woman with autism spectrum disorder, intellectual disability, and epilepsy was scheduled for the extraction of 4 impacted third molars under general anesthesia. After induction with sevoflurane, an intravenous (IV) line was established in the left cephalic vein. Rocuronium was administered; however, subcutaneous swelling at the IV site was observed immediately. Spontaneous ventilations were maintained until additional rocuronium was administered via a new IV line. After heat pack application, the swelling disappeared 60 minutes after infiltration, and no tissue damage was observed. A strategy was developed to continue neuromuscular monitoring until recovery occurred. Acceleromyography was used, and the train-of-4 ratios at 99, 130, and 140 minutes after infiltration were 0.79, 0.91, and 1.0, respectively. Sugammadex was administered to prevent neuromuscular blockade recurrence. The patient was extubated once adequate return of muscle function and consciousness were observed. No neuromuscular block prolongation or recurrence were observed postoperatively. When rocuronium infiltration is suspected, it is important to eliminate swelling at the infiltration site and determine a management strategy based on neuromuscular monitoring.
  • Smith-Magenis症候群患者の歯科治療における全身麻酔管理経験

    Japanese, 症例は6歳9ヵ月女児で、重度の睡眠障害、行動異常を伴うSmith-Magenis症候群患者の多数歯う蝕に対し、全身麻酔下に保存修復処置、根管治療、抜歯など全顎的歯科治療が予定された。意思疎通困難で未知の事象に対する恐怖心が強い傾向にあり、衝動的な摂食行動を抑制すると自傷他害が起こる可能性があったため、前日入院時に周囲に飲食物がない環境を整え、当日は本患児と他患者の朝食の配膳を調整することで異常行動なく手術室に入室できた。麻酔導入・覚醒時の興奮に備えて、導入時の恐怖心を減らすための配慮とプロポフォールでの麻酔維持を行った。その結果、抑制の必要なく緩徐導入が可能となり、麻酔維持中は特記事項なく推移し、覚醒後も行動異常や危険行動を認めなかった。本症候群患者の周術期管理に際しては、術前に行動異常の有無とその内容を十分把握した上で、対策を講じることが重要である。
