  • Effects of Composite Resin Teeth Versus Porcelain Teeth in Complete Dentures on Oral Health-Related Quality of Life, Masticatory Function, and Patient Satisfaction: A Randomized Controlled Trial.

    English, Scientific journal, Background/Objectives: Artificial teeth in complete dentures are classified according to the materials used: porcelain (PO) or composite resin (CR). However, these materials' effects on function, patient satisfaction, and quality of life (QOL), as well as occlusal wear, remain unclear. We compared PO and CR complete dentures in edentulous patients by assessing masticatory function, patient satisfaction, and oral health-related QOL at 3, 6, and 12 months post-insertion, as well as occlusal surface morphology owing to material differences. Methods: In this open-label, randomized, single-center, parallel-group study, participants were edentulous patients who visited our hospital and underwent treatment with new complete dentures. The outcomes were oral health-related QOL; subjective satisfaction, assessed using a visual analog scale; and masticatory performance, evaluated with gummy jelly and were assessed at baseline and 3, 6, and 12 months post-denture insertion. Occlusal surface impressions were taken twice, digitized as STL models, superimposed, and analyzed for wear. The Wilcoxon rank-sum test was used to compare between groups. Results: All evaluated items showed improvement. However, no significant differences were observed between the PO and CR groups, including between the amount of wear observed in the two groups. However, the PO group showed a tendency toward less wear. Extended observation may be required to clarify the long-term effects of artificial tooth materials. Conclusions: In the short term, the artificial tooth material did not influence masticatory function, oral health-related QOL, or patient satisfaction.
  • Carbon nanohorn coating on titanium promotes M2 macrophage polarization.

    English, Scientific journal, This study investigated the effects of carbon nanohorns (CNHs) coated on titanium surfaces on macrophage responses. The CNHs were electrodeposited on titanium discs to create a uniform coating with a thickness of 0.2-0.3 µm. Analysis of macrophages cultured on CNH-modified titanium (CNH/Ti) revealed decreased production of pro-inflammatory cytokines (TNFα and IL6) and increased IL10 gene expression compared to unmodified titanium. Immunofluorescence analysis showed higher expression of the M2 marker CD206 and lower expression of the M1 marker CD86 on the CNH/Ti surfaces. Gene expression analysis indicated downregulation of genes involved in DNA transcription, repair, and replication, correlating with controlled cell proliferation on CNH/Ti. These findings demonstrate that CNH modification on titanium surfaces influences macrophage cytokine production and phenotype marker expression, suggesting its potential application in dental implant development.

  • Mechanical Effect of an Implant Under Denture Base in Implant-Supported Distal Free-End Removable Partial Dentures.

    English, Scientific journal, Background: In recent years, implant-assisted removable partial dentures (IARPDs) have been used clinically. However, the extent to which additional implants reduce the burden of supporting tissues is unclear. The aim of this study was therefore to investigate the influence of implanted IARPDs on stress sharing among supporting tissues, using finite element (FE) analysis. Methods: FE models were constructed based on the computed tomography (CT) of a patient with a unilateral defect of the mandibular premolars and molars and the surface data of an RPD with cuspids as abutments, designed using computer-aided design software. A titanium implant was placed in the area equivalent to the first premolar, second premolar, or first molar (IARPD4, IARPD5, and IARPD6, respectively). FE analysis was performed for laterally symmetrical models, i.e., bilateral distal free-end IARPDs. A vertical load of 200 N was applied to the central fossa of the artificial premolars or molars (L4, L5, or L6). Results: Equivalent stress in the alveolar mucosa and vertical displacement of the denture was smaller, with IARPDs under L5 and L6 loads, compared to RPDs. However, abutment teeth were displaced upward under an L6 load in the IARPD5 model. Conclusions: Within the limitations of this study, the area corresponding to the first molar was recommended as the location for an implant under the denture base of bilateral distal free-end IARPDs. Implants located in the area corresponding to the second premolar may apply non-physiological extrusion force on abutment teeth under the load on the artificial second molar.
  • Effect of masticatory movements on head and trunk sways, and sitting and foot pressure distributions during sitting position.

    English, Scientific journal, BACKGROUND: The head plays an important role in the postural control. Chewing co-activates jaw and neck muscles leading to coordinated jaw and head-neck movements. Therefore, to examine effect of masticatory movements on head and trunk sways, and sitting and foot pressure distributions during mastication is helpful in the attempt to understand the interrelationship between stomatognathic function and posture control system in the sitting position. OBJECTIVES: The purpose of this study was to test the hypothesis in healthy subjects that masticatory movements affect head and trunk sways and sitting and foot pressure distributions during sitting position. METHODS: A total of 30 healthy male subjects with an average age of 25.3  years (range, 22-32  years) were evaluated. The CONFORMat™ and MatScan™ system were used to analyse changes in sitting pressure distribution center of sitting pressure (COSP) and changes in foot pressure distribution center of foot pressure (COFP), respectively, and the three-dimensional motion analysis system was used to analyse changes in head and trunk postures while subjects remained sitting position with rest position, centric occlusion and chewing. The total trajectory length of COSP/COFP, COSP/COFP area, and head and trunk sway values were compared between the three conditions to evaluate whether masticatory movement affected the stability of head and trunk sways and sitting and foot pressure distributions. RESULTS: Total trajectory length of COSP and COSP area during chewing were significantly shorter and smaller respectively than it was in rest position and centric occlusion (p  ⟨  .016). Head sway value during chewing was significantly larger than it was in rest position and centric occlusion (p  ⟨  .016). CONCLUSION: Masticatory movements affect sitting pressure distribution and head movements during sitting position.
  • Effect of sitting posture with and without sole-ground contact on chewing stability and masticatory performance.

    English, Scientific journal, PURPOSE: To verify the effect of sitting posture with and without sole-ground contact on chewing stability and masticatory performance. METHODS: Thirty healthy subjects were evaluated. The Conformat was used to analyze the center of sitting pressure (COSP), and the three-dimensional motion analysis system was used to analyze changes in head and trunk postures while subjects remained in a sitting position with and without sole-ground contact. The parameters of masticatory performance and movement were calculated as follows. For evaluating masticatory performance, the amount of glucose extraction (AGE) during chewing of a gummy jelly was measured. For evaluating masticatory movements, the movement of the mandibular incisal point was recorded using the Motion Visi-Trainer V1, and parameters of the stabilities of movement path and rhythm were calculated. RESULTS: Head and trunk sway values and the displacement of COSP were significantly smaller with sole-ground contact than those without sole-ground contact. The masticatory movement path with sole-ground contact showed less variation in the opening distance and more stable movement path compared to those without sole-ground contact. The AGE was significantly greater with sole-ground contact than that without sole-ground contact. CONCLUSION: Sitting posture with and without sole-ground contact affects chewing stability and masticatory performance.

    健康寿命(2000年にWHOが提唱した「健康上の問題で 日常生活が制限されることなく生活できる期間」)は,男 性が約71歳,女性が約74歳(厚生労働省,2013年)であ り,平均寿命(男性:約80歳,女性:約86歳)との差が約 10年もあると報告されている.この差を解消し,健康寿命の延伸を図るために多くの試みが実施されている.厚生労働省の「健康日本21」(第2次:2013~2022年度)では,「健全な口腔機能を生涯にわたり維持することができるよう,疾病予防の観点から,歯周病予防,う蝕予防に着目するとともに,歯の喪失防止に加え,口腔機能の維持・向上について設定する」としている.健全な口腔機能を維持するためには,良好に咀嚼できることが必須である.また,健康やQOLに関する国民の意識レベルの向上に伴い,質の高い医療と治療効果を客観的に評価し,患者に呈示するエビデンスに基づいた医療が求められている. 平成23年から実施されていた先進医療技術「有床義歯補綴治療における総合的咬合・咀嚼機能検査」が,平成28年度診療報酬改定により,「有床義歯咀嚼機能検査」として保険導入された.本検査は,咀嚼時の下顎運動と咀嚼能力の検査を行うことにより,咬合と咀嚼機能の状態を総合的に評価するものである.これらの検査により,口腔機能,特に咀嚼機能について,治療前の障害程度,治療後の回復程度,および定期検査時の維持程度に関する客観的評価が可能となるため,患者の口腔の健康を維持・増進するために活用することができる. 北大病院歯科診療センターにおいても本検査の施設基準に係る届出が,厚生労働省北海道厚生局に受理されている.本稿においては,本検査法の概要とその臨床応用について紹介する., 北海道歯学会, Japanese

  • Effect of habitual chewing side on masticatory function

    Purpose: The purpose of this study was to elucidate the effect of the habitual chewing side on masticatory performance by examining the functional difference between chewing on the habitual and the non-habitual chewing sides. Methods: Ten asymptomatic subjects (ages 26-33 years, mean age 28.7 years) who participated in the 7th seminar of the Japanese Society of Stomatognathic Function were included in this study. The stability of masticatory path and rhythm and the patterns of masticatory movement path were compared between the habitual chewing side and the non-habitual chewing side during gum chewing. The glucose extraction as the indicator for masticatory function was compared between the two sides. Results: The values of the three indicators representing the stability of masticatory path and rhythm on the habitual chewing side were smaller compared to those on the non-habitual chewing side in 6 of 10 subjects. The glucose extraction on the habitual chewing side was significantly larger than that on the non-habitual chewing side. The majority of subjects, 8 of 10 subjects, showed Pattern I and Pattern III during chewing on the habitual chewing side. When the frequent pattern on the two sides was scored in order to evaluate quantitatively the stability of masticatory movement, the score for the habitual chewing side was larger than it for the non-habitual chewing side in 5 of 10 subjects, and the scores on the two sides were equal in 3 of 10 subjects. Conclusion: The present results suggested the following possibility: 1. Functional differences were found between chewing on the habitual chewing side and chewing on the non-habitual chewing side. Moreover, it was concluded that masticatory performance for chewing on the habitual chewing side was superior to it for chewing on the non-habitual chewing side. 2.     Patient: The patient was a 75 years edentulous male with the chief complaint of chewing difficulty. The anterior half of the maxillary alveolar ridge was flabby-gum tissue, and both maxillary and mandibular alveolar ridges inclined anteriorly. Mandibular position was deviated to the anterior right side, and the patient had a habit of unilateral mastication. Treatment dentures were used to improve mandibular position, unilateral mastication and masticatory function.
    Discussion: Some methods to arrange the artificial teeth were adopted in order to avoid vertical and horizontal dislodgement of dentures when new dentures were fabricated. Moreover, stability and satisfaction with new dentures were obtained by improvements of mandibular position and unilateral mastication.
    Conclusion: As a result of appropriate distribution of occlusal force by considering impression taking, registration of inter-occlusal relation and arrangement of artificial teeth, a favorable prognosis was obtained., Japan Prosthodontic Society, Japanese

    本解析法を用いて顎口腔系の機能評価を行う場合には,咀嚼側口角結節部標点と非咀嚼側頬部下方標点の2つが運動の円滑さの差を認めやすい標点であり,全ての標点を用いずとも,これらの2つの標点を用いれば顎口腔機能の評価を行えた.咀嚼側上唇部標点,正中上唇部標点,下唇正中部標点,非咀嚼側上唇部標点といった口唇周囲の標点は有意差を生じにくい標点であった.更に,本研究の解析項目の中で加速度のピーク数は最も感度の高い解析法であった, 北海道歯学会, Japanese

    我々はこれまでに,咀嚼運動は,立位と座位ともに姿勢の安定性を高める可能性があることを報告してきた.しかしながら,それぞれの姿勢において,咀嚼運動が姿勢制御にどのような影響を及ぼし,姿勢の安定化が図られるのかについては不明な点が残されていた.今年度は,立位における咀嚼運動が頭部動揺,体幹動揺,足底圧荷重分布に及ぼす影響とこれらの関連性について明らかにすることを目的とした.
    立位(自然直立姿勢)において,下顎安静位,咬頭嵌合位,グミ咀嚼時の各条件下,頭部動揺,体幹動揺,足底圧荷重分布の同時計測を行った.頭部動揺と体幹動揺の計測は,3次元画像解析システムを用いて,頭部と体幹にそれぞれ設定した4計測点の動作解析を行った.頭部動揺の安定性評価の定量的指標には,頭部に設定した4計測点の重心の平均座標を仮想中心として,仮想中心と各重心座標との平均距離を頭部動揺値と定義して用いた.体幹動揺の安定性評価についても同様の計算を行い,体幹動揺値と定義して用いた.足底圧荷重分布は足底圧分布測定システムを用いて計測した.足底圧荷重分布の安定性評価の定量的指標には,足底圧荷重中心移動の総軌跡長と面積を用いた.
    立位における頭部動揺値と体幹動揺値は,下顎安静位,咬頭嵌合位,グミ咀嚼の順に小さくなった.足底圧荷重中心移動の総軌跡長と面積は,下顎安静位,咬頭嵌合位,グミ咀嚼の順に,それぞれ短く,かつ小さくなった.立位におけるグミ咀嚼時には,頭部動揺と体幹動揺,頭部動揺と足底圧荷重分布,体幹動揺と足底圧荷重分布それぞれの間に正の相関が認められた.これらの結果から,立位では,下顎安静位,咬頭嵌合位を保持した時と比較して,グミ咀嚼時には,頭部動揺,体幹動揺,足底圧荷重分布がすべて小さくなり,それぞれの動揺が相関をもって姿勢の安定性に関係することが明らかとなった.

  • Elucidation of interrelationship and consequence between postural control, stomatognathic and body functions using biomechanical techniques

    初年度は,ヒトが咀嚼を行う一般的な姿勢である座位において,咀嚼が姿勢制御に影響を及ぼし,姿勢の安定性を高める可能性があることを明らかにした.一方,座位姿勢の変化が咀嚼能力に及ぼす影響についての報告は少なく,十分には解明されていなかった.顎口腔機能と姿勢制御の相互連関性を解明するうえで,「顎口腔機能が姿勢に及ぼす影響」のみならず,「姿勢の変化が顎口腔機能に及ぼす影響」についても検討することはきわめて有意義である.
    本年度は,姿勢の変化が咀嚼能力に及ぼす影響について明らかにすることを目的とした.足底接地の座位姿勢(両足底が床面に全面接地した端座位)と足底非接地の座位姿勢(両足底が床面から浮いた端座位)の2つの姿勢の条件下において,グミ咀嚼時における頭部運動,体幹運動,座圧荷重分布および咀嚼能力の測定を行った.その結果,1.足底接地の座位姿勢における頭部動揺値,体幹動揺値,ならびに平均中心変位は,足底非接地の座位姿勢時と比較してそれぞれ小さかった(p<0.05).2.足底接地の座位姿勢におけるグルコース溶出量は,足底非接地の座位姿勢時と比較して多かった(p<0.01).これらの結果から,咀嚼時の座位姿勢は,足底が接地しているほうが非接地と比較して安定していることが示された.また,足底接地の座位姿勢における咀嚼能力は,足底非接地の座位姿勢時と比較して高いことがわかった.
    以上のことから,足底接地の座位姿勢における咀嚼時には,足底非接地の座位姿勢時と比較して,姿勢の安定化が図られるとともに,高い咀嚼能力を発揮することが明らかとなった.すなわち,座位姿勢の変化は,咀嚼能力に影響を及ぼすことが示唆された.

  • Comprehensive approaches for the interrelationship between dental occlusion, masticatory function and body posture

    The purpose of this study was to examine the effect of masticatory movements on the postural control in sitting posture. When subjects masticated gummy jelly with sitting position, head sway during mastication was larger than it in the rest position and centric occlusion (p<0.05), trunk sway during mastication was smaller than it in the two mandibular positions (p<0.05), while fluctuations of the lower limbs during mastication was showing a tendency to increase compared to those in the two mandibular positions. There were found to have correlations in head and trunk sways, seat and plantar pressure distributions during mastication (p<0.05). These findings suggest that masticatory movements affect the postural control by enhancing the postural stability during sitting position, evoking the interrelationship between movements of head, trunk and lower limbs.

  • Comprehensive analysis of the interrelationship between dental occlusion, masticatory function and whole-body posture

    The purpose of this study was to evaluate to the effect of masticatory movements on head, trunk and body sways during standing position. When subjects maintained standing position with masticating chewing gum, head, trunk and body sways were significantly smaller than when they maintained their mandibles in the rest position and centric occlusion (p<0.05). No significant differences were found in the weight distribution between the 2 mandibular positions and masticating chewing gum. It was founded that masticating chewing gum affected the stability of head, trunk and body sways during standing position. These findings suggest that masticatory movements affect the postural control by enhancing the postural stability during standing position.

    We built the simultaneous measurements of body sway and head position, and those of the electromyographic (EMG) activity of craniocervical muscles and dental occlusion in order to establish the methodology for the quantitative evaluation elucidating the interrelationship between stomatognathic function and the general health of the body.   • MEMスペクトル解析による咀嚼機能および顎口腔機能評価

    まずは、現有システムのバージョンアップを図った。具体的には、現有パソコンにリアルタイム3次元座標計測ソフト(ライブラリー・Radish)を搭載することにより、3次元計測のリアルタイム化が可能となった。得られる計測値とこれまでの計測結果との精度検定を行ったところ、左右・上下方向成分で1/1000mm、前後方向成分で1/100mmオーダーの誤差が確認された。しかし、咀嚼運動解析結果の検証を行ったところ、これまでの報告と同様の結果が得られ、リアルタイム化に伴う影響は無かった。次に、咀嚼運動時の下顎運動と下顔面皮膚上の運動の時系列データに対して、MEMスペクトル解析(ジー・エム・エス・MemCalc/Win)を行った。1.下顎運動咀嚼運動のspectrumで最も大きなpowerを示すmaximum peakが出現する周波数は、これまでの時間領域における咀嚼周期の報告と比較検討した結果、ほぼ同じであった。このことから、maximum peakが出現する周波数は、咀嚼運動の最も基本的な周波数である咀嚼周期と考えられる。また、spectrumのpeakは、maximum peakの高周波域にみられたが、低周波域には認められなかった。2.下顔面皮膚上の運動下顎運動のspectrumとは異なり、maximum peakの低周波域と高周波域にたくさんの側帯域波が認められた。しかし、maximum...

  • 咀嚼運動時の下顔面皮膚表面の時空間的構造解析による顎口腔機能評価

    これまで、咀嚼時における下顔面皮膚上の動態解析を行い、その咀嚼運動評価法としての有効性を報告してきた。そして、健常有歯顎者の習慣性咀嚼側における咀嚼では、下顔面皮膚上の運動は下顎運動と協調し、運動周期は同じであるが運動パターンは測定点によって異なることを明らかにした。しかし、咀嚼運動の巧緻性は、健常者であれば非習慣性咀嚼側であっても存在すると考えられるが、それを咀嚼側間で比較した研究は極めて少なく、下顔面皮膚上の運動解析に至っては皆無である。そこで本研究では、個体内における咀嚼時下顔面皮膚上の運動巧緻性を咀嚼側間で比較することにより、その運動機能的差異を評価することとした。解析パラメータとして、時間的評価に咀嚼周期を、そして、空間的評価に測定点間の咀嚼経路の平均2乗誤差と平均差分ベクトルを用いた。咀嚼側間における各パラメータの有意差検定は、ウイルコクソン順位和検定(p<0.01)を行った。その結果、それぞれの咀嚼側における下顔面皮膚上の運動は、時間的に安定しており、かつ下顎運動と同じく正確なリズムで協調していることが分かった。空間的には、運動の経路と方向の類似性(運動パターン)が安定しており、咀嚼側間で鏡像関係にあることが分かった。さらに、咀嚼側間での運動安定性の評価を行ったところ、時間的に有意差はなく、空間的にも運動経路と方向の安定性に有意差はなかった。以上のことから、個体...
