英語, 研究論文(学術雑誌), Recent epidemiologic studies have shown that patients with age-related macular degeneration (AMD) have a considerably higher risk of developing Parkinson disease (PD) later in life, suggesting a possible link between these diseases. However, the common mechanisms between these two diseases remain obscure, although the pathophysiology of each has been well investigated. This study was designed to explore the shared pathologic features of AMD and PD by focusing on leucine-rich repeat kinase 2 (LRRK2) and α-synuclein, both of which play crucial roles in PD pathogenesis. Immunohistochemistry for LRRK2 and α-synuclein was performed on human eye specimens. The effect of LRRK2 on retinal pigment epithelium (RPE) cell function was investigated using the RPE cell line hTERT-RPE1. Retinal morphology and function were examined in LRRK2-G2019S transgenic mice, representing mutants with increased kinase activity of LRRK2. Immunohistochemistry revealed that LRRK2 and α-synuclein were present in the RPE layer of the human eye. Overexpression of LRRK2 in RPE cells increased α-synuclein and induced cell death. LRRK2 inhibited α-synuclein degradation via phosphorylation of RAB, member RAS oncogene GTPases. LRRK2-G2019S transgenic mice exhibited apoptosis of RPE and photoreceptors, choroidal thinning, and reduced electroretinogram amplitude, on top of α-synuclein protein accumulation in the RPE cell layer. Taken together, the current study revealed that LRRK2 is one of the key molecules involved in the common pathologic mechanisms of AMD and PD. - A Case of Metastatic Iris Tumor from Adenocarcinoma of the Cecum Diagnosed by Combined Tumor Markers in Aqueous Humor and Fine Needle Aspiration Cytology.

英語, 研究論文(学術雑誌), INTRODUCTION: The diagnosis of metastatic iris tumor is made by a fine-needle aspiration biopsy of part of the solid component of the tumor and tumor cells seeded in the anterior chamber; however, sometimes the diagnosis is not made due to insufficient sample volumes. We report a case of metastatic iris tumor in which measurement of tumor marker levels in the aqueous humor together with fine needle aspiration cytology contributed to the diagnosis. CASE REPORT: An 80-year-old Japanese woman was referred to our hospital because of an iris mass. Six months before the initial visit, the patient was diagnosed with adenocarcinoma of the cecum. Slit-lamp microscopy revealed an elevated iris mass. Fine-needle aspiration cytology suggested adenocarcinoma. The carcinoembryonic antigen (CEA) level in the anterior chamber was 5,716.1 ng/mL, and serum CEA level measured on the same day were 678.5 ng/mL. These data indicated CEA levels synchronously elevated in both anterior chamber and serum. Based on aspiration cytology and high CEA levels of anterior chamber, she was diagnosed with metastatic iris tumor derived from adenocarcinoma of the cecum. One month after the initial visit, right eye irradiation was performed for metastatic iris tumor. CONCLUSIONS: Measurement of tumor marker levels in the aqueous humor, together with fine needle aspiration cytology of the tumor, contributes to the diagnosis of metastatic iris tumors. - Aggressive Orbital Dedifferentiated Liposarcoma With Osseus Components in a Young Woman.

英語, 研究論文(学術雑誌), BACKGROUND/AIM: Dedifferentiated liposarcoma (DDLS) is a rare but aggressive subtype of liposarcoma that arises in soft tissues. In orbital liposarcoma, malignant transformation from well-differentiated to DDLS typically takes at least a year. We herein report a case of a young female with an aggressive DDLS, that developed from an orbital lipoma within one year. CASE REPORT: A 25-year-old woman presented with left upper eyelid swelling. MRI revealed abnormal adipose tissue in the medial orbit. The excised tissue was diagnosed as an orbital lipoma. The eyelid swelling recurred three months later, and the orbital tumor enlarged. An additional resection was performed, which again revealed a lipoma without malignant features. Three months later, the swelling worsened, and tumor resection confirmed well differentiated liposarcoma with a Ki-67 labeling index of 10%. Two months later, CT imaging depicted calcified lesions within the tumor. The patient subsequently underwent orbital exenteration followed by proton beam radiation. Histopathological examination of the exenterated tissue revealed DDLS with extensive osseous components. The Ki-67 labeling index exceeded 50%. CONCLUSION: Ocular oncologists should pay attention to the possibility of rapid malignant transformation in DDLS in primary orbital liposarcoma. - Lacrimal gland pleomorphic adenoma: two referral center analyses in Hokkaido, Japan.

英語, 研究論文(学術雑誌), AIM: To evaluate the clinical features of primary lacrimal gland pleomorphic adenoma (LGPA). METHODS: This was a 2-center, retrospective, observational study of primary LGPA patients who underwent tumor resection. Ophthalmic examinations and orbital computed tomography (CT) and/or magnetic resonance imaging (MRI) were performed. RESULTS: Totally 18 patients (10 males and 8 females, mean age 56.3±13.8y) were enrolled. Initial symptoms were unilateral proptosis in 8 patients, diplopia in 3 patients, and pressure sensation and no chief complaint in 2 patients each. The best-corrected visual acuity of the affected eye was 0.26±0.44 logMAR, and the intraocular pressure (IOP) of the affected and healthy eyes was 20.1±9.9 and 15.8±4.3 mm Hg, respectively. The difference in degree of proptosis between the affected and healthy eyes was 4.1±2.2 mm based on the Hertel ocular protrusion meter. One case had a history of breast cancer. Seventeen of 18 patients with an orbital lacrimal gland origin underwent total tumor excision by anterior/lateral orbitotomy. CONCLUSION: Orbital LGPA can complicate IOP elevation, and require total tumor excision by orbitotomy. In rare cases, systemic malignancy may complicate LGPA, and in such cases total removal of the lacrimal gland tumor should be considered at the initial surgery.
